Acerflex (Nutricia) USE: Isoleucine-, leucine-, valine-free medical formula used in the dietary management of maple syrup urine disease (MSUD) in children over 1 year of age. Must be consumed in conjunction with natural protein, fluid and other nutrients in quantities prescribed by a physician or dietitian to meet protein and general nutrition requirements. Milk: Goat Milk (Poplar Hill, Meyenberg) USE: May be helpful when there is a cow’s milk sensitivity and/or intolerance or allergy to soy milk. The types of casein in goat milk, plus the evaporation process (in evaporated milk), render the milk more digestible and less allergenic. The fat has a high proportion of short chain and medium-chain fatty acids and thereby may be more digestible. The lactose content is slightly less (9 gm/ 8 oz) than cow’s milk (12 gm/8 oz). Milk Substitute: Rice Beverage USE: Beverage made from rice. There are significant nutritional differences between rice beverage and cow’s milk. Rice beverage contains a lot of carbohydrate and is low in protein. Cow’s milk has protein, fat, and carbohydrate. If a participant plans to purchase it on their own, suggest they purchase “fortified” or ‘enriched’ rice beverage. Rice beverages are not WIC-allowed because they do not meet the FDA’s standard of identity for milk. CAUTION: Not recommended for infants less than 1 year of age. While rice beverage may be used as a beverage, it should not be used as a substitute for milk. Rice beverage is low in protein. Products that are not fortified do not contain calcium and vitamin D.

Milk Substitute: Soy Beverage USE: Beverage made from soybeans. Lactose- and casein-free. Soy beverage might be used by persons who are lactose intolerant, vegetarians, or for religious reasons. Although many brands do not provide the amount of protein and nutrients that milk provides, the WIC-allowed soy beverages contain a nutrient profile comparable to milk. CAUTION: Not recommended for infants less than 1 year of age. In addition to inappropriate composition for infants, an estimated 20-50% of infants with cow’s milk allergy have an adverse reaction to soy beverage as well. Many toddlers later outgrow cow’s milk allergy, but are less likely to outgrow a soy allergy. Therefore some specialists do not recommend giving soy-based beverages to infants and toddlers with cow’s milk allergy (J. Joneja).
