What resources does AbbVie have in place to help me and my family?

• AbbVie’s Employee Assistance Program (EAP) provides confidential, professional counseling and referralservices, including short-term counseling (up to eight sessions) and information and referrals, at no cost to you. Assistance is available 24 hours a day, seven days a week. Call 855-809-2013 to speak with a program representative. All employees and eligible dependents can receive help. • Health Advocate can help AbbVie employees and their families locate in-network medical providers in a newarea or where you live. Call 866-799-2731 to speak with a representative. They can also assist with:

o Finding facilities that will be able to provide temporary assistanceo Transferring medical records and prescriptionso Helping get a short-term supply of medications if prescriptions have been losto Answering benefit and treatment questionso Helping with elderly parents

• Circles Concierge can assist AbbVie employees and their families in locating reputable service providers for firerestoration, rebuilding, junk removal, moving and storage, relocation, and obtaining basic daily services such as household supplies and groceries, etc. Contact Circles 24 hours a day by calling 866-433-8543 or email circlesconcierge@circles.com.

• AbbVie Savings Plan loans/hardship withdrawals can be considered for home repairs. For answers to yourspecific account questions, call the AbbVie Retirement Plans Service Center at 855-538-7796.

What about my medical benefits?If you have questions about your specific health care benefits, contact your health plan: Kaiser HMO at 800-464-4000 (CA only), Blue Cross and Blue Shield at 877-238-5951 or UnitedHealthcare at 888-249-6760. If you have other benefits questions, call the AbbVie Benefits Center at 844-729-2236.

How about prescription drugs?If you have Caremark coverage, Caremark can provide you with a short-term supply of critical medications during a crisis. For more information, contact Caremark Customer Care at 855-298-2488 and request an Emergency Override for your prescription.

How can I replace my glasses and contact lenses?If you or your family members are covered under AbbVie’s vision care benefit and are in need of eye care services, including replacing broken or lost eyewear, please call VSP Member Services at 800-877-7195.

I will miss my deadline to submit my application for tuition assistance. What can I do?Contact EdAssist to create a Virtual Help Desk ticket through “Contact Us” on the website tamsonline.org/abbvie or call 866-872-7391 and request an extension due to an individual hardship to be routed for approval.
