The BSN application for admission will be evaluated on the following basis:

a. Successful completion of admission criteria as listed in the Lamar University General Catalog for the

Dishman School of Nursing by the provided the deadline.

b. Grade point averages, previous attempts for successful course completion, and overall academic

performance at all previous and current institutions.

c. Evolve Reach Powered by HESI Admission Assessment Test (HESI A2) scores. The test scores are

utilized as a competitive part of the admission process.

All applicants must meet the following requirements prior to admission into the nursing program:

1) Successful completion of required courses by Summer I semester (which ends in July 2021):

a. Successfully complete all prerequisite courses with a minimum grade of “C”,

b. Successfully complete all required core courses (except POLS 2301 and POLS 2302),

c. Successfully complete all required science courses with a minimum grade of “C”.

2) Grade Point Average (GPA) requirements:

a. Science courses: minimum 2.5 GPA (includes: Anatomy & Physiology I, Anatomy & Physiology II,

Microbiology, and Chemistry).

b. Preadmission and prerequisite courses: minimum 2.5 GPA.

c. Overall grade point average: minimum 2.0 career GPA.

d. Lamar University GPA minimum of 2.0 GPA and/or be of good academic standing at transferring

institutions.

e. Please note: GPA calculations may include previous course failures. See your pre-nursing advisor

for help in calculating GPAs to ensure qualification.

3) To be considered eligible to apply for admission to the School of Nursing, students may have no

more than 6 unsuccessful prerequisite course attempts. An unsuccessful course attempt is defined

as not meeting the minimum required grade, “C” or “D” as required by the nursing program.

Failure in courses not required by the nursing program would not count toward this eligibility

criteria.

4) Students must schedule and complete the Evolve Reach Powered by HESI Admission - Assessment

Test (HESI A2) no later than February 15, 2021. Upon completion of the HESI A2 test, please submit

a copy of the test scores to the Dishman School of Nursing Information Center (MMW 100) by

February 15, 2021. To be considered eligible for consideration, all required components must be

taken and a minimum score of 79% is required in the Reading Comprehension section. Please note:

Critical Thinking has been added as a required HESI A2 component for the Fall 2021 application.

HESI A2 General Instructions a. The sections of the test students are required to complete are: 1) Reading Comprehension, 2)

Vocabulary, 3) Grammar, 4) Mathematics, 5) Anatomy and Physiology, 6) Chemistry, and 7)

Critical Thinking. All required components must be taken during the same testing block in

order to be considered for admission.

b. Students may sit for this test only one time per application period (October 1st – February

15th), with the exception of the Reading Comprehension section if a score of less than 79% is

received on the initial attempt. You will be allowed to retake the Reading Comprehension

section one additional time in the application period in order to qualify for admission. The

Reading Comprehension section retake must occur within the application period to be eligible

for consideration. Applicants who fail to achieve 79% or greater after the retake attempt will be

considered Not Qualified for admission during the current application cycle. Students who are

unsuccessful in meeting the Reading Comprehension score qualifications are urged to retake

the full HESI A2 during the next application period.

c. The HESI A2 scores used for the admission application must be less than two years old at the

time of application (no older than February 15, 2019).

HESI A2 Testing Center Information

a. To take the HESI test here at Lamar University: Registration for this test at the Lamar University

Career and Testing Center may be done online at https://www.lamar.edu/career-and-testing-

services/testing/administered-exams/hesi.html. The Lamar University Career and Testing

Center may be contacted at (409) 880-8878 for any questions. The HESI A2 testing fee is $55

plus the online registration fee ($3.50), and it must be paid prior to the testing date.

b. To take the HESI test at other locations: Students wanting to test at another Prometric Testing

Center (other than the Lamar University Testing Center), will need to follow the steps listed

below:

1) Create an Evolve student account by going to https://evolve.elsevier.com/ and clicking on

“HESI Exams” and then “Register for HESI”

2) Once you are logged in, click on “HESI Assessment Student Access”

3) Under the “My Exams” tab, click on “Exam History”

4) Click on “Register for a Distance Exam”

5) Complete the required information in the box given. Make sure to include the Lamar

University School of Nursing ID (197020) and your first and last name. Your first and last

name must match what is on your government-issued photo ID or driver’s license. Do not

use your school’s Department ID as your Student ID when creating your student account.

6) Students will receive an email confirmation notice indicating that they need to schedule

the exam. It is important to carefully review the information in the confirmation notice.

7) An Eligibility ID will be issued to the student within 1-3 business days from the date of the

eligibility request.

10) The HESI A2 testing fee may vary and it must be paid prior to the testing date.

The number of applicants admitted to the BSN Nursing Program is based on faculty and space

availability. Qualified applicants meeting early admission criteria will receive early acceptance

notices. All other students that apply will receive notices of their application status (conditional

acceptance, alternate list, or otherwise) by email in late June 2021. Applicants from the alternate list

may be contacted based on ranked order and offered conditional acceptance as space becomes

available through August 2021.

APPLICATION INSTRUCTIONS: Please read carefully and follow all instructions.

1) If not currently a Lamar University student, the applicant must also apply for admission to Lamar

University following the guidelines in the General Catalog. Along with application to the University,

submit Official Transcripts from all universities and colleges attended to the Office of Admissions,

P.O. Box 10009, Beaumont, TX 77710. New transfer students can visit

https://www.lamar.edu/admissions/how-to-apply/transfer/index.html for more information on the

application and transfer process.

a. New transfer students should apply for admission to the semester they plan to enroll in

Lamar University courses or NURS 2213 Theories of Nursing.

b. Returning Lamar University students not enrolled at Lamar for Spring 2021, must reapply to

Lamar University for the Fall 2021 semester through www.ApplyTexas.org.

c. The Lamar University application is separate from the one required for the Bachelor of

Science in Nursing (BSN) Program. Applicants will be considered for admission to the

Nursing Program only after they are admitted to Lamar University.

2) The application available on the School of Nursing webpage should be completed electronically

(except for written signature). Once completed, please print, sign and submit the application for

admission to the Bachelor of Science in Nursing Program to the Dishman School of Nursing

Information Center (Mamie McFaddin Ward Building, Room 100), or to P.O. Box 10081, Beaumont,

Texas 77710 by February 15, 2021. NO handwritten applications will be accepted.

3) All applications, when submitted, must include the receipt for payment of the non-refundable $30

nursing Application Fee. This fee can only be paid online at www.lamar.edu/nursing. Click on the

“BSN Application Fee” link below the red “Payments” button on the right side of the page. CHECKS,

MONEY ORDERS, AND CASH WILL NOT BE ACCEPTED. Print out the receipt showing proof of

payment and submit it with your completed application. Applications submitted without a dated

receipt will not be processed.

4) Applicants who completed all coursework at Lamar University Beaumont are required to submit an

unofficial transcript to the Dishman School of Nursing Information Center (MMW 100) by February

15, 2021. If you have not registered for Spring and/or Summer courses yet, then submit an

unofficial transcript as soon as you have been registered for Spring and/or Summer semester as

proof that you are registered.

5) Students with coursework from other institutions will need to submit official transcripts from all

institutions attended to the Lamar University Office of Admissions AND to the Dishman School of

Nursing Information Center (MMW 100) by February 15, 2021. Core equivalent classes accepted

by Lamar University will be utilized for application requirements. Contact your Pre-Nursing Advisor

at the Undergraduate Advising Center (409-880-8822) for questions and additional information on

course equivalency and petitioning. The following requirements apply to new transfer students or

students taking courses at other locations:

a. Applicants who plan to take any classes at any college or university during the Spring semester

(in order to complete their remaining academic requirements), must show course enrollment

unofficial transcripts by February 15, 2021 as proof that you have been registered to take

those classes If you have not registered for Spring courses yet, then submit an unofficial

transcript as soon as you have been registered for Spring semester as proof that you are

registered to take those classes.

b. Immediately upon completion of any courses taken during the Spring 2021 or Summer 2021

semester, you must submit transcripts showing your final course grades, as follows:

A) for any courses that were not taken at Lamar University, you must submit official transcripts

to the Dishman School of Nursing Information Center AND to the Lamar University Office of

Admissions.

B) for any courses that were taken at Lamar University, you must submit unofficial transcripts

to the Dishman School of Nursing Information Center.

c. New transfer students must submit proof of advisement with Lamar University Pre-Nursing

Advisors. This can be done by accessing Degree Audit on Self Service Banner, clicking on the

“Plans” tab, and printing a screen shot of the webpage or by providing a screen shot of your

appointment confirmation email.

Proof of advisement should be submitted with the application by February 15, 2021.

i. REMEMBER: ADVISING IS MANDATORY - if you have not yet made an appointment with

a Lamar University Pre-Nursing Advisor, you must do so as soon as possible. If a student

has not been previously advised, please call the Undergraduate Advising Center at 409-

880-8822 and ask to make an appointment with a Pre-Nursing Advisor to ensure all

application requirements are met. Potential transfer students should first apply to Lamar

University, submit official transcripts to Lamar University Admissions Office for

evaluation, and then make an appointment with a Pre-Nursing Advisor.

ii. Students must be advised by a Pre-Nursing Advisor before registering for NURS 2373

Basic Pathophysiology and NURS 2213 Theories in Nursing.

iii. There is an online NURS 2373 Basic Pathophysiology course available for students who

live out of the area. Students who want to take the online class must first contact their

Pre-Nursing Advisor who will request approval from the nursing program for the student

to take the class online.

6) The Dishman School of Nursing will submit all applicants’ names to the Texas Board of Nursing

(TBON) for the required Criminal Background Check. TBON will later communicate directly with

each applicant via email with instructions for making an appointment to be fingerprinted. Each

applicant who successfully completes the Criminal Background Check will receive a blue post card

(Blue Card) or letter in the mail, indicating that no significant criminal records were identified.

Applicants who have recently had their fingerprints done for TBON may not need to be re-

fingerprinted, but will still receive a Blue Card or a possibly, a letter. Applicants with a positive

Criminal Background Check will be notified by TBON and given instructions for the required

Declaratory Order process. The Blue Card or letter (or if applicable, the Declaratory Order

outcome letter) is a mandatory requirement in order to obtain full acceptance into the nursing

program. (See Criminal Background Screening Policy on pages H-J for more information.) It is

extremely important to check your email and, if necessary, your spam folder for correspondence

from TBON in the weeks after the Application deadline for these instructions. See the Checklist on

page “L”, items 8 and 9, for deadlines and requirements regarding the Blue Card. Required Notice to

Students: The email address that you provide to TBON is required in order to schedule fingerprinting for your

criminal background check. The email address that you provide to TBON is subject to release to the public

pursuant to the Texas Public Information Act.

7) After the online application for admission to the Dishman School of Nursing has been printed, you

must sign it in all three places on pages 5 and 6. Your signatures verify the acknowledgement of the

Dishman School of Nursing Criminal Background Screening Policy and the Texas Board of Nursing

Eligibility for Licensure Standards Information, along with certifying the information you provide in

this application is true and accurate

8) If you have previously completed the Hepatitis B series (often administered during childhood) then

you should obtain proof of your Hepatitis B immunization for submission if accepted to the nursing

program. Previous immunization records may be located at your physician’s office and/or high

school administrative offices. If you have not previously completed the Hepatitis B series, it is

suggested you receive your first Hepatitis B immunization by February 15 and your second

Hepatitis B immunization by April 1st. At least two immunizations must be completed prior to

entering the program and providing patient care. If you are unable to locate proof of

immunizations, it may be appropriate to do a lab serology (titer) test or repeat the adult series of

the listed immunizations. Please consult with your health care provider for questions related to

your immunization status. (CDC Requirement for Health Care Providers.) The full Immunization

Policy and Requirements can be found in the current Baccalaureate Student Handbook. An online

9) Nursing studies at Lamar University include practicum learning experiences, which may present a

potential risk of contracting and/or spreading infections. It is imperative that nursing students

maintain optimal health. The student health policies address the requirements as directed by

Lamar University, the Texas Department of State Health Services and the Center for Disease

Control (CDC). Student Health Policies can be found in the current Baccalaureate Student

Handbook. An online version of the handbook is available at www.lamar.edu/nursing. Click on

“Resources”, then “Forms.”

10) Upon acceptance to the nursing program, students are required to purchase professional liability

insurance. Further information will be provided prior to entering the practicum portion of nursing

courses. Liability Insurance information can be found in the current Baccalaureate Student

Handbook. An online version of the handbook is available at www.lamar.edu/nursing. Click on

“Resources”, then “Forms.”

11) Students accepted into the Baccalaureate Nursing Program at Lamar University will be subject to

admission drug screening as per the Dishman School of Nursing’s Drug & Alcohol Screening Policy.

Drug & Alcohol Screening Policy (prelicensure) information can be found in the current

Baccalaureate Student Handbook. An online version of the handbook is available at

TEXAS BOARD OF NURSING (BON) ELIGIBILITY FOR LICENSURE STANDARDS INFORMATION

CRIMINAL BACKGROUND SCREENING POLICY

Purpose The Lamar University (LU) JoAnne Gay Dishman School of Nursing adheres to the policies of all practicum facilities with which the School of Nursing is affiliated for student practicum learning experiences. Many practicum facilities and school districts require criminal background screening of all students. Practicum agencies used by the LU JoAnne Gay Dishman School of Nursing (LU SON) stipulate in the practicum affiliation agreements that students’ criminal background be prescreened before they are permitted into the practicum facility. This prescreening requirement is the same as that required of employees of public and private practicum agencies. The rationale for this requirement for practicum students is based on the concept of due diligence and competency assessment of all individuals whose assignments bring them in contact with patients and employees. Competency assessment extends beyond technical skills to include an individual’s past behavior as indicated by their criminal history. This approach ensures uniform compliance with Joint Commission standards pertaining to human resource management. Moreover, the public is demanding greater diligence in light of the national reports of deaths resulting from medical errors. Successful completion of a background check does not guarantee eligibility to take the NCLEX-RN examination, licensure or employment after graduation. Full acceptance into the nursing program is contingent upon satisfactory results of the Criminal Background check. Policy Timing After the application deadline, the Dishman School of Nursing will submit all applicants’ names to the Texas Board of Nursing (TBON) for the required Criminal Background check (CBC). Applicants who need to be fingerprinted will receive an email from IdentoGO with instructions for making an appointment. Applicants who have recently completed a TBON CBC will not receive an email from IdentoGO to make an appointment because their previous fingerprints will be used for the CBC and a new Blue Card will be mailed to them. All first-time applicants with a clear criminal background will receive a Blue Card in the mail. All Applicants with a positive CBC will be notified by TBON and given instructions to complete the required Declaratory Order. The Blue Card (or if applicable, the Declaratory Order outcome letter) is a mandatory requirement in order to obtain full acceptance into the nursing program – The Blue Card, or the Declaratory Outcome letter, must be received by LU SON 10 days prior to the first-class day of the nursing program. Please refer to “Process” section below for further information regarding the Declaratory Order. The results will be accepted for the duration of the student’s enrollment in the nursing program if the participating student has not had a break in the nursing program and if the student has had no qualifying legal incidents or convictions while enrolled. A break in enrollment is defined as nonattendance of one full semester or more. Attendance must be verifiable through the university. Process The LU SON accepts criminal background screenings completed by the Texas Board of Nursing. The screening will include the student’s complete criminal history, including the cities and counties of all known residences. The student is responsible for any cost associated with the criminal background screening. A student does not qualify for the blue card/clear report anytime there is a record of criminal activity located or they are self-disclosing an incident not yet reported to DPS/FBI. The TX BON provides a process by which minor infractions can be handled in a timely manner. More serious findings will take more time and may lead to a delay in qualifying for admission. In general, based on Texas Board of Nursing guidelines, the following histories will disqualify an individual from consideration for admission in the nursing program:

1. Misdemeanor convictions/deferred adjudication or felony convictions/deferred adjudications involving crimes against persons;

2. Misdemeanor convictions/deferred adjudication related to moral turpitude;

3. Misdemeanor/felony convictions/deferred adjudications for the sale, possession, distribution, or transfer of narcotics or controlled substances;

4. Registered sex offenders. Individuals with any of the above histories will not be eligible to enroll in the nursing program, and if the history is discovered or a conviction occurs after enrollment, the student, will be required to withdraw from the program. A student who is convicted of a criminal offense while enrolled in the program must report the conviction to the LU SON Chair within three days of the conviction. The term conviction for these purposes includes probated sentences and deferred adjudications. A student may be considered for admission to the program if the TX BON issues a Declaratory Order stating the individual is eligible for initial licensure or license renewal.

Notes

1. The student should be aware that the disqualifications listed above are used for employment eligibility by most hospitals in Texas.

2. Completion of a specific program will be determined by the student’s ability to successfully complete the required practicum rotations.

3. Practicum rotations are completed at sites specified by and contracted with Lamar University. Lamar University will not locate or provide alternative sites for practicum rotations for student ineligible to attend practicum rotations at the specified sites.

Compliance and Record Keeping

1. The Texas Board of Nursing will notify Lamar University (JoAnne Gay Dishman School of Nursing Chair) of all individuals who fail a criminal background check.

2. Verification sent by the TX BON to LU will include only the student’s name and social security number. Details of the criminal record will not be included in the report.

3. The LU JoAnne Gay Dishman School of Nursing will send verification on adherence to the policy to the practicum affiliate prior to the practicum rotation start date. Verification is accomplished by sending a letter from the JoAnne Gay Dishman School of Nursing on letterhead stating that these standards have been met by the student, listing the student’s full name and practicum rotation start date. If more than one student is attending a practicum rotation, a comprehensive list with all of the student’s names may be submitted.

Note: Verification information will be filed in a secured area to ensure confidentiality. In the event that the student feels that an error has been made in the results of the criminal background check, it is the responsibility of the student to contact the external vendor for a verification check and the student is responsible for any cost associated with this check. Other than error relative to identify, there will be no appeal of this policy.

STANDARDS FOR NURSING STUDENTS

Responsibilities and standards in nursing practice are determined by the Board of Nursing, national accreditation guidelines, and clinical agencies. Additionally, as described in various clinical evaluation tools and course guidelines, students must be able to provide safe patient care. Technical Standards for Nursing Practice Nursing is a practice discipline and successful completion of the nursing program requires that students demonstrate ability to effectively and safely perform several essential skills. All students are assessed in practice labs and clinical practicums throughout the program to confirm adherence to the technical standards, with or without accommodation. Intellectual Requirements: Critical thinking is an important part of clinical judgment needed to provide safe patient care. Students must be able to measure, calculate, reason, and quickly analyze information in order to apply complex information in the classroom and clinical settings. Gross and Fine Motor Requirements: Nursing students must be able to provide general care (including preparing medications and administering treatment and completing patient assessments). For example, students must be able to perform basic life support (CPR), function in an emergency situation, and have the ability (within reasonable limits) to assist a patient in moving, such as from the chair to the bed. Observation Skills: Student nurses must be able to observe patients accurately to assess their condition and perceived signs of disease and responses to treatments through physical examination, including visual images, auditory information (patient voice, heart tones, bowel, and lung sounds), and palpable changes in certain organs and tissues. Communication Requirements: Nursing students must be able to communicate effectively with patients, staff, physicians, and family members, verbally and in writing. Students must be able to process and accurately convey information about the patient status to faculty, staff, and other health care providers in a timely manner. Social and Behavioral Requirements: Student nurses must possess sufficient mental and emotional health to exercise good clinical judgment, promptly complete assessments, and function in what can be a physically and emotionally demanding dynamic environment. Nursing students must be able to demonstrate compassion, empathy, and concern for others while maintaining a mature, sensitive, and effective relationship with patients, families, and other professionals.

3. Submit a current transcript from each and every college or university attended with the application to the Dishman School of Nursing. Official transcripts are required for classes taken at colleges or universities other than Lamar University. If classes were taken at Lamar University, then an unofficial transcript from Lamar University will be accepted.

4. PROOF OF ENROLLMENT for courses “In Progress”: Students currently enrolled in classes during the application period (either at Lamar University, or elsewhere) must submit an unofficial transcript as proof of enrollment in the classes. a. TRANSCRIPTS – At the end of the semester, as soon as final grades have been posted, students who

were enrolled in classes during the application period must submit transcripts showing the final grades for the courses that were taken, per the following criteria: i. IF the classes were taken at Lamar University during the application period, then students must submit an unofficial transcript to the Dishman School of Nursing (as proof of the final grade that was made). ii. IF the classes were taken at any other college or university during the application period, then students must submit an official transcript to both the Lamar University Office of Admissions and to the Dishman School of Nursing (as proof of the final grade that was made).

5. Submit a copy of your HESI A2 test scores. Remember: you must make 79% or better on the Reading Comprehension component of the HESI. The HESI may be taken only one time per application period but, if you make less than 79% on Reading Comprehension, you may re-take the Reading Comprehension section. Do not forget to submit your Reading re-take score, if applicable. Be sure that ALL SEVEN required components of the HESI A2 have been taken. All components should be taken in the same testing block. See item “a” of page B for a list of the required components.

6. Transfer students: Submit proof of advisement or appointment confirmation (if after February 15th) with an Undergraduate Advising Center Pre-Nursing Advisor. This can be done through Degree Audit on Self-Service Banner or email.

7. Criminal Background Check – If this is your first application to the Dishman School of Nursing and if, by 45 days after the application deadline (04/01/2021), you have not made an appointment to be fingerprinted by following the instructions that were emailed to you from IdentoGo, then you must call the Nursing Information Center 409-880-8868 or 409-880-2307, to receive further instructions. Remember, the Blue Card will not come in an envelope. You should submit it to the Information Center as soon as possible after you receive it.

9. Criminal Background Check – If you have previously applied to the nursing program, then you will receive a new Blue Card or letter for this application period. You should submit it to the Information Center as soon as possible after you receive it.
