ABSTrACT: Modern Endodontics has given better prognosis to the teeth since year 1996. But sometimes clinicians get carried away during some procedures. Fractures or cracks have become major reason for endodontic failures. Its time to decide that to achieve straight line access, one has to balance the conservation of tooth structure. We have seen that ideal straight line access may lead to lot of destruction of the tooth. This may lead to facture of the tooth under heavy masticatory loads. Follow up of some teeth which are treated by greater taper preparations show cracks in the teeth after 2 to 3 years.Differential diagnosis of these cracks could be traumatic bite, selection of the post and destruction of the tooth structure during access preparation and selection of larger taper in shaping. This lecture will elaborate the factors influencing access cavity preparation and how to achieve straight line access with a MIE and which taper one should opt for.

It is time to evaluate the conservation of the tooth structure during endodontic therapy to improve longetivity of the tooth.

delayed your treatment and if you can take vaccines and tetanus shots uncomplainingly, why cannot you tolerate my small prick from a superfine needle?” We cannot…. Because we need patients as much as they need us.

Patients today demand pain free and complication free treatment. To win their trust and faith, we must provide them so. So to give them pain free treatment, Local anaesthesia is a necessity, not an option, almost every procedure requires this and patients judge us on how light our hand was, how many pricks did they get and how painless was the procedure!! Does L.A mean only lignocaine? What if there is an allergy? Is there a substitute? Which is the most effective one? How do I inject painlessly? What are the complications? And what is the reason for them ? How do I ensure long duration with minimal dose.

(PlATElET rICH FIBrIN) IN DENTISTrY

PRF (platelet rich fibrin) was first developed in France for use in the field of oral and maxillofacial surgery. Choukroun's platelet—rich fibrin (PRF)

is a leukocyte and platelet rich fibrin biomaterial with a specific composition and three-dimensional architecture. PRF is classified as a second generation platelet concentrate as it is prepared as a natural concentrate without the addition of any anticoagulants. PRF is often called Choukroun's PRF as there are other platelet concentrates with similar names such as Vivostat PRF (considered a pure platelet—rich plasma) or Fibrinet PRF (without leukocytes). PRF has a dense fibrin network with leukocytes, cytokines, structural glycoproteins and also growth factors such as transforming growth factorH1, platelet-derived growth factor, vascular endothelial growth factor and glycoproteins such as thrombospondin—I during > / =7 Days. Leukocytes that are concentrated in PRF scaffold play an important role in growth factor release, immune regulation, anti— infectious activities, and matrix remodeling during wound healing. The slow polymerization mode of PRF and cicatricial capacity creates a physiologic architecture favorable for wound.

TOPIC: HOW TO.....DO NO POST, NO CrOWN DENTISTrY....A MINIMAl INvASIvE WAY

Not every tooth going for an endodontic treatment needs a crown, we can preserve more crown structure specially enamel, by preserving what is remaining and replacing what has gone by with

only direct composite or semidirect composite onlays. For these kind of restorations we have to follow certain rules in which first thing is rubberdam isolation. I will share my few cases with the same protocol.

TOPIC: HOW TO.....NEgOTIATE CUrvED CANAlS

Successful endodontic treatment involves thorough cleaning and shaping of the root canal system. Unfortunately, not all root canals are straight. Clinicians regularly encounter curves of different

degrees and in various places all along the root canal system. These curved canals may also restrict the mechanical and chemical preparation of the curvature or may lead to some procedural errors affecting the prognosis. Negotiation of curved canals in endodontic treatment might be thought of as a series of very small steps that collectively produce the desired endodontic result. Said differently, to obturate a curved canal with an ideal shape requires patience and the achievement of many minor objectives to arrive at the desired end point. This lecture will examine and detail many of the small but essential steps needed to help clinicians negotiate canal curvatures safely and efficiently.

FACUlTY: Dr. POrUS TUrNErTOPIC: ExCEllENCE IN COMPOSITE rESIN rESTOrATIONSThe advent of composite resins and adhesion to the tooth has truly revolutionized dentistry. It is now possible to restore broken, carious, eroded, abraded and discolored teeth in one single sitting using tooth

colored light cured restorations. However to do this restorations successfully with excellent long term results without damage to the pulp requires one to follow and use the correct materials and techniques. The presenter who is a pioneer in light cured composite resin restorations will outline step by step how to bond restorations to the dentin and enamel. He will also describe how to restore Class IV fractured teeth using a revolutionary medium translucency restorative resin without the need of opaque or enamel shades. Diastema closures aesthetically has always posed a problem but with the technique perfected by the presenter these restorations can be done with excellent long term results. Another area which is also a concern is that of posterior composites. Post-operative sensitivity, pulpal damage, wear of composites, marginal and secondary decay is a problem with adhesive composite restorations, however using the proper materials and techniques, posterior composites can now be considered a true alternative to silver amalgam. However in Class II composite restorations using Tofflemire type matrix bands results in less than efficient contacts and contours. The presentation will highlight a revolutionary new matrix system which ensures excellent contacts and contours every time. Restorations of endodontically treated teeth using new low shrinkage stress relieving dentin replacement flowable composites which bond and support the weak dentin will be highlighted. In many cases these restorations are less destructive to the tooth and in some cases may eliminate the need to crown all endodontically treated teeth. Finally polishing and finishing of composites will be described using 12 bladed tungsten carbide burs, followed by aluminum oxide and diamond impregnated resin discs.
