April 22, 2020 Dear New Dental Hygiene Student: Congratulations on your acceptance to the Dental Hygiene Program at Fayetteville Technical Community College. We hope you are looking forward to a challenging career choice. Your acceptance into the Dental Hygiene Program is subject to the following conditions: 1. You must continue to maintain a major grade point average of 2.5 or above and cumulative grade point average of 2.0 or above prior to entering the Dental Hygiene Program. 2. Once you enter the Dental Hygiene Program, you are subject to the rules and regulations of the Dental Hygiene Program. Prior to entrance into the Dental Hygiene Program, you must fulfill certain obligations: 1. Your Viewpoint Screening Account must be set up and your immunizations submitted by July 15. The first Hepatitis B vaccination must have been completed prior to the start of classes. You can access the Viewpoint Screening portal on the FTCC webpage at: Health Selected Students Click the tab “Viewpoint Screening Portal” and follow the steps under the “Dental Hygiene” tab to set up your account and submit your immunization records. You can also access the requirements using the following link: Dental Hygiene Requirements 2. Complete the Medcom videos on HIPAA (3 videos) and Preventing Bloodborne Infection (3 videos) located on the Viewpoint Screening portal. 3. CPR for BLS Provider (including AED) must be completed and documentation submitted prior to the start of classes. *CPR must be renewed annually in the Dental Hygiene Program and must be American Heart Association, BLS Providers CPR. 4. A required uniform package has been selected for you at Castle Uniforms, 1800 Skibo Road #228, location (485-4429). Castle Uniforms has arranged a reduced group rate for this package. You should be prepared to pay a minimum of ½ down deposit on the day of package order. You should visit Castle Uniforms before July 1 to be fitted, as uniform orders take approximately 4-6 weeks for delivery. 5. A required summer Orientation is scheduled for Friday, June 26, 9:00- 3:00. Location will be HTC 111. 6. You will be registered for your fall courses during the summer Orientation. Dear New Dental Hygiene Student:

Please allow 4-6 weeks delivery time for student uniform package. Please visit us by July 1, 2020. Personal Shopping Account- before ordering , please visit www.shopcastleuniforms.com and click on Sign In, located in the

top right corner to establish your student account and assure accuracy of shipping address. If you have shopped with us before, phone or email us for your account number to link to your existing account.

Online Ordering- If you are unable to make it to Castle Uniforms you may visit www.shopcastleuniforms.com and click on Group Sign-In located at the top and order your FTCC DH student uniforms there. Your Group Sign-In code is FTCCDH20

Payment Information

Pre- payment is required on your custom student package at time of placing order. A minimum of ½ deposit is required to begin processing your order. We accept MC/VISA/ Debit Cards, personal check, money orders and cash at time of order. Balances can be paid online with Credit/Debit card or paid in-store. Castle Uniforms will work with approved 3rd party billing agencies. Students using Financial aid via the FTCC Bookstore need to come in and begin the ordering process this summer. Bookstore

orders will not be available for pickup until they have been approved by FTCC.

Start Your Order To get started:

Visit https://www.viewpointscreening.com/faytechcc and click on "Start Your Order".

- Select your program and package option- Enter your information (name, dob, etc.)

***Important*** Please make sure you are entering your correct email address. You will be unable to log in or receive communications from Viewpoint Screening if your email address is not valid.

Once your order is submitted, you will receive a confirmation email containing a password. Use this info to log into your account to review other instructions you may have. You will also need this password to view your background check report.

Drug Test - You will receive an email with the subject line: “Viewpoint Screening Drug-screen registration” within 24-48 hours. This email will contain instructions and explain where you need to go to complete your drug test.

Health Portal You will have the capability to upload specific documents required by your school for immunization, medical or certification records.

● After you have placed your initial order, you will begin to get emails that notify you of additional items you need to take care of that are required by your school for clinical placement.

● To see the list of required immunizations and documents, after you have placed your order, go to: viewpointscreening.com and click on LOG IN in the right corner; use your email and password to log in.

● When logged in, click on Health Portal to view your specific requirements. Click on the Question Mark to expand the requirement and view the details of what is acceptable. Be sure to read these thoroughly, so you know what kind of documents you will need.

● As you complete your requirements, you can begin to upload them into your account at any time.

● To associate a document with a requirement: Click on the “Upload Document” button next to each requirement and select the correct file to upload. This can be done on a desktop computer, tablet or smartphone. All uploaded documents are typically reviewed within 24hours. If your document is not compliant, you will receive an email notifying you why it was not compliant, and how to fix it. This information can also be found in the “Student Messages” section of your account.

You will receive weekly email reminders to upload required documents, and you will be notified 30 days in advanced when a document is about to expire.

FOR BEST RESULTS:

DO: Be sure your name is visible on the document you upload. If your name is not on the document, it will be denied, and you’ll have to upload it again.

DO: Make sure you uploaded the correct document for a particular requirement.

DO: Be sure to read what is acceptable. For example, receipts are not acceptable as proof of influenza vaccination.

DO: Make sure document isn’t expired, and won’t expire during your clinical rotation.
