We believe that the highest quality care for our patients should be
comprehensive, coordinated and patient centered. The relationship between
our doctors and patients promotes physical, mental and social well-being.
Our care is accessible to you, and delivered by a team of providers who are
trained in many aspects of care through all stages of life.
Family Health Center Our Mission Statement
• Is a patient centered medical home (PCMH), a model of healthcare delivery,
and health care setting that facilitates
partnerships between individual patients,
and their personal physicians,
and when appropriate, the patient’s family.
• Our state-of-the-art facility has been thoroughly modernized and expanded to provide improved access to
care, quieter adult waiting rooms,
educational tools, and a Reading
Corner for children. Baltimore themed décor
is prominent with a variety of individual
photos of Baltimore adorning the walls
of hallways, waiting rooms, and examining rooms.

Our Faculty Our attending doctors are board certified with many years
of experience completing fellowships and have areas of
emphasis in health care policy, advocacy, sports medicine,
maternity care and women's health, procedures, geriatrics,
HIV and Hepatitis C care, homeless healthcare, behavioral
health, pediatrics, and dermatology. Our Faculty serve on
many medical boards which keeps them well versed in the
ever changing medical field.
Our Residents The Family Health Center supports the Family Medicine Residency Program, one of the first and foremost
Family Medicine Residency programs in the U.S. Our resident doctors provide patient care during their
weekly round and bring the latest technologies and medical treatments while partnering with the patient
to promote health and wellness. Our residents have received numerous awards in regards to scholarly
activity.
Look for the Family Health Center to be featured in the 2012 MedStar Health Annual Report
and in an October 2012 issue of the Baltimore Business Journal!

Meet Our Coordinated Care Team Team members:
- Identify and assess the adult high risk patient
population.
- Manage social barriers to care with goal of reducing
over utilization of ER and Hospitals.
- Follow patients through various transitions of care
(Hospital, Home Visits, Hospice, Nursing Home,
Home Care, Pharmacy Visits) .
- Offer counseling services to general patient
population.
- Offer chronic disease management providing
education and follow up.
About Quality and Safety …
•We practice evidenced-based medicine and use tools within our electronic medical record to
make up-to-date medical decisions.
•We have ongoing projects to measure our
performance which allows us to continually improve.
•You actively participate in the medical decision making, and we will
continually seek feedback from you to ensure that expectations are being met.
•We use information technology to enhance patient education and also
communicate between you and your physicians. Our patient portal is a way you can
communicate with your physician using a computer.
•We report to outside agencies to make sure we are providing appropriate
services in our medical home.
•We’ve received the highest level of recognition from the National Committee
for Quality Assurance (NCQA).

Chronic Disease Management: High Blood Pressure, Acne, Diabetes, Hypothyroidism, Depression, Anxiety,
Allergies, Asthma, Complex Disabilities
Acute Care: Injuries (sprains, car accidents, burns, etc.), Illnesses (sore
throats, coughs, STD's, headaches, etc.)
Procedures: Skin removals, mole biopsy, laceration repair, colposcopy
Enhanced Access
Location: Medical Arts Building
Convenient Hours: Appointments available
Monday through Thursday (8am-8pm),
Friday (8am-4:30pm) as well as Saturday
mornings.
Enhanced Communications:
At least two of our physicians are on-call at
all times, and phone consultation is available
24 hours a day.
Our Patient Portal:
This portal allows you to communicate with your
physician and the office via e-mail.
