Documentation  of  Education     If you have completed an associate degree or higher, please sign the statement below to be eligible to sit for the Health & Wellness Coach Certifying Examination. (If you have not completed an associate degree, please complete the HWC Work Experience Document.) The NCCHWC/NBME is not requiring an official transcript, however a percentage of applicants will be audited to verify completion of an associate’s degree or higher. On my honor, I attest/certify/promise that I have an associate’s degree or higher and am eligible to sit for the Health & Wellness Coach Certifying Examination.
