This is document 6(b) of 7.

1. Background/OverviewDuring your work placement, you will be undertaking a variety of duties and tasks that relate to your course and the role of a disability worker.

You will need to successfully demonstrate that you can perform tasks required in the work environment and collect evidence.

Your supervisor will confirm that you have completed each task according to the task instructions, and so you need to ensure your supervisor witnesses each task as you complete them.

Before you move onto the third and final part of the portfolio, make sure you have completed all the tasks in this, the second section, and you have uploaded your work to Open Space.

This is Activity 5 of 13

Using the Individual Care Plans from Activity 1 in the previous portfolio section, complete the following tasks using one client for each task.

Task 1

Client 1 – Engage in a discussion that you facilitate about the progress and barriers being faced in regards to implementing the client’s community engagement plan. Make notes on the discussion. Ensure the discussion identifies possible solutions to identified barriers.

Write your notes up in the report on the next page.
