W• On Wednesday, November 2, 2011, a 28 year old

bulldozer operator, with approximately 8 years of mining experience, was injured at a surface mine. The victim was conducting reclamation work on top of a graded slope when he lost control of the bulldozer and it rolled over several times, traveling approximately 290 feet to the bottom of a 28 degree slope.

• The operator was wearing a seat belt, but sustained serious injuries.

• He was hospitalized and later died on November 14, 2011 from injuries he had sustained.

1. Always check the work area for unstable material and be aware of the machine’s location at all times.2. While traveling in reverse, always look in the direction of travel.3. While tramming a dozer with the push blade up off the ground, be aware that the machine’s stability has changed.4. Familiarize yourself with the machine’s maximum operating angle.
