SEXUAL HARASSMENT POLICY: It is the policy of Meridian Community College, in keeping with efforts to establish an environment in which the dignity and worth of all members of the institutional community are respected, that sexual harassment of students and employees at Meridian Community College is unacceptable conduct and will not be tolerated. Any unwelcome behavior of a sexual nature is defined as sexual

harassment. The College’s sexual harassment policy and guidelines outlining procedures for filing a

complaint are located in the Office of the Dean of Student Services.

GRIEVANCE POLICY: A “Student Grievance Process” provides students with a procedure to resolve grievances, complaints or concerns in an equitable, orderly and timely manner can be found in the on-line catalog or in the office of Mrs. Soraya Welden, Dean of Student Services. For grade appeals, refer to the “Grade Review Policy” as summarized in the current MCC Catalog or in the college’s on-line catalog.

ATTENDANCE POLICY: Meridian Community College believes strongly that the single greatest predictor of

student success is attendance in class. Students are expected to attend class regularly. Specific attendance requirements for each course are determined and enforced by the instructor and vary from instructor to instructor. Information concerning attendance may be found in the current MCC Catalog.

Students will be dropped for:

1. Non-attendance if they do not login and complete the weekly attendance assignment for 2

weeks in a row.

2. Missing two homework assignments or quizzes.

3. If they receive two zeroes on homework assignments and quizzes.

4. If tests (proctored or unproctored) are not completed on or before the deadline dates,

unless a doctor’s excuse brought to the instructor during office hours within 7 days of the

scheduled test.

Posting an introduction on the Discussion Board is the first assignment and a zero will be

recorded if the student does not participate within the posted deadline.

****Once two assignments are missed the student will be dropped from the class. This

includes quizzes, discussion board and assignments. If a test (proctored or unproctored) is

not taken by deadline date the student will be dropped.

***REOPENING TEST: A quiz/test will only be reopened once. If a student needs a test / quiz

reopened more than once because they have been “kicked out”, the student will be required

to come and take the quiz/test on campus proctored at the instructors convenience within 48

hours. If I am not notified within 48 hours a grade of 0 will be recorded. The instructor will only

reopen a maximum of 2 quizzes/ test. If the instructor has already reopened a maximum of

two quizzes/tests during the semester then the student will be required to come to campus

and take all quizzes/ test proctored. Guests, including children, should not be brought into the testing site.

CELL PHONES AND PAGERS: Because of the noise distraction during class, use of these devices will not be allowed in the classroom. Any questions with this policy should be discussed with the instructor. No food or drink of any kind is allowed in the classroom.

POLICY ON STUDENT DUE PROCESS PROCEDURES, DISCIPLINE, DRUG AND CLASSROOM

BEHAVIOR: All students are expected to read and understand the College’s statement on student rights and responsibilities, discipline, drug use, and classroom behavior. These statements are found in the MCC Catalog.

STUDENT IDENTIFICATION POLICY: Students must display a current MCC ID card on his/her person at all times while on campus. ID cards are provided and validated by the Campus Police office, located on the first floor of Ivy Hall. The ID card must be worn on the person above the waist, which means it can not be on a purse, book bag, etc. Individuals without an ID card in plain view will be stopped to verify their status as a student, staff, or authorized visitor. Displaying of the ID is also required for use of the MCC library and most other services and activities. Individuals who fail to display the identification card may be subject to sanctions and penalties including charges of trespassing and/or suspension from school.

CONDUCT ON CAMPUS MCC strives to preserve a learning environment on the campus which is free from disturbances and disruptions. Students are expected to exhibit self-reliant, acceptable and desirable behavior. The guiding principles to meet these expectations include respecting the rights of others, being truthful, not participating in unacceptable behavior, or violating federal, state and local laws and ordinances. The following code is applicable to any facility owned or operated by MCC.

FINE IMPOSED UNACCEPTABLE BEHAVIOR The purpose of this section is to outline different situations and types of unacceptable behavior where a fine will be imposed. The following will be subject to a fine of $25 for the first offense, $50 for the second offense, $100 for the third offense and disciplinary action including, but not limited to, probation or suspension.

1. Public profanity, cursing or vulgarity 2. Smoking near any building entrance or smoking outside of designated smoking areas 3. Failure, refusal or inability to present a current MCC ID card upon request (must wear on visible location

of one’s person at all times when on College property) 4. Unauthorized entry to or use of College facilities 5. Unusually loud noise which creates a public disturbance including, but not limited to, shouting, use of

portable radios, use of automobile radios, use of laptops or musical instruments

TOBACCO POLICY Meridian Community College has specific guidelines for the use of tobacco products on campus. MCC prohibits tobacco use inside any facility owned or operated by MCC. The use of tobacco products is limited to designated

areas. See the campus map for the location of all designated tobacco usage areas. Smoking near any building

entrance is prohibited. This policy applies to all students, faculty, staff and visitors. Violators will face a fine and

could face further disciplinary action.

TUITION/FEE REFUND POLICY It is the student's responsibility to officially withdraw from any and all courses.

Refunds of tuition and fees are computed according to the date the student personally initiates the withdrawal process, not the last date of class attendance.

Requests for exceptions because of extenuating circumstances must be submitted in writing to Business Office within 90 days after the end of the semester of withdrawal. Any exception to this policy must be approved by an administrative committee.

The college will not refund fees originally paid by check until the check has successfully cleared the student's bank.

Prior to the first class day, a 100% refund will be given.

Withdrawal Procedures

It is the student's responsibility to officially withdraw from any and all courses.

A student can request to be withdrawn from courses by completing the following steps: 1. Log on to Eaglenet and click on the “My Student Services” tab 2. Select the “Registration” tab 3. Select the “Request to be Dropped from a Class” link 4. Select the course or courses to be dropped 5. Select “Update Drop Request”

Once the request is submitted, the instructor will record the last date of class attendance and grade of “W” will be recorded for the class(es) if submitted by the withdrawal deadline.

Refer to the Academic Calendar in the online catalog at www.meridiancc.edu for withdrawal deadlines.

Make sure to review the attendance policy for each course, each class attendance policy still applies.

REQUIRED COURSEWORK

Quizzes: There will be a quiz for chapters 2-15. The quizzes are not proctored and may be taken at home on your computer. These quizzes will be timed. Points will be deducted for exceeding the time limit. The deduction will be 2 points per minute. The quizzes will be added together and divided by the total number of quizzes required to get your average grade for the quiz. This will then count as 10% of your grade. If you get kicked out of a quiz I will reopen it for you one time during the semester. If you get kicked out again then you will have to schedule an appointment with me within 48 hours to take the quiz proctored or to notify me of the reason why you are unable to take the quiz at the scheduled time. If you do not notify me I will assign you a zero (0).

EVALUATION OF STUDENT LEARNING: There will be 6 unit tests and 14 quizzes. Students are expected to take each quiz and each test. The questions for all tests and quizzes will be taken from material covered in the textbook Tests will be structured for the student to only be allowed one attempt to take it and each unproctored test will be timed. The correct answers will not be made available at the time the student takes the test. There will be two proctored tests where the student will be responsible for scheduling an appointment with the

distance learning lab to take these. ATTENTION: It is your responsibility to follow the due dates for quizzes and tests. You are provided this schedule of dates from the beginning of the course; therefore you will not be allowed to make up any quizzes/tests missed. These will be calculated as zeroes and averaged into your overall grade.

Special Notes: If you are taking an unproctored quiz or test and are “booted off” the quiz or test will only be reopend once. If you get “booted off” again you will be required to come complete the quiz/test in a proctored setting. If this happens you need to contact me ASAP by e-mail to make arrangements to finish this assignment. If you have not contacted me within 48 hours and made arrangements you will be given the grade that you have earned for the incomplete exam. Further, throughout the semester I will keep a log of students who encounter these problems. If you are booted off of two assignments (at any point throughout semester) you will be required to take the remainder of your quizzes/tests in a proctored setting. Please make sure you have a secure internet connection before beginning an assignment for this class. If the student shall experience a problem with unproctored testing the student shall call E-Learning Center at (601)-484-8669. For proctored test the student must schedule to take the test in the E-Learning Center. The student shall sign on Blackboard and go to the link that states schedule testing and type in the appropriate information to schedule a test.

Material to be covered throughout semester: We will be coving a large amount of material. You cannot afford to get behind. There are clear deadlines. You will be allowed an adequate amount of time to complete each assignment as long as you follow the schedule. You are to take each quiz/test by the deadline listed. There will be no exceptions. Please do not request any extensions, as they will not be granted. This includes missing quizzes/tests because you failed to refer back to your schedule for this class. It is your responsibility to be aware of deadlines for all assignments.

TESTING CENTER: Guests, including children, are not allowed in the testing center. Pagers and cell phones are not allowed in the testing center. Testing center employees may report any

student with a cell phone that is visible during testing. Any questions regarding this policy should be discussed with the instructor.

No food or drink of any kind is allowed in the testing center.

SPECIFIC COURSE POLICIES

COURSE FORMAT: The course is organized according to body systems. You will find supplemental reading/study material in Course Documents in your course site, which will provide more instruction in the terms associated with the body systems. Your class textbook AND the practice CD that accompanies the book should be your main study resources. Read each chapter as assigned in your textbook, review the supplemental on-line modules, and review the CD.

ASSIGNMENTS/EMAILS

All email correspondence/ assignments must follow this format in the subject line of the email: AHT

1113 Medical Vocabulary –First Name, Last Name, and subject of email. Emails will not be opened if

they are not submitted in this manner. If the email contains an assignment and these instructions are

not followed, a “0” will be recorded.

TESTING POLICIES: There are six unit tests that are required to be taken within the timeframes noted below.

There will be two proctored tests. It is your responsibility to schedule an appointment with the Distance Learning Lab to take these. To schedule a test you will do this through Blackboard. You will sign on to Blackboard and then look under the section MCC E-Learning Assessment Center. There will be a link to click on to put in a request to schedule a test time. You will need the test ID number. It is on your course schedule. Please do not wait until the last minute or there may not be a slot available for you to take your test. If you do not schedule ahead of time, testing conditions cannot be guaranteed. Each test is timed for 50 minutes, which

is ample time for completion. When the reminder alerts you that you have one minute left, you must stop the test and submit it at that point. Two points will be deducted for each minute over 50 minutes spent on unit tests. Do not wait until the last day of availability to take your unit tests. They are available for at several weeks and you should plan ahead to take tests before the deadline. If something comes up and you cannot take the test on the deadline date, it will cause you to be dropped, unless a doctor’s excuse is faxed to the instructor.

GRADING POLICY: You will have 6 unit tests. There will be 4 unproctored tests and 2 proctored tests. The average of the unproctored tests will make up 50% of your grade. The average of the two proctored tests will make up 40% of your grade. The other 10% of your grade will come from the quizzes and any other assignments you are given. There will be 14 quizzes. Assignments will vary and may include but not limited to written assignments, participation in discussion board forum, etc. It is your responsibility to look for announcements regarding assignments due. I recommend checking Blackboard on a daily basis for any new announcements/assignments. GRADING SCALE: A = 90-100; B = 80-89; C = 79-70; D = 69-60; F = 59 or below Unproctored Tests (4 total) – 50% Proctored Tests (2 total) – 40% Quizzes/Assignments/Class Participation – 10%

Please print the following schedule and refer to it to be sure you are aware of all quiz/test dates. I highly

recommend printing this schedule out and making sure you are aware of all deadlines. Each test will be available

for a four day period of time. This is ample time to make sure you complete the test. I would also recommend

reviewing the schedule at least once per week.
