Forms Online Update
Sunsetting the Old Classic Forms
Way Back in 2005
• When Forms Online was first introduced in 2005, it used an entirely different method of creating forms.
• 2005 was the Jurassic Age in the electronic world!
• January 1, 2010: Forms Online introduced a new forms creation system.
• July 1, 2010: A switch was flipped and the old-style forms were no longer available for new transactions.
• Since that time, we have maintained the old-style forms in the old transactions in which they were originally created.

Things Are About to Change
•In order to move forward with new servers and new software, the old classic forms must be taken down.
•If not, updates to Microsoft language will make these old-style forms unusable and inaccessible.
When Will the Change Happen?
At 5:00 p.m. on Thursday, September 20, 2012, all old-
style classic forms will be removed from Forms Online.
Only Forms Online users with transactions dated before July 1, 2010, will be impacted by this
change.
Who is Impacted?
What This Means to You
1. This will impact all transactions you created using old-style classic forms before July 1, 2010.
2. Remember: • Your transactions and the transaction data will
not be affected. • Only the forms and form-specific data will
disappear. • Transaction data will continue to be available
in Forms Online.
How You Can Prepare
• If you want back-up hard copies of these forms, print them before Thursday, September 20.
• Remember this applies only to transactions created before July 1, 2010 and only to the old-style classic forms.
• Everything else remains the same.
Thank You
Don’t forget that change is coming on Thursday,
September 20!
