1.Introduction

This manual explains how to use Congo Conflict Minerals System when you

register a Conflict Minerals survey report on our system.

Section 1502 of Dodd-Frank Wall Street Reform and Consumer Protection Act

established in July 2010 requires U.S. listed companies to report on the use of

conflict minerals (tantalum, tin, tungsten, and gold). We therefore need to

conduct a conflict minerals survey of all of our parts and materials suppliers,

including your company. This survey will cover all matters up to and including

the identification of which smelters are processing the minerals.

We would like to ask you for participation in this survey and register you

report on our system, Congo Conflict Minerals System, once you have

completed your survey.

Congo Conflict Minerals System is one of the modules of Procurement

Information System. You can see an outline of the System in the next page.

You will be using this system in the future when you are requested to provide

us with your company information, supply chain information, BCP

information in an emergency and Green Procurement information.

3.Before Using System 3-1． Submit an application for system use

Fill out a designated form and submit it to our person in charge of System, Procurement

Division.


ID and temporary password are sent to you within one week after the receipt of application.

3-3． Change the temporary password to formal

Validity of temporary password is two weeks, so change it immediately after receipt.

※ Adding a user, change, information about deleting application, direct application can be from on the Web.

①Move to the next screen by clicking “Register User Information” of the

menu on the left of the screen.

＊This screen is displayed only if the registration of your company information is

complete. So please register your company information first.

Information manager needs to appoint a proper person in User List to the person in

charge of Congo and provide its authority.

“Edit User Information” is displayed if you click “User ID” on the User List screen.

Be sure to set Authority first since all User ID is provided with Manager Authority in initial

settings.

1)User Edit screen appear if you click User ID link.

2)Check the box which you want to provide in order to set User Authority.

3)Be sure to click “Assignment of Authorization” to register as you set in

this screen. At the same time, notice E-mail is sent to the user whose

authority is changed.

Request List

Register the reply date you are planning for each combination of Report Name

and TOSHIBA Site Name.

① Select the Report Name and the TOSHIBA Site Name from the pull-

down.

*If a report relates to several TOSHIBA sites, register a date for each

site respectively.

② Click the “Reply Date” pull-down button to display a calendar.

③ Select a reply date from the calendar.

④ Click the “Register” button to complete registration.

(The "Register" button will be available after selecting a reply date.)

Click the “Select” button, and select the report to check.

You may register the same report as many times as necessary;

however, only the last registered report is saved.

① If you click the “Delete” button, the selected Report is deleted.

(The “Delete” button becomes enabled after selecting the report file .)

② If the Caution Statements are displayed in green , your cooperation with

correction of CMRT is appreciated, by referring to "8-5-2. If the Caution

statement is displayed" . You may register the report without the correction

of CMRT if you will.

③ Be sure to click the “Register” button to register Report when Report check is

completed.


8-5．Re-entry when errors occur

Error messages are displayed when a report file has not been checked successfully.

Correct the error, and then check the report again.

-Error messages are displayed. Correct the file accordingly.

-The report cannot be registered if an error message appears.

The Caution Statements are displayed if a report file includes "Non-CFS" or "No to

Question 7".

Your cooperation with correct of the report file is appreciated, by referring to

“8-5-3. CMRT Registration while Caution Statement is displayed“.

You may register the report while the Caution Statements are displayed if the

errors are not displayed.

If you want to register the CMRT without correction, please register “CFSE Answer”

by the deadline, after entering the initial screen to select "CFSE Registration/Check

Progress“ .

the “Register” button.

Check the CMRT format This message is displayed in one of the following cases.

･Make sure that you use the CMRT.

･Make sure that you have neither changed a sheet name nor have added/deleted a sheet of the template.

Check the revision of the CMRT Check the revision of the reply file. Use Revision6.1.

Check the version of Excel sheet If you create CMRT ver2.02 or later in Excel2007,our system cannot read the file. Please recreate it in Excel97-

2003 format.

Check the half-width alphanumeric of CMRT Full-width characters are entered in a field that accept only half-width alphanumeric. Check the field shown

below this message and correct it.

It should be noted that the alphabet with a letter symbol spelling "ç", "ã" and "ñ" are considered as a full-

width(double-byte) characters, so please use the half-width characters, such as "c","a" and "n".

Check the number of letters exceeding the limit The number of letters exceeded the limit. Check the field shown below this message and make sure that the

number of letters does not exceed the limit.

•If the answers to Question 1 are correct, please delete the answers to Questions 2 to 8 with regard to the metal.

•If the answers to Questions 2 to 8 are correct, please select "Yes" to Question 1 with regard to the metal.

• If you have selected "Yes" in Question 3, you cannot select "Unknown" in Question 5 because Question 5 is

drop-down of Smelter Look-up.

If the selection of Smelter Look-up "Smelter Not Listed" is correct, please fill in the smelter information such as

You have answered "No" in Question 5 and "Yes" in Question 7 in the Declaration sheet, but all the answers are

・With regard to the mine being grounded in "No" in Question 5, please select "No" or "Unknown" or leave

blank in the question (column P) of Smelter List sheet, if the minerals originate from recycled/scrap sources and

mine.

・Please select "Yes" in Question 5 in the Declaration sheet, if all minerals originate from recycled or scrap

sources.




Check the Product List A product that is not in the Request Product List is filled in the "Product or Item Number" on the "Product

List" sheet of the CMRT.

• Make sure that Report Name and Toshiba Site Name is correct.

• Delete the product that is not in the Request Product List.

Check the Product List The product List in the CMRT does not contain any product.

Check the 100% of the smelter’s feedstock originate

from recycled or scrap sources.

To the question (column P) "Does 100% of the smelter’s feedstock originate from recycled or scrap sources?"

in Smelter List sheet, please select "Yes"/"No"/"Unknown" from the drop-down or leave blank.

Inconsistency between Smelter Look-up of Declaration

and Smelter Names of Declaration

If you have not selected "Smelter Not Listed" in Smelter Look-up, You don't have to fill in the Smelter Names.

Check Mandatory field of Question E. If you have answered "Yes, using other format (describe)" in Question E in the Declaration sheet, You have to

fill Comments in Question E.

Check smelter ID Please enter the same Smelter ID Number in column A and F.

Check the "No to Question 7" Answer to Question 7(Tantalum) is "No".

Answer to Question 7(Tin) is "No".

Answer to Question 7(Gold) is "No".

Answer to Question 7(Tungsten) is "No".

Check the "No to Question 7" or "Non-CFS" Caution statement

This CMRT includes the following <Subjects to be considered>.

"CFSE Registration/Check Progress" button to open an answer screen, and check/answer the questions. At the

next login, select the "CFSE Registration/Check Progress" button on an initial screen, and answer the

conditions you select at ①.

Search Conditions Description

Report Name Select the survey names you want, then the list of designated products is displayed.

If you select blank, all survey names are displayed

Product or Item Number Enter the product or item number, then the list of designated products is displayed.

If you enter a part of letter or number of product or item number, a product

containing this letter or number is displayed.

Reply Date Select any from the Reply Date box: “Y“ or “N” or blank,

then the list of products relevant to the selection is displayed.

If you select blank, the list of all products is displayed.

Registered Report Select any from the Registered Report box: “Y“ or “N” or blank,

then the list of products relevant to the selection is displayed.


Toshiba Site Name Select any from the Toshiba site name box, then the list of products relevant to its

site is displayed.


CMRT File ID Of the products surveyed, only products that match the CMRT File ID that you

specify is displayed in the list.

You can specify more than one CMRT File ID if separated by commas.

Report Name Survey name is displayed.

TOSHIBA Site Name Name of Toshiba site, where the request is registered, is displayed.

Product or Item Number Product or Item Number is displayed.

Request Date Date of transmitting request is displayed.

Reply Date Scheduled reply date is displayed.

User ID(Register of Reply Date) User’s ID who register reply date schedule is displayed.

User Name(Register of Reply Date) User’s Name who register reply date schedule is displayed.

Report Registration Date Report registration date is displayed.

User ID(Register of Report) User’s ID who register report is displayed.

10-1-1. Search on Screen

You have two ways to go to the screen “CFSE Registration/Check Progress”

１）If you register CMRT while the Caution Statement is displayed, the CFSE

status relating the file ID of answered CMRT is displayed on an initial screen.

Refer to 8-5-3 CMRT Registration while Caution Statement is displayed.

２）If you select the “CFSE Registration/Check Progress” button.

You can check the contents of “CFSE Answer”.

① If you want to see the CFSE of smelter, click the “Smelter”

radio button . If you want to see the CFSE of Question 7,

click the “Question 7” radio button.

Number of Unanswered Questions： The number of

unanswered “CFSE Answer”.

Total Number： The number of answered and

unanswered “CFSE Answer” .

② If you want to add a condition to specify what you are

searching for, click here and select what you need.

Click the "Search" button if you don’t add the conditions.

10-2. Register CFSE Answer /CFSE Planned Date You have two ways to register the “CFSE Answer /CFSE Planned Date”.

Use one of two ways.

１）Direct Entry from WEB: Enter the answer on the screen

２）File Upload: Correct the file that you downloaded in EXCEL, and then upload

① Click the "CFSE Answer" column, and select the "CFSE Answer".

In the case of smelter X. CFSE is completed

Y. CFSE will be completed by the planned date

In the case of Question 7 A. All of the smelters have been identified, and CFSE is completed

B. All of the smelters will be identified, and CFSE will be completed by the planned date

② Click the "CFSE Planned Date" column, and select the "CFSE Planned Date".

Default date is automatically displayed if you click the entry field at the first time. You can clear the date if you click the

“×“ button.

③ Be sure to click the “Register” button to register the Report when the “CFSE Answer” is completed.
