VISION STATEMENTThe Florida Orthopaedic Society will be the

premier organization that promotes the highest quality musculoskeletal health for the citizens

of Florida

MISSION STATEMENT The Florida Orthopaedic Society will champion

the interests of musculoskeletal patients and orthopaedic surgeons in Florida.

ACCREDITATION STATEMENT This activity has been planned and implemented

in accordance with the accreditation requirements and policies of the Accreditation Council for

Continuing Medical Education (ACCME) through the joint providership of the American Academy of Orthopaedic Surgeons and the Florida Orthopaedic

Society. The American Academy of Orthopaedic Surgeons is accredited by the ACCME to provide

continuing medical education for physicians.

The annual FOS Vendor Appreciation Welcome Reception will be held the first

evening of the meeting, Thursday, May 19th from 6:00 to 7:00 pm

on Valencia Court at the hotel. All meeting attendees, vendors, and their families are encouraged to attend. Please RSVP so we have an accurate head count. This is an outdoor event so resort casual

Dr. Miguel E. Cabanela is an Emeritus Professor

of Orthopaedic Surgery at the Mayo Clinic in

Rochester, MN. He received his Medical Degree

(Summa cum laude) from the University of

Santiago Medical School in Spain. He completed

a Residency in Orthopaedic Surgery at the Mayo

Graduate School of Medicine in Rochester, MN.

Dr. Cabanela is the Executive Director of the

Mid-America Orthopaedic Association. He has

served in leadership roles in many professional

societies including serving as President in the

Hip Society, the International Hip Society, and

the Minnesota Orthopedic Society. He has also

served in numerous volunteer positions with the

American Academy of Orthopaedic Surgeons

including Chair of the Committee on International

Education and Relations and the Committee on

Injuries.

Dr. Cabanela currently serves as a Reviewer for

the Journal of Arthroplasty, Journal of Bone and

Joint Surgery, and Clinical Orthopaedics and

Related Research.

Keynote Speakers and Sessions

Doctor James R. Andrews is one of the

founding members of Andrews Sports

Medicine and Orthopaedic Center in

Birmingham, Alabama. He is also

co- founder of the American Sports

Medicine Institute (ASMI) a non-

profit institute dedicated to injury prevention,

education and research in orthopaedic and sports

medicine. Doctor Andrews continues to serve

as Chairman of the Board and Medical Director

of ASMI. Doctor Andrews is also a founding

partner and Medical Director of the Andrews

Institute in Gulf Breeze, Florida. He is President

and Chairman of the Board of the Andrews

Research and Education Foundation also

dedicated to prevention, education and research

at the Andrews Institute. He has mentored more

than 314 orthopaedic/sports medicine Fellows

and more than 84 primary care sports medicine

Fellows who have trained under him through

these Sports Medicine Fellowship Programs.

Doctor Andrews is known and recognized for

his skills as an orthopaedic surgeon as well as

his scientific and clinic research contributions

in knee, shoulder and elbow injury

prevention and treatment. In addition,

he has made major presentations

around the world, and has authored

numerous scientific articles and

books. He has recently written a book,

Any Given Monday, about prevention of youth

sports injuries and how to prevent them for

athletes, parents and coaches. The book is based

on his life in sports medicine and was released in

January 2013 by Simon & Schuster.

Doctor Andrews graduated from Louisiana

State University (LSU) in 1963, where he was

Southeastern Conference indoor and outdoor

pole vault champion. He completed LSU

School of Medicine in 1967 and completed his

orthopaedic residency at Tulane Medical School

in 1972. He had surgical fellowships in sports

medicine at the University of Virginia School of

Medicine in 1972 with Doctor Frank McCue, III

and at the University of Lyon, Lyon, France in

1972 with the late professor Albert Trillat, M.D.,

who is known as the Father of European Knee

Surgery.

Gregory A. Chaires has been Board

Certified by the Florida Bar as a

specialist in Health Law since 2001

and licensed to practice law in

Florida since 1992. He has extensive

experience representing health

care providers, medical group

practices, hospitals, and other

health care facilities. He routinely advises on

issues involving the federal Stark Self-Referral

and Anti-kickback statutes, the Florida Patient

Self-Referral Act, the Florida Anti-kickback

Statute, as well as the False Claims Act and fee

splitting prohibitions. Mr. Chaires provides

representation concerning Medicare/Medicaid

fraud and abuse investigations and prevention,

HIPAA privacy and security regulations,

reimbursement issues, compliance programs,

physician employment agreements and medical

staff privilege matters. Lastly, Mr. Chaires

routinely represents physicians and

other health care provides in

professional license investigations

and disciplinary actions before the

various medical quality assurance

boards such as the Florida Boards

of Medicine, Osteopathic Medicine,

Dentistry, Pharmacy, Chiropractic

and Nursing.

Prior to founding the firm, Mr. Chaires served the

State of Florida as Assistant General Counsel to

the Board of Medicine, as well as General Counsel

to the Boards of Nursing, Dentistry, Respiratory

Care and Opticianry through which he gained a

significant knowledge and understanding of the

regulatory process, invaluable in his assisting

clients. In addition Mr. Chaires was the legal

advisor to the Medicaid Fraud Control Unit for

the Southern District of Florida for the Florida

Office of the Attorney General.

Dr. Shaffer is the Medical Director

of the American Academy of

Orthopaedic Surgeons (AAOS).

He is located in the Washington,

DC office. As medical director,

Dr. Shaffer oversees the office

of government relations and the

department of research and scientific affairs.

Dr. Shaffer assumed his position with AAOS in

December 2013. He has a true passion for science

and evidence-based medicine, and has been

working in this area more than 15 years. Dr.

Shaffer believes “We are at a place in medicine

where healthcare has become very valuable and

quality-based and evidence-based medicine is in

the best interest of our patients.”

Specializing in the treatment and care of the

spine, and currently retired from the Navy,

Dr. Shaffer earned his medical degree from

the University of Michigan and joined the U.S.

Navy, completing an orthopaedic surgery

internship at the Naval Hospital Oakland & an

aerospace preventive medicine fellowship at the

Naval Aerospace Medical Institute in Pensacola,

FL. He served two years aboard USS America,

USS Independence, and USS Nimitz,

with tours in the Mediterranean and

Caribbean.

Dr. Shaffer later completed an

orthopaedic surgery residency at

the Naval Hospital in Portsmouth,

Va., followed by a tour at the Naval

Hospital in Yokosuka, Japan, where he received

the Navy Commendation Medal, and a Spine

Fellowship at the University of Iowa and Chief

of Orthopaedics at National Naval Hospital

Bethesda, Md.

Dr. Shaffer spent time in private practice at

Ventura Orthopaedic & Sports Medicine in

Ventura, Calif., before joining the Saint Louis

University School of Medicine’s Orthopaedic

Surgery Department, where he reestablished the

spine program in both orthopaedic and neurologic

surgery. He later established a spine practice at

the University of Kentucky in Lexington, Ky.,

where he served as professor, vice-chairman,

and orthopaedic residency coordinator. Later,

and prior to joining the AAOS, Dr. Shaffer’s

practice was part of the Northwest Iowa Bone,

Joint and Sports Surgeons, in Spencer, Iowa.

Jan Vest is the Chief Executive

Officer of Signature Medical Group,

Inc. (SMG), an independent multi-

specialty group of 130 physicians

in 17 specialties and over 850

employees in St. Louis, Kansas City

and rural Missouri. As an awardee

convener in the Center for Medicare and

Medicaid Innovation’s Bundled Payments for

Care Improvement program, SMG is leading

the largest orthopedic initiative in the country

designed to improve quality of care and reduce

cost in Medicare joint replacement patients.

The SMG program clinically integrates 2,000

orthopedic surgeons in 26 states and 60 cities

across the country. SMG is managing 45,000

total joints a year in the program. Also, through

a grant received from CMMI to participant in

the Strong Start for Mothers and Newborns

program, SMG has successfully

implemented new models of prenatal

care that have reduced premature

births in its population of Missouri

Medicaid women by almost 50

percent. He consults with a wide

range of healthcare organizations and

regularly presents to business organizations,

academic leaders, physician groups and

healthcare professionals throughout the country.

He is a former member of the Federal Reserve’s

Health Care Industry Council and is currently

a member of the boards of the American

Association of Orthopedic Executives, the

Midwest Healthcare Initiative and the Signature

Healthcare Foundation. Jan was selected as the

Medical Practice Executive of The Year 2015

by the Medical Group Practice Management

Association (MGMA).
