ATTENTION 10th & 12th GRADERS — ILLINOIS YOUTH SURVEY

Parents of 10th and 12th Graders:

District 303 is working with the Center for Prevention Research and Development at the University of Illinois to conduct a survey of middle and high school students. The Illinois Youth Survey will be administered through Physical Education classes to 10th and 12th grade students on Tuesday, March 6, 2018. The purpose of the Illinois Youth Survey is to better understand Illinois youth risk and protective behaviors as they relate to student problems and academic success. District 303 has participated in this survey for several years and the data supports our efforts to reduce risk behaviors among students and improve academic success. The survey is optional and the opt-out form is attached. If you choose to opt-out of the survey, please sign and return the form to the Main Office by March 1, 2018. I have linked a Parent FAQ from the Center for Prevention Research and Development for your review. A hard copy sample of the survey is available in the Main Office. Thank you for your support of this survey and our continued efforts to support our students! Matt Gill, Assistant Principal - College & Career Readiness
