5.Gotothe“NewUser”boxandclick“BeginHereforSign‐up”(redarrow)tocreateanonlineaccountwithaUserIDandpasswordForALLUSERS:EvenifyouhavepreviouslyusedtheBoard’sonlineservicesbefore,youmustcreateanewaccountinthisnewsystem.
6.Provideallrequestedinformationtobeginnewaccountregistration.Indicatetheemailtowhichyouwantallboardcommunicationssend.Under“Communication”(yellowarrow),wesuggestselecting“Yes”toE‐mailCommunicationtoassuretimelyreceiptofnotices/informationfromTNDepartmentofHealth.

7.Afteryou’veregisteredandcreatedanewaccount,[email protected].**Onlyadesktopcomputerorlaptop(notphonesortablets)shouldbeusedtoaccessthisemailaddressandactivatelinkscontainedwithinitscontents.

Ifthereisanexistingrequestalreadyopen,whichhasnotbeencompleted(redarrow),youwillhavetheoption(yellowarrow)toproceedwiththeincompleterequestortostartanewapplication.Ifyouchoosetostartanewapplication,yourexistingincompleterequestwillbecancelled.
13.Subsequentscreensinthe“APRNSupervisoryRequest”section,willenableyoutoreviewandupdateinformation.
Beginningwiththefirstpage,“Introduction”(greenarrow),click“Next”inthelowerrightcorner(redarrow)tosequentiallyreviewandeditinformation,asneeded.

Althoughthetabslocatedintheleftmargin(bluearrow)mayalsobeusedtonavigatepagecontent,itissuggested,foraninitialinformationreview,pagesbereviewedintheorderlisted. Subsequentreturnstothesystemtoupdateinformationmaybefacilitatedbyuseofleftmarginnavigationtabs.

NOTE:The“SupervisorSelection”tab(redarrow)willallowthefollowingfunctions: VERIFYaphysician DELETEaphysician ADDaphysician.
*YoumayonlyaddONEphysicianintheexistingform.Ifyouneedtoaddmorethanonephysician,youwillneedtocompleteandexittheexistingform,andstartanew“APRNSupervisoryRequest”.

Fromthe“APRNSupervisoryRequest–RelatedLicensesListing”screen,scrolltothebottomandselect“Add”(yellowarrow).
Youwillalsoneedyoursupervisingphysician’slicensenumberinordertoaddthem(bluearrow).Onlyenterthenumbers(NOLETTERS).Seebelowinstructionstoobtainaphysician’slicensurenumberonline.

Ataminimum,providefirst/lastnameandprovidednumeric/lettercode.Click“Submit”.
Onceallrequiredinformationhasbeensubmittedandyouhaveselected“Next”,yoursupervisorwillbeaddedtoyoursupervisorylist.Select“Next”againtoproceedtothe“APRNSupervisoryRequest–SupervisorSelection”screenshownbelow.Selectthemostrecentlyaddedsupervisorandclick“Next”.

15.VerifyyourNon‐ControlledLegendDrugSelectiona.Usingthedropdownmenu(yellowarrow),diligentlyreviewandselecteachclassofdrug
yoursupervisingphysicianhasagreedyoumayprescribeunderhis/herdelegation.Atthistime,youarerequiredtomakeaselectionandclick“Add”foreachclassofdrug.Enhancementsarependingsoyoumaymakemultipleselectionsfromadropdownmenu.Whenallclassesofdrugshavebeenadded,select“Next”tocontinue.

PLEASENOTE:Yoursupervisoryrelationshipwillnotbevalidatedunlessanduntilyoursupervisingphysicianvalidatestherelationship.Anoticeofyourpendingrequestwillbesenttoyourandyoursupervisorat15,30,and45daysaftertheinitialrequest.Requestsnotvalidatedwithin60dayswillexpire.
21.SubmitacopyofyourcompletedAPRN“CollaborativeRequest”document(pdf)withyourinitialcredentialingpacketor“OnePacket”toProviderSupportServices([email protected]).
