MASCC • Supportive Care makes excellent cancer care possible Multinational Association of Supportive Care in Cancer • www.mascc.org February 2018 Several articles in this month’s issue of the Society News are encouraging for MASCC. One is our engagement with other societies that include supportive care as part of their focus. I particularly acknowledge our new MOU with SIOG. We believe this collaboration enriches both organizations. The second is watching the Vienna meeting develop. Following the completion of the parallel sessions, we now are pleased to announce the 2018 Pre- Conference Workshops. As in the last couple of years, these workshops are generally designed to appeal to members of multiple Study Groups. We are also very pleased with the number of new members who joined in December and, of course, we want to ensure that we retain our current members as well. The last 18 months have seen great change in MASCC, with a new executive staff and a new strategic plan. However, it is vital that we keep delivering the services, resources, and experiences that our members want and, perhaps as important, adopt new initiatives that will encourage others to join MASCC because we can deliver value to them. I encourage any member to write to Melissa Chin or any of our Board members to float new ideas. Here is one to start. What would you think of a mentoring program that links members from developing nations with those from well-resourced countries, where the mentor also funds the membership fee of the mentee? The two individuals could meet face to face at MASCC meetings, but would also communicate year-round by videoconference, email, or social media. I believe this would not only be a mutually beneficial experience, but would also serve to promote supportive care in cancer in the developing world. There is the devil in the details, of course — such as how such pairings might occur and how to increase their chances of success. But these issues could be explored if there were sufficient support among members. I would also like to highlight some of our current members’ new ideas for increasing the value of MASCC in my subsequent messages. Finally,I hope that the diversity of Annual Meeting locations for the next four years is attractive and ensures ease of access to members from many countries over that time. I have just spent some leave time in the Galápagos Islands, so it seems appropriate to end with a quote from Charles Darwin, which could apply as well to societies like MASCC as it does to species: “It is not the strongest of the species that survives, nor the most intelligent, but the one most responsive to change.” ~ Ian Olver NEWS Welcome to the February Issue of the MASCC Society News This month we are pleased to announce the 2018 MASCC/ISOO Pre-Conference Workshops to take place prior to our Annual Meeting in Vienna. We have a report on the natural history and resolution of medication-related ONJ in cancer patients based on an international collaboration involving three MASCC members. As usual, we list upcoming meetings related to supportive cancer care. We extend a warm welcome to the new members who joined us in December. And we encourage every member to be a MASCC ambassador! New MOU: MASCC Partners with SIOG InJanuary,MASCCandtheInternationalSocietyofGeriatricOncology(SIOG)formalizedtheircloserelationship inaMemorandumofUnderstanding.SIOG,amultidisciplinarysocietythatincludesphysiciansandotherhealthprofessionalswithaninterestingeriatriconcology,aims

tooptimizetreatmentofolderadultswithcancer.SIOGworkstoincreaseeducation,improveclinicalpractice,andpromotenewresearchingeriatriconcology.Itsmemberscurrentlyrepresent93countriesaroundtheworld.

ThepurposeofthisMOUistoincreasecollaboration,mutualrecognition,andshared,culturallysensitiveapproachestothework of both organizations. BothMASCC and SIOG are committed to international collaboration. TheMOU recognizes amutual intention to support one another in developing partnerships and to work together toward shared educational,research, and clinical goals. Both organizations have agreed to promote supportive care in older cancer patients, fostermembershipinbothorganizations,supportoneanother’sactivities,andpursuejointprogramsandprojects.

MASCC Executive DirectorMelissa Chin notes that, “Through this association, our two organizations can better leveragesynergiesandseizenewopportunitiestocreateanddeployeducationandresearchsolutionsinternationally.Weareexcitedat the possibilities this formalMOU brings for furthering supportive cancer care around the world andmaking excellentcancercarepossible.”

SIOG President Stuart Lichtman, MD, FACP, adds that he’s very pleased about this new understanding between ourorganizations,addingthat,“Thisrelationshipwillcapitalizeonanaturalsynergy.ItwillbeaproductiveinteractionofMASCC,which has improved the lives of cancer patients by educating oncologists on appropriate symptom management andsupportivecare,andSIOG,whichhasbecometheleaderingeriatriconcologythrougheducation,stimulatingresearch,andmentoringyounginvestigators.Thisrelationshipwillfurtherenhancethemissionsofbothorganizations.Iamlookingforwardtoourjointendeavors."

Natural History & Resolution of Medication-Related ONJ in Cancer Patients

MASCC members Mark Chambers, Ourania Nicolatou-Galitis, and Deborah Saunderswere part of an international research team that conducted an observational caseregistry study todescribe the rateand timecourseofosteonecrosisof the jaw (ONJ)associated with cancer chemotherapy. ONJ risk factors, treatment patterns, andantiresorptiveagentswerealso tracked in this study,whichwill appear in SupportiveCareinCancerandisavailableonline(SchiødtMetal.,seebelow*).

ONJisanadversedrugreactionthatleadstoprogressivedestructionoftheboneinthemaxillofacialregion.Itisassociatedwithantiresorptivebone-targetedagentsusedtopreventbonecomplications inpatientswithsolidtumorsthathavebonemetastasis. The risk of ONJ is also related to time exposure, number of treatments, duration of therapy, and cumulativedosage. ONJ management, ranging from conservative treatment to surgical intervention, is challenging and depends ondiseasestage.Therapeuticgoalsincludecontinuedcancertherapywhilecontrollinginfection,pain,andprogressionofbonenecrosis.While the incidence of ONJ associated with antiresorptive therapy, as well as other risk factors, is well known,informationaboutthemanagementandresolutionofONJinclinicalpracticeislimited.Thiscaseregistrystudyrepresentsamajorsteptowardunderstandingthecourseofmedication-relatedONJ.

Inall,327patientswereenrolledinthisstudyandevaluatedbyadentalspecialistatbaselineandevery3monthsfor2years,and then every 6 months for 3 years until death, withdrawal, or loss to follow-up.Most of these patients had receivedantiresorptivemedicationbeforedevelopingONJ. The results are consistentwith thoseobserved in clinical evaluationsofskeletal-relatedeventsinpatientswithadvancedmalignancyinvolvingbone.Nearly70%ofpatientswithONJhadresolutionorimprovementduringthestudy,withhalfofthesehavingfullresolution.

Mostpreviouspublishedfindingsonmedication-relatedONJhavebeenbasedonsmallnumbersofpatientsoraggregateddataandsystematicreviews.ThesignificanceofthiscaseregistryreportisthatitdocumentedtheeffectofspecifictherapiesonONJinasubstantialnumberofpatientswithadvancedcancer.Italsoreportstheeffectsoflocaloralandmedicationriskfactors for ONJ, as well as treatment, management, and patient outcomes. MucositisStudyDesign:WhatDoWeNeedtoKnow?The overall goal of this workshop, presented by theMucositis Study Group, is to bring together basic, translational, andclinical researchers in an informal setting to advance the understanding ofmucositismechanisms andmanagement. Thisyear’sprogramwillhaveastrongfocusondesigningmucositisstudies.LedbySharonElad,theworkshophasthefollowingobjectives:

Topresentthelateststateofthescienceinmucositisresearch;Tofacilitateexchangeofknowledgebetweenexpertsandyoungresearchersfromallovertheworld;Toprovideanopportunityforyounginvestigatorstopresenttheirworktoanexpertaudienceforspecializedfeedbackandresearchadvice;To provide an opportunity for face-to-face discussions regarding collaborative research and development ofinternationalresearchnetworks;Todeveloplinkswithmultidisciplinaryteamstopursuenewresearchandfundingopportunities.

UpdateandNewDevelopmentsinSupportiveCareofPatientswithChemotherapy-InducedAlopeciaOver the past two years, new data has emerged concerning the use of scalp-cooling devices to prevent chemotherapy-inducedalopecia.Thepurposeofthisworkshopistogiveanupdateonnewdevelopmentsandproviderecommendationsonthesupportivecaremanagementofchemotherapy-inducedalopecia.TheworkshopispresentedbytheSkinToxicitiesStudyGroupandwillbeledbyJenniferChoiandMarioLacouture.

WhatDoestheFutureHold?PrognosticationinAdvancedCancerandClinicalDecisionMakingPatientswithadvancedcanceroftenwanttoknowhowlongtheywillliveandwhattheycanexpectattheendoflife.Indeed,theabilitytoestimateprognosisaccuratelyandcommunicatethisinformationsensitivelyisoneofthemostimportantclinicalskills.Thiscomprehensiveworkshopwillprovideanup-to-dateoverviewofthelatestliteratureandadvancesinthescienceandartofprognostication.Itwillincludediscussionofsurvivalpredictionforbothadultsandchildren,areviewofestablishedandnovelprognosticfactors,andpresentationofthelatestprognosticmodelsandwebsites.Itwillalsoaddresspredictionofsurvivalandotheroutcomesandasummaryofcommunicationtechniquesandtools.Thepanelofexpertsinprognosticationwill also discuss prognostication research methods and make predictions on where this field is going. The workshoprepresentsacollaborationbetweentheEducation,PalliativeCare,Nutrition&Cachexia,andPediatricsStudyGroupsandwillbeledbyDavidHui,MariannevandeWetering,PazFernández-Ortega,andEgidioDelFabbro.

UpdateandNewDevelopmentsinSupportiveCareofPatientsUndergoingImmunotherapyThisworkshopwill focuson the very topical issuesof advances in the fieldof immuno-oncology and themanagementofrelatedsideeffects.Overthepasttwoyears,newdatahasemergedconcerningtheuseofimmunecheckpointinhibitorsincombination with chemotherapy, other checkpoint inhibitors, immune-modulatory molecules, and oncolytic viruses. Thepurposeofthisworkshopistogiveanupdateonnewdevelopmentsandproviderecommendationsonthesupportivecaremanagementofpatientswithimmunotherapy-relatedadverseevents.TheworkshopispresentedjointlybytheNeutropenia,InfectionandMyelosuppressionStudyGroupandtheSkinToxicitiesStudyGroup,andwillbe ledbyJeanKlastersky,MarioLacouture,JenniferChoi,andBernardoRapoport.

ChallengingCommunicationsinOncologyCommunication is fundamentaltothepracticeofoncology,butcanoftenbechallengingforhealthcareprofessionals—aswellasforpatientsandtheircarers.Theaimofthisworkshopistoemphasizethepositiveeffectsof"good"communication,to highlight the negative effects of "poor" communication, and to provide the attendees with advice and guidance forundertaking difficult conversations. The mixed format will include lectures, panel discussions, and (voluntary) audienceparticipation.Theworkshop,whichrepresentsacollaborationbetweenthePsychosocialStudyGroupandthePalliativeCareStudyGroup,willbeledbyAndrewDavies,LidiaSchapira,PennySchofield,andGregCrawford.
