Welcome to the Florida Top Dog All-Stars! We are honored that you have selected our program and we welcome you to our Ohana! Our mission at Top Dog is to continue to strive to be one of the top competitive cheer programs in the country along with instilling positive characteristics that your child will take with him/her into his/her future. Our goal each day is to teach our athletes the value of commitment, teamwork, integrity, leadership, self-confidence and to allow them to create memories that will last a lifetime.

Teams at Top Dog will be formed based on ages as well as skills. Our staff will strive to put together the best possible competitive teams that will score well in the division. There may be athletes on any given team that tumble at a different level than the rest of the their teammates. We try to match up the levels the best we can, but please remember that stunting, jumps, pyramids, dance, motions, and age are a huge factor as well. We choose our teams based on the positions that we need to fill. Athletes should come prepared the first day with all the required paperwork, first installment payment and dressed in a Top Dog t-shirt, tank, etc. A Top Dog shirt should be worn each day of evaluations and female athletes must also wear a bow. Athletes will be evaluated in groups and be asked to demonstrate jumps, motions, tumbling, and stunting. Tryouts are closed to spectators except the 5-8 year old sessions.

LEVELS There are up to 6 competitive levels of teams that can be formed based on the amount of athletes that tryout. Within those levels, different age groups can be selected. Level 1 being the beginning level with level 5 being the most advanced level. These levels make evaluations less stressful as we are able to make as many teams as possible from beginners all the way to the more advanced athlete.

ALL-Star Prep In addition to our full year teams, we will also be fielding a mid-season team that will run from May-December. This will be our Power Pack Session 1 team. Please see specific info about this team in the packet.

Evaluations for our Power Pack Session 2 team will take place in December 2016.

Summer Absence Policy

During the month of May, athletes will be excused for end of the year school activities, graduations, etc. During the months of June - August 14, an athlete may only have 4 missed practices that will be allowed without doing make-up classes. After the 4th miss, an athlete must make up his/her absence in the gym by completing a class, private, open gym, etc. One hour in the gym = one practice missed. One 30 minute private = 1 practice missed. Classes are not included in gym fees this year and must be reserved online through our Just Tumblin’ portal. Monthly classes for Top Dog Athletes will be given at a discounted rate. Please note that athletes that have excessive summer absences may jeopardize his/her position on the team. Summer practices involve skill building and conditioning and when an athlete misses a lot of practices, he or she may fall behind. Cheerleaders must fill out an Absence Request Form (found on our website) when missing a practice.

Competition Dates: Our competition dates for full year teams run (December - April). Half-Season (Session 1) competition dates will run (November - December). The following dates, are only possible dates to keep open and not all teams will attend these events. Most of our events are local with very little travel. A final schedule will be released in August.

Competitive cheerleading is an expensive sport and families should carefully consider the costs involved before committing. Florida Top Dog has one of the best air-conditioned state-of-the-art facilities in the state. Our facility encourages progressive skill learning as athletes can safely accomplish new skills by using our in-ground apparatuses and variety of tumbling surfaces and equipment. We bring in some of the best instructors in the sport to work with our athletes throughout the season. Victor Rosario (owner of Top Gun) and Debbie Love (highly sought after tumbling coach and trainer) will instruct our team camps. Ryan Maag (head judge, choreographer and competition score sheet facilitator comes to our gym several times throughout the year to make sure our routines are hitting the score sheet. We are open and up front with our fees and have built in a monthly charge that covers most of the expenses throughout the year. All fees will be paid directly to Florida Top Dog All-Stars. In doing this, each cheerleader has the opportunity to raise 100% of his/her share of participation expenses. Athletes can raise their funds through sponsorships as well as fundraising offered. All funds turned in are considered sponsorship and donations to the Florida Top Dog All-Stars; therefore if a cheerleader quits or is dismissed from the team, no money will be refunded nor can money be requested to transfer to another cheerleader. Florida Top Dog All-Stars is a not-for-profit 501(c)3. Costs Twelve monthly Installments will include gym fee as well as all-star fees. Gym fees cover your child’s practice times and all-star fees cover competition fees, routine music, bow, gym registration, insurance, administration and coaches fees, end of the year awards, summer skills camps for levels 2-5 -- (Power Pack not included). A $50 tryout fee must be paid online in order to register to participate in the tryout selection. The first installment of $285 is paid the first day of tryouts. First installments will be deposited after the first practice. If a cheerleader does not make a team or chooses not to cheer, the first installment will be refunded as long as he or she has not attended practice. May payments will be applied as credits if applicable for certain levels. For families with more than one cheerleader involved in Top Dog, the following discounts apply for gym fees only: (2nd child -- $20 off) and (3rd child -- $40.00 off). These discounts will be adjusted in the installment.

All-Star Fees Do Not include:

• USASF Gym Membership: $30 • Uniform: $160-$350 -- (1/2 due September 1st and ½ due October 1st.) • Choreography: $100-$250 -- (1/2 due July 1st and ½ due August 1st.) • Hotel and Travel • Shoes • Warm-ups -- (optional) • Practice Attire • The Cheerleading WORLDS -- (for Level 5 teams) • The Summit -- (for Levels 2-4 & Restricted 5) • Make-up Kit

Level 5 Worlds teams may participate in multiple out of town events. Please be prepared to cover higher travel costs if your child is on a World’s team.

Uniform and Choreography is included in monthly all-star fees for Power Pack only.

Practice Attire ~ There will be a required practice outfit schedule. Levels 1-5 will be sized for practice wear to include 2 sublimated outfits, 2 competition/practice shirts (1 is a team shirt, 1 program shirt), and a team practice bow. Competition/Practice shirts will be available after summer.

Power Pack teams will purchase 2 competition shirts only as part of their required package. Any Top Dog team shorts can be worn with competition/practice shirts.

Full season teams will also be required to purchase one team outfit (team tank, shirt, etc.) along with matching team (shorts, capri's, or sweats). This outfit will be selected by the coaches of the team and will have the Top Dog name and team logo/name on it.

Practice times for each team will vary, based on skill levels. Level 1: ......... 2 practices per week ~ 1 hour or 1 hour and 15 minutes each practice. Level 2: ......... 2 practices per week ~ 1 hour and 30 minutes each practice. Level 3-4: ...... 2 practices per week ~ 2 hours each practice Level 5: ......... 2 practices per week ~ 2 hours each practice. One additional practice the week of a competition.

Additional tumbling/jump classes are NOT included in Gym fees. Monthly classes will be given at a discounted rate for Top Dog Athletes:

• $30 a month for 1 class per week -- (4 classes total) • $55 for 2 classes per week • $80 for 3 classes per week

Athletes must sign up online on our JUST TUMBLIN’ Portal and must pay at the front desk to get the monthly discounted rate.

Financial Information and Payment Policy

• You will be paying monthly dues that will be deducted from your credit/debit card at the beginning of each month. All team members must have a Credit Card Authorization Form on file for their account. This form will be due with the tryout packet and returned if the athlete does not make the team.

• All Top Dog fees will be billed on or by the 1st of each month. All fees must be paid in

full on or by the 1st of each month. On the 2nd of each month, any outstanding balances will be charged to your credit card.

• Once your child has been placed on a team, you must go online to your parent portal

and input your payment information. Select “manage payment options” and choose how you want your fees to be paid. Your credit/debit/e-check numbers will be stored and drafted on the 2nd of the month through your online account. Auto-pay is not saved on our computer nor is it saved within iClass Pro. It is saved by the Payment Gateway Program.

• Even though all families are required to be on Auto-pay, you may still make payments at

the front desk by using cash, money order, check or credit card. These payments must be made on or before the 2nd of the month. THIS INCLUDES CHEERBOOSTER PAYMENTS.

• Any payments resulting in NSF, declined credit card or expired credit card will incur a

$30 service charge. Return checks and fees must be paid in cash. It is your responsibility to update your information if your information changes.

• An athlete’s account must be current and in good standing in order to participate in

practices, private lessons, competitions, and special events.

• Florida Top Dog reserves the right to remove any athlete from his/her team for failure to keep up with financial obligations.

• All accounts must be current before an athlete may collect any clothing/uniforms, or

other retail items.

• If an athlete chooses to leave, or is asked to leave for any reason before the season is over, any and all funds are non-refundable.

• Gym fees and All-star fees cannot be pro-rated (with the exception of May gym fees)

and will not be adjusted during times the gym is closed, vacation days, practices cancelled, etc.

Our Parent Organized Fundraiser Committee will have several optional fundraisers to choose from that will benefit each cheerleader’s account. In order to protect both the parent and the organization, we asked that any money turned in for a fundraiser, be acknowledge through a receipt provided by the fundraiser chairman. Please do not turn money in without obtaining a receipt. If a parent chooses to participate in an optional fundraiser, the funds are the responsibility of the parent until turned in and a receipt obtained. Profits for fundraisers will only be dispersed to cheerleader’s accounts once all money is turned in. Each cheerleader will be REQUIRED to participate in our annual Silent Auction in order to offset the costs of our budget. All families are required to donate a gift basket valued at $40 or gift certificate valued at a minimum of $50.00 or pay a buyout of $50.00. Cheerleaders who choose to tryout after tryout dates, will be required to pay all back installments up front in order to bring their finances current with the rest of the team.

MAY-DECEMBER ~ Half-Season Team!!! Half-Season - "Power Pack" -- (Session 1)

Florida Top Dog will be offering a Half-Season Team Session 1. This competitive all-star prep season will run May-December. The team will compete at 3 one-day events. Athletes that participate on this team may finish their season in December or may chose to continue on with our Half-Season Team Session 2 that will run from December-April. Athletes that are in good standing from Session 1 will not need to participate in the tryout selection for Session 2 and will automatically be placed on a Session 2 team. The coaching staff will place the individual on the team that we feel will be the best fit for the athlete based on age and skill. Athletes that choose to continue with Session 2 will need to sign and agree to any policies with the Session 2 program as pricing and competition schedule will be different than Session 1.

All candidates trying out for full season teams may be considered for the all-star prep team as well. If you wish to only be considered for the Half-Season Team, then please be sure to mark this on your application. If you wish to tryout only for this team, then you may pay the (Half-Season Team Session 1) May installment of $133.00 instead of the full season May installment of $285.00.

Athletes that paid the $285 May installment and end up making the Half-Season Team will be credited the extra monies paid into their account.

Please see the special pricing for this team in the packet as well as the practice outfit requirements. Half-Season Teams will not attend skill camps. Practices will be twice a week for up to an hour and 30 minutes each practice. There will only be a few select Sunday afternoon practices.

• 3 events in addition to the Top Dog Parent Showcase at the gym (November 13). Events will be posted in August but will most likely take place in November and early December.

1. Attendance is crucial to team success. An absent request form is available online and must be filled out prior to the absence. Absences and tardies will be noted and reviewed on a regular basis. As of August 15, 2016 (Summer Absence Policy is over), a cheerleader who accumulates 5 unexcused absences may be dismissed from the team. A cheerleader with more than 10 absences (excused and unexcused combined) may be dismissed from the team (This does not include summer misses.). Extreme circumstances such as injury may be taken into consideration.

Excused reasons for being absent: a. Death in the family. b. An Illness that is so severe that it prevents the cheerleader from being at practice and

watching to see if any changes are being made to the routine and his/her position. Parents must call the coaches at home or at Just Tumblin’ before the scheduled activity to notify of absence.

c. Special circumstances that may be approved by the coach. d. Religious Holidays.

Unexcused reasons for being absent:

a. Lessons of any kind (piano, dance, etc.) b. Doctor or dentist appointments, unless it is an emergency. Appointments should be

scheduled so they will not conflict with cheerleading activities. c. Jobs. If a cheerleader takes on the responsibility of a job, it must be scheduled so that it will

not conflict with cheerleading duties. d. Family outings or vacations after the schedule has been set. This includes holidays. e. Concerts f. School activities not classified under Special Circumstances. g. Involvement in other sports or teams. h. Don't have a ride. i. Have too much homework, need to study for a test, etc. j. Withholding your child from practice as a form of discipline.

2. Tardies—If a cheerleader is more than 10 minutes late to a practice or leaves more than10 minutes early from a practice, it is considered an unexcused tardy. Three unexcused tardies will be counted as one unexcused absence. It will be at the coach’s discretion as to whether the tardy is considered excused or unexcused. If a cheerleader is absent for more than half of the practice, it is counted as an absence. Please see reasons for excused and unexcused absences. 3. Cheerleaders may participate on school and recreation teams with the understanding that all Top Dog practices, competitions, and activities take first priority. Top Dog practices missed due to a school or rec. squad conflict will be UNEXCUSED. Cheerleaders will be allowed to compete with their high schools as long as they compete within the guidelines that have been set for Pinellas County Schools. If a cheerleader misses a Top Dog competition due to a school cheerleading competition, she/he will be removed from the team. Cheerleaders will not be allowed to compete on another all-star team during the Top Dog Cheer Season.

4. Cheerleaders may not take any absences one week before the first competition of the season and an absence one practice before any other competition. If he/she misses a practice one week prior to the competition, then he/she will have to sit out of one competition. A cheerleader who is ill should still come to practice. The coach can send him/her home if illness is severe. Extreme circumstances will be determined by the coaching staff. 5. Practices will be planned depending on the needs of the squad. If necessary, additional practices, and even Sunday practices, may be scheduled to prepare for competitions. (There is no extra gym charge for additional practices or hours.) Coaches may schedule practices for everyday of the week one week prior to a large competition if a team is in need of extra practice. 6. Encourage good sportsmanship by being an example. Be positive in communicating with your teammates. We will not tolerate bad mouthing other all-star programs , teammates, coaches etc. from our athletes and/or parents. 7. No jewelry should be worn during practices and competitions. Cell phones should be put away. 8. We are not responsible for lost or stolen items. 9. Top Dog practice wear must be worn for all practices. A schedule of what to wear on certain days will be posted on the website each month. Cheerleaders who do not have Top Dog attire on, will have conditioning. Items that are lost or damaged will need to be replaced at the cheerleader’s expense. Shorts or sweats should not be rolled more than one time. Female cheerleaders need to wear bloomers or spandex shorts under their practice shorts. Cheerleaders are required to wear a bow for each practice. 10. Cheerleaders are required to stay with the team before, during, and after each competition until properly dismissed by the coaches. This includes overnight trips and practices. If a parent chooses to pull a child from practice or an event without the permission from the coach, it will be interpreted that the parent is pulling the cheerleader off the team and will have the same consequences as when a cheerleader quits. Cheerleaders must abide by Top Dog rules until dismissed by their coaches. 11. Due to the fact that some competitions require a day or two for travel, cheerleaders may need to miss a few days of school in order to travel with the team. Cheerleaders are responsible for notifying their school and obtaining schoolwork from their teachers. Please do not schedule travel time that will conflict with practices. It is not optional for a cheerleader to choose whether or not he or she will compete at an event. 12. Parents are required to provide transportation and lodging if applicable to all competitions. Parents need to provide proper supervision for their athlete. 13. A cheerleader is a leader and should be an example to all. If a cheerleader is found to have been drinking, smoking, using drugs, stealing, lying, using foul language, or involved in any immoral behavior upon arrival or during any Top Dog activity, it will be grounds for dismissal or suspension from the team at the coaches’ discretion.

14. All cheerleaders must maintain a 2.0 grade point average. Education is very important and is one of the many ways of being a good example to others and upholding high standards for the team. 15. If a cheerleader becomes injured or severely ill, and is not capable of performing his/her duties prior to a competition, he or she may be replaced by another cheerleader. The participate pass or ticket is then given to the athlete that is replacing the injured cheerleader. If the injured cheerleader wants to still attend the event, then he or she must purchase a spectator pass into the competition. Once the injured athlete is cleared by the doctor to return to activities, it will be up to the TD staff to determine if the athlete is ready to compete. If a cheerleader misses a competition for any reason, the cheerleader will be replaced with another cheerleader. 16. All funds turned in are considered sponsorship and donations to the Florida Top Dog All-Stars; therefore if a cheerleader quits or is dismissed from the team, no money will be refunded nor can money be requested to transfer to another cheerleader. 17. If a cheerleader quits or is dismissed from the team, he or she relinquishes the right to any items that have not been distributed. Special order items bought through the Cheer shop will need to be picked up within 30 days of delivery date. After 30 days, the item will be put back on the shelf to sell and money will not be refunded.

18. The coaches maintain the right to dismiss or suspend a cheerleader at any time for breech of the Top Dog Policies and/or Cheerleading Contract. In the best interest of each team, the coach has the right to suspend or dismiss a cheerleader at any time if he or she feels that a cheerleader’s behavior or actions have brought discord and/or dissention among the team. Please see items #20 and #21 for policies regarding funds and Top Dog items. 19. Once a cheerleader quits a team or is dismissed, he/she will not be able to allowed to rejoin the team for the remainder of the season. If a parent chooses to pull a child from a team for academic or disciplinary reasons, the cheerleader will not be able to rejoin the team for the remainder of the season. Extreme circumstances will be determined by the staff. 20. Cheerleaders must refrain from using social media for voicing frustrations and complaints regarding their teammates, coaches, the program, etc. The cheerleader should not engage in negative banter between other all-star programs. All posts should uphold the positive standards that Top Dog requires of their cheerleaders. Positive standards include refraining from using foul language, as well profanity abbreviations, and posting photos that are inappropriate. 21. Most of our communication is done through email and through team facebook pages. Please check this regularly. 22. You cannot create or sell your own Top Dog clothing using the Florida Top Dog Name, Logos, or specially designed team logos. 23. Due to the busy nature of our facility and programs, we ask that athletes do not arrive more than 15 minutes prior to their scheduled practice/class time unless they have permission from their coach to come in early to work on necessary skills for their routine.

Top Dog Parent Commitment

My son/daughter ________________________is trying out for the Florida Top Dog All-Star Program. If he/she makes a team, I commit myself to the following:

o I will provide transportation to and from practices, fundraisers, and other activities. I will provide transportation and/or lodging for competitions.

o I will call the coaches when my son/daughter will be absent from a cheerleading activity. o I will schedule all family vacations so they will not conflict with the cheerleading activities having been

given the cheerleading schedule in advance. I understand the importance of this sacrificial commitment and how it affects the entire team.

o I understand that my actions could affect my child’s position in the Top Dog program including removal if I do not abide by the parent commitment contract that I have signed.

As legal guardians of ________________________________, I hereby consent to the aforementioned person participating in Terrick Training, Inc., d/b/a Just Tumblin’(hereafter referred to as “Just Tumblin’ “), programs and/or Florida Top Dog All-Stars, Inc. team events, including, but not limited to, competition and practice, utilizing the Just Tumblin’ facilities. I recognize that potentially severe injuries can occur in any activity involving height and/or motion, including tumbling, stunting, jumping, cheerleading, and other related activities, including trampoline and dance. It is recommended that a physical be performed by a medical doctor prior to participation in the aforementioned enumerated activities for the child’s benefit. By virtue of the execution of this document, I hereby affirmatively state that my child is physically able to participate in any and all Just Tumblin' and/or Florida Top Dog All-Stars, Inc. activities.

In consideration for allowing my child to use the facilities identified as the Just Tumblin’ gym, as well other locations where cheerleading practice, competition, exhibitions and any other use, as designated by Just Tumblin’ or Florida Top Dog All-Stars, Inc., may occur, I hereby forever release Just Tumblin’ and/or Florida Top Dog All-Stars, Inc., a not-for-profit entity, its employees, instructors, coaches, agents, and any and all other volunteers, from any and all liability for any and all damages and injuries suffered by my child while under the instruction, supervision, or control, direct or indirect, of Just Tumblin’ and/or Florida Top Dog all-Stars, Inc., its employees, instructors, coaches, agents, and any and all other volunteers, whether caused by their own negligence or otherwise.

As legal guardian of the aforementioned person, I hereby acknowledge that it is my sole responsibility to provide for any medical expenses or other attendant costs which may be incurred by my child as a result of any injury sustained while under the instruction of Just Tumblin’ and/or Florida Top Dog All-Stars, Inc., its employees, instructors, coaches, agents, and any and all volunteers. In recognition of this Acknowledgement of Risk, Waiver of Liability, Indemnification, and Consent, I further agree to indemnify Just Tumblin’ and/or Florida Top Dog All-Stars, Inc., its employees, instructors, coaches, agents, and any and all other volunteers, from any claim for damage or injury of said child, or by said child, independently, upon reaching the age of majority, including reasonable attorney’s fees incurred in defending any such claim.

This Acknowledgement of Risk, Waiver of Liability, Indemnification, and Consent, having been read and understood completely, is signed voluntarily as to its consent and intent. This Acknowledgement of Risk, Waiver of Liability, Indemnification, and Consent shall continue in full force and effect until such time as revoked, in writing, by the undersigned, without limitation as to its duration, as it is acknowledged and understood that my child shall participate in cheerleading and/or tumbling activities, including, but not limited to, practice, exhibitions, and other related activities, on an on-going basis.

The undersigned further acknowledge and agree that Just Tumblin’ and/or Florida Top Dog All-Stars, Inc. shall be granted specific permission to use the image and/or likeness of the aforementioned child for any purpose as it deemed applicable by Just Tumblin’ and/or Florida Top Dog All-Stars, Inc., including photographs and/or videotape, and to further use said child’s name, face, likeness, voice, and appearance in connection with exhibits, publicity, advertising, and promotional materials without the reservation or limitation as to the duration of the consent.

The undersigned expressly agrees that this Acknowledgement of Risk, Waiver of Liability, Indemnification, and Consent is

intended to be as broad and inclusive as permitted by the laws of the State of Florida, and that if any portion of this document is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. The undersigned further acknowledges that he/she is authorized to execute this Acknowledgement of Risk, Waiver of Liability, Indemnification, and Consent on behalf of the child and the parents, guardian, legal representative, assigns, heirs, and next of kin.

I authorize Just Tumblin’ and/or Florida Top Dog All-Stars to retain and rely upon any document executed including but not

limited to the Release in electronic form and shall not challenge its authenticity in the event the original is destroyed after electronic preservation.
