YEAR 12 VCE STUDY CONFERENCE A Compulsory Year 12 Study Conference is held for Year 12 students who are completing their VCE/VCAL in 2020 to help them settle in to this most challenging and demanding year. The conference will be held for 2 days – Monday 10th February 2020 and Tuesday 11th February 2020. The purpose of the conference is recreational and educational, with these two purposes closely inter-related. The success of individual students often depends very much upon the morale of the whole group. The conference is intended to develop a very positive co-operative attitude among all Year 12 Students. There will be a number of study sessions running at this conference as part of our total year 12 curriculum. Consequently, we consider it vital for students completing their final VCE/VCAL year to attend the conference to avoid missing important work. WELLBEING DAY AND TERTIARY INFORMATION SESSION (TIS) These events will be held in the second half of the year and more information will be provided closer to the events. COST The charge per student will be $125 which covers the cost of transport, materials, workshops, special speakers and also covers the cost of a ‘Wellbeing Day’ and a Tertiary Information Session (TIS). If you are eligible for the Camps, Sports and Excursions Fund (CSEF) you may use this towards the payment. Please make this payment prior to the promotion interview. Recording - Authorisation and Privacy Consent Form Attachment A – Frequently Asked Questions

Who can sign this form?

If you are or the Student and you are under 18 years of age and not considered a mature minor – one of the following people can sign this form (whichever is applicable in the individual circumstances):

• person who has parental responsibility for “major long term issues” as defined in the Family Law Act 1975 (Cth)

• a person appointed as “guardian” pursuant to the Children Youth and Families Act 2005 (Vic)

Where neither of the above people are available or cannot be contacted, consent may be obtained in the following ways:

1. Court Appointed Guardian

If you or the Student have a person appointed as a “guardian” pursuant to the Guardianship and Administration Act 1986 (Vic), the guardian should sign the consent form.

2. Informal Carer

An Informal Carer is a relative or other responsible adult with whom the Student lives, and who has day to day care of the Student. See the School Policy Advisory Guide for information on informal carers:

http://www.education.vic.gov.au/school/principals/spag/participation/Pages/admission.aspx.

It is important to note the following: • The informal carer should provide an Informal

Relative Carer Statutory Declaration to confirm their status as an informal carer. A copy of this statutory declaration can be obtained from http://www.ccyp.vic.gov.au.

• If a person with parental responsibility for major long term issues and/or a person granted guardianship can subsequently be contacted, their consent should be sought.

• If a person with parental responsibility for major long term issues and/or a person granted guardianship subsequently refuses or withdraws consent, their decision prevails, and the consent and authority under this form will cease immediately.

3. Mature Minor Students

FAIRHILLS HIGH SCHOOL 2020 ANNUAL CONFIRMATION OF ENROLMENT

Revised 4/11/19 This Confirmation of Enrolment form includes a comprehensive set of important understandings and responsibilities whereby parents can assist the staff to provide a safe environment and the best possible learning and developmental outcomes for their students at Fairhills. Please read it through carefully, tick each of the boxes that are relevant to you, sign it and return it asap but no later than the due date of 20th December. If you would like further information on any item before signing, check the ‘Enrolment’ section of the school website www.fairhillshs.vic.edu.au or phone the Associate Principal on 9758 5022 preferably before the due date.

A. Current Family Details It is imperative that we have up-to-date contact details for all our current students. In the case of illness, accident or emergency we may need to contact you immediately. As our funding is affected by parental level of education as well as current employment status and occupation group, can you please complete the DET Data Collection Form in the pack to ensure details are correct.

B. Student Well Being We understand that Educational research repeatedly shows that student outcomes are enhanced when parents:

(i) actively participate in their student’s learning (ii) consistently support the school’s ethos, structures and processes (iii) constructively work together with the school staff on matters affecting their particular student.

We will ensure that our student attends school punctually and regularly (except when ill or other good reason) and phone the Attendance Hotline (or send an email) by 9.00am on any day of absence. We will ensure that our student is in full school uniform each day or provide a note on the day if there is a temporary reason for not doing so. We have read the document ‘Expectations for Homework, Years 7-12’ and will encourage and assist our student to consistently meet these expectations. We will take the opportunity to keep our student’s Home Group Tutor informed of any personal circumstances which we consider may be relevant and consult with him/her as a first port of call if we have any concerns. We will take the opportunity to attend at least one of the two Parent/Teacher/Student interview sessions scheduled each year.

C. Behavioural Expectations We understand that parents overwhelmingly want their students’ school to have clear and fair standards and rules about student behaviour so that the school environment is safe, orderly, conducive to effective student learning and respectful of property, students, parents and staff. We want to work conscientiously with the school to assist our student to further develop his/her ability to consistently choose behaviour which is responsible and respectful whether they are at school, at home or in the community. We see it as our parental responsibility to support the school’s behavioural expectations, rules and procedures (carefully developed by School Council and staff), to reinforce them at home and constructively work with school staff to agreeably resolve any issues that may arise. Consequently we will discuss with our student the various items on the ‘School Behavioural Expectations’ sheet which the school asks students to agree to and sign at the start of each year.

Page 14: FAIRHILLS HIGH SCHOOL 2020 YEAR 12 RE ......FAIRHILLS HIGH SCHOOL 2020 YEAR 12 RE-ENROLMENT PACK STEP 1: COMPLETE FORMS/PAYMENT Please complete the following attached forms and return

D. Resourcing of Classroom Activities We understand that the School Council allocates approximately $150,000 per year to provide the equipment, materials, consumables, support services etc. used day to day in the classrooms of all subjects at all year levels. We further understand that only about 10% of this comes from the school’s Government grant (for the basic essentials only) and that the other 90% (for enhanced Teaching and Learning activities) is obtained from the subject fees paid by parents at $585 per year. We understand that bulk-buying benefits save the school around 20% of the costs of individually purchased materials and that these savings are used by School Council to provide assistance subsidies to those parents who have significant financial difficulties in paying these subject fees. We want to support this important school strategy for enhancing T & L beyond that provided by the basic government funding, and will do so by paying the subject fees according to the option we have indicated.

E. Extra-Curricular Opportunities We understand that the school has a strong commitment to providing optional Extra-curricular activities for students, but these cannot be reasonably done at the expense of the normal day-to-day classroom programs. Consequently, we accept that additional costs to parents are usually involved and that the school considers these a lower priority than the payment of the subject fees for the core curriculum. These extra activities include The Musical, non-essential camps and excursions, some sporting activities, Presentation Ball, Dance Competition etc. We want to provide extra-curricular opportunities for our student (when they are appropriate and when we can afford them) and will act responsibly by ensuring that we have met our obligations to the subject fees. We will ensure that our student selects and actively participates in at least one ILP (Independent Learning Program) each semester. (Only applies to students in years 7 – 9).

F. Community Image and Reputation We understand that maintaining the good reputation that Fairhills has in our community is very important. Our effectiveness as a school, our ability to attract a full complement of Year 7 students each year, our funding allocation from the Government and our consequential capacity to provide a wide range of opportunities for all students depend upon the continuation of our good reputation. We offer to do our part to enhance the Fairhills image by: 1. Taking opportunities to speak well of its strengths/achievements to our friends, other parents, family and community contacts. 2. Reinforcing with our student the real benefits he/she is receiving in attending the school that we chose together as the one most suited for our needs. 3. Drawing the school’s attention in a supportive manner to any mistakes or deficiencies we see and where appropriate, working constructively with the staff to rectify the situation and further enhance the school.

In signing this Confirmation of Enrolment for 2020 for our student ________________________________, we endorse all the above items that we have ticked. Adult A full name: ______________________________ Signature ___________________________ Adult B full name: ______________________________ Signature ___________________________ Date: ________________ Please keep one copy for future reference and return the other to the school ASAP but no later than 20th December.

FAIRHILLS HIGH SCHOOL 2020 BEHAVIOURAL EXPECTATIONS OF STUDENTS

Revised 4/11/19 Students at Fairhills High School are expected to take full responsibility for their education so please take time to ensure that the following expectations are fully understood before the personal commitment is signed and returned to the school. School Rules 1. The behavioural expectations of all students are detailed in the Document section of Compass. In

summary: “we expect that all students should be responsible persons showing respect for people, (both themselves and others) and for property (their own, that of others and of the school)”. The students are expected to become familiar with the school and class rules and to comply with them. Clarification (if required) can be obtained from the class teacher or the appropriate Subschool Leader.

Attendance 2. Students are expected to arrive at school by 8.30am and attend all classes punctually. 3. When students are absent, parents are required by the Department to provide a prompt

explanation. We ask parents to leave a message on the Attendance Hotline (9758 5022) by 9.00am on the day of absence, or mark on Compass. Alternatively, you may send an email ([email protected]) also by 9.00am on the day of absence.

4. Students attending excursions, camps or other school functions held at the school or elsewhere after hours are expected to also comply with the school rules and to respond to directions given by staff members who are present.

Uniform 5. The School Council at Fairhills High School has determined that all students are expected to wear

full school uniform. It is the responsibility of the parent to ensure that the student attends school in uniform and provide a note if there is a problem. Students who do not conform and fail to provide a note of explanation will receive a detention.

6. The Uniform Shop is located in room R4 and is open Thursday 12.00pm – 4.00pm. Financial assistance is available if required by contacting Ms Karen Dean, Associate Principal.

Study Organisation 7. Assessments are based on a series of Work Requirements and Assessment Tasks set by subject

teachers. Students are expected to complete all work set by individual subject teachers. If work does not reach a satisfactory standard students will be asked to resubmit. The details are placed on Compass so that parents can monitor their child’s progress.

8. Students who are involved in extra-curricular activities (such as interschool sport and instrumental music) are expected to be responsible for the notification to teachers of their absence from class. It is also their responsibility to catch up on the work missed. The Subschool Leader may withdraw permission to attend these activities if they believe the student’s progress is being adversely affected.

9. Students are expected to allocate sufficient time out of school for set homework, the completion of unfinished tasks and revision. (See the document ‘Homework Expectations Years 7 – 12’ for details).

10. Students are encouraged to have part-time employment, but this should not exceed 8 hours per week so that their studies are not negatively affected.

General Attitude 11. Students are expected to behave in a courteous and fair manner towards their teachers, support

staff and peers. It is expected that students will co-operate with, and fully support all staff and adult volunteers in carrying out the work of the school.

12. Students are expected to take good care of their own materials and also the facilities and equipment provided by the school.

13. Parents should ensure that their student’s mobiles are turned off at night so they have uninterrupted sleep, that their student’s notebook use is adequately supervised and that the batteries are recharged each night.

We understand and support the above. Student’s Name: _________________________________________________________________ Student’s Signature: ______________________________________________________________ Parent/Guardian Signature: ________________________________Parent Phone No: _________
