Yourinformationisvaluable.Bysharingyouroutcomes,you’llhelptomakesureourcommunitycaresystemworksforyouandyourfamily.
At least once each year, you will be asked about your quality of life and your care experiences using a simple survey that should take less than ½ hour to complete. You may also be asked to help test new tools such as the neighbourhood exchange, or compassionate care coaching. This is completely free and will not affect access to the regular care you receive. This project has received clearance from the University of Windsor Research Ethics Board.
