Income earned on Real Estate Investment Trust (REIT) is subject to withholding tax when the beneficiary is a non-SA resident. Interest earned on certain debt is subject to Interest Withholding Tax (IWT) when the beneficiary is a non-SA resident. REITs tax and IWT is withheld by us and paid to SARS on your behalf where applicable. If you are a non-SA resident for tax purposes, you may in addition qualify for a partial exemption of DWT/IWT. Financial adviser fees The Financial Adviser may receive an initial advice fee and an annual service fee from the client. Please specify all financial adviser fees that have not been pre-populated in the above table. The total fee may not exceed 1.20% p.a. per portfolio, with the exception of the PSG Money Market Fund which may not exceed 0.50%. Refer to the table above for details of such maximums. In respect of initial fees and annual service fees, if no sale of units service fees have been specified, 0% will apply in respect thereof. If a fee higher than the maximum has been specified, the maximum will apply. Initial fees may only be taken in 0.25% increments, and annual service fees in 0.05% increments.

I understand that the Financial Adviser annual Advice Fee will be facilitated by the repurchase of units on a monthly basis from my investment before being paid to my appointed financial adviser.

Phasing in is only allowed from the PSG Money Market Fund and is processed monthly. Phase-in’s are processed on the 5th of each month. Where this falls on a weekend or public holiday, they are processed on the first business day thereafter. Please note that a phase-in can only be processed 14 days after processing of the original investment in the case of once-off collection and cheque deposits. This may result in the phase-in being carried over to the next available date, i.e. the 5th of the following month.

L. FINANCIAL SERVICE PROVIDER (FSP) AUTHORISATION . FINANCIAL SERVICE PROVIDER (FSP) AUTHORISATION I have met with the investor prior to the investment being placed with PSG Asset Management and the necessary due diligence process as required by legislation has been satisfactorily carried out.

I, an accountable institution so defined, warrant that I have established and verified the true identity of the investor in accordance with the Financial Intelligence Centre Act 38 of 2001 (FICA) and confirm that identity checks have been undertaken to confirm that the investor’s name, address and identity as shown in this application form are true and correct. Attached please find copies of the documents evidencing the legal structure and existence of the Investor as required in Section N of the Application Form.

I confirm that I subscribe and adhere to the fit and proper requirement and other statutory obligations and criteria imposed by legislation covering advice dispensed and/ or intermediary services rendered to investors.

I declare that I am required to sign an Intermediary Partner Agreement and I shall comply with the provisions set out in the Intermediary Partner Agreement entered into between myself and PSG Asset Management.

M. EFFECTIVE ANNUAL COST

The Effective Annual Cost (EAC) is a measure which has been introduced to allow you to compare the charges you incur and their impact on investment returns when you invest in different Financial Products. It is expressed as an annualised percentage.

The EAC is made up of four components, which are added together, as shown in the results of the EAC Calculator available on our website. The effect of some of the charges may vary, depending on your investment period. The EAC calculation assumes that an investor terminates his or her investment in the Financial Product at the end of the relevant periods shown in the table. We recommend that you perform the EAC calculation before investing. N. FICA REQUIREMENTS (COMPULSORY FOR ALL INVESTORS)

In terms of the Financial Intelligence Centre Act 38 of 2001, and the regulations thereto, PSG Asset Management is compelled to identify and verify the identity of its clients. We require the certified copies of:

3.1 Certificate of Incorporation (CM1 or CoR15.2); Certificate of Change of Name of Company (CM9 or CoR14.3), where applicable; Notice of Registered Office and Postal Address (CoR21) bearing Registrar’s stamp and signed by the Company Secretary and Certificate of confirmation issued by CIPC or Register of Directors, Auditors and Officer (CoR39).

3.2 Proof of business address (less than 3 months old). 3.3 Document(s) confirming Income Tax number as registered by SARS. 3.4 Documents as set out in 1.1 and 1.2 above in respect of the manager, all authorised representatives, and all individuals holding 25% or more of the voting

rights at a general meeting. 3.5 Proof of authority to act for company i.e. Director’s resolution. 3.6 Documents as set out in 3.1 & 3.2 for legal persons holding 25% or more of the voting rights at a general meeting.

4. Trusts4.1 The main deed or other founding document e.g. a will. 4.2 Letters of authority from Master (SA trust) or foreign regulator (foreign trusts) to trustees. 4.3 Document(s) confirming Income Tax number as registered by SARS. 4.4 Documents as set out in 1.1 and 1.2 respectively in respect of an individual founder of the trust, each individual trustee, each individual beneficiary and all

authorised representatives. 4.5 A death certificate or certificate of deregistration where founder died or the trust ceased to exist. 4.6 Documents as set out in 3.1 and 3.2 in respect of any institutional beneficiaries, any institutional trustees or institutional founder of the trust.

O. TERMS AND CONDITIONS

• I hereby apply to invest the mentioned amount (s), in accordance with the provisions of the Trust Deed of the Fund, on the terms and conditions of this ApplicationForm at the price ruling on the date of receipt of the funds by PSG Asset Management subject to the application having been received before 11h00 for the PSG Money Market Fund, 12h00 for once-off collections and before 14h30 for all the other funds on the business day.

• This application form and the Deed will form the agreement between the investor and PSG Asset Management. The Agreement will be administered in terms of the Collective Investment Schemes Control Act, 45 of 2002 (“the Act”). This Agreement shall be construed and interpreted in accordance with the laws of the Republic of South Africa.

• It is the policy of the PSG Asset Management Board of Directors to comply with South African money laundering legislation to prohibit and actively prevent the use of its worldwide operations for criminal purposes. When any investment is made, additional documentation for identification and verification purposes may be requested to enable compliance with money laundering regulations.

• Participatory interests will be bought at the NAV price calculated on that day.

• Participatory interests will be sold at the NAV price and paid to the investor in accordance with the requirements of the Act and the relevant Trust Deed.

• The fully completed application form, together with proof of deposit and any additional supporting documentation as required by PSG Asset Management must be received before 14h30, or 11h00 for the PSG Money Market Fund, to be transacted at the closing price for that day.

• Requests to redeem from the PSG Money Market Fund must be received by PSG Asset Management before 11h00 to be transacted on that day.

• PSG Asset Management will not proceed with any transaction if there is any doubt as to the validity of any signatures or if PSG Asset Management deem the instructionto be incomplete in any way. An instruction will be deemed complete on receipt of cleared identifiable funds in the correct bank account and all requested information.

• All payments will be electronically transferred into the bank account of the investor only. Payments will only be made to the banking details stipulated underSection H. PSG Asset Management does not make any third party payments.

• Only signed written instructions from the investor or its duly authorised representative will be acted upon.

• In the event that this application form or any subsequent instruction is faxed to PSG Asset Management, the responsibility of ensuring that the instruction has been received and actioned by PSG Asset Management will lie with the instructor (investor). A fax confirmation in the hands of the sender will not be regarded as proof that PSG Asset Management has received a specific document.

• The Manager may borrow up to 10% of the market value of the portfolio where insufficient liquidity exists in the portfolio, or where assets cannot be realised torepurchase, or cancel participatory interests.

• A schedule of fees and charges and maximum commissions is available from PSG Asset Management. Please refer to the fund summary of this application form marked Annexure A for a list of the fees applicable to each of the portfolios.

• The annual management fee is levied monthly on the daily value of the portfolio.

• Commission and incentives may be paid and if so, would be included in the overall cost.

• Distributions are reinvested, unless otherwise instructed, and investors are allocated additional participatory interests in the portfolio.

• Investment statements will be issued quarterly.

• PSG Asset Management will not be liable for any loss incurred due to incorrect information being supplied by the investor.

• Without prejudice to any other rights which PSG Asset Management may have in terms hereof or at law the Investor agrees that PSG Asset Management shall be entitled to recover from the Investor any amount of money paid to the investor, which the investor is not entitled to for whatsoever reason, including interest thereon.

• The investor’s domicilium citandi et executandi for all notices and processes to be given in terms hereof, is the address detailed on this application form or asnotified in writing to PSG Asset Management.

• Should PSG Asset Management be prevented from fulfilling any of its obligations in terms of this application as a result of an event not within the reasonable control of PSG Asset Management, those obligations shall be deemed to have been suspended to the extent that and for as long as PSG Asset Management is so preventedfrom fulfilling those obligations.

• Only emailed instructions sent to [email protected] will be acted on.

• A domestic prominent influential person is an individual who holds including in an acting position for a period exceeding six months, or has held at any time in thepreceding 12 months, in the Republic, a prominent public function including that of: The President or Deputy President, A government minister or deputy minister, The Premier of a province, A member of the Executive Council of a province, An Executive mayor of a municipality, A leader of a political party, the head, accounting officer or chief financial officer of a national or provincial department, all municipal managers, chairperson of the controlling body of a public entity listed to the Public Finance Management Act, an executive officer of a Municipal entity, member of a royal family, a constitutional court judge, an officer of the South African National DefenceForce above the ranking of the major general, an ambassador or high commissioner or other senior representative of a Foreign government based in the Republic, the position of chairperson of the Board of Directors and other executives of the a Company as defined in the Company’s Act, if the company provides goods or servicesto an organ or state and the position of head or other executive directly accountable to that head of an international organization based in the Republic.

• A foreign prominent public official is an individual who holds, or has held at any time in the preceding 12 months, in any foreign country a prominent Publicfunction including that of a, head of state, country or government, member of foreign royal family, senior judicial official, high-ranking member of the military.

P. UNCLAIMED ASSETS

• As a member of the Association for Savings and Investment SA (ASISA), PSG Asset Management follows ASISA’s Unclaimed Assets standard. An unclaimed asset is any unit trust investment on which there have been no transactions for ten years, or in the case of a deceased estate, more than three years have passed since we havereceived the notice of death. PSG Asset Management will continue reinvesting all income distributions and redemption payments until the unit trust is sold, regardless of the time frame.

• PSG Asset Management will follow a process of tracing investors or beneficiaries on any unclaimed assets. Any reasonable administrative costs incurred to trace theinvestor may be recovered from the value of the portfolio, thereby reducing the value of your investment. Tracing will cease where the value of the unit trust investmentis less than R1000 and the costs of tracing exceeds the benefit of tracing.

• To prevent your investment becoming an unclaimed asset, it is your responsibility to inform us of any change in your personal information.

• I consent to PSG Asset Management using, including sharing my personal information, to facilitate the tracing process where required.

Q. PROCESSING OF YOUR PERSONAL INFORMATION

• PSG Asset Management collect personal information about you to process your investment. PSG Asset Management will not share this information outside of the PSG Group, its associated groups or agents, without your consent. To provide you with an effective service, PSG Asset Management may be required to share thisinformation with its administrators or agents who perform certain services for us (e.g. mail houses who post your statements on PSG’s behalf), your financial adviser or broker, as well as with any regulatory bodies as the law requires. PSG Asset Management may share your information with foreign regulatory bodies if required.

• Your information is used for administrative, operational, audit, marketing, research, legal and record keeping purposes. PSG Asset Management will take all reasonable steps necessary to secure the integrity of any personal information which we hold about you and to safeguard it against unauthorised access. You have access to your information at any time and can ask us to correct any information we have in our possession.

Collective Investment Schemes in Securities (Unit Trusts) are generally medium to long term investments. The value of participatory interests (units) may go down as well as up and past performance is not a guide as to future performance. Collective Investment Schemes are traded at ruling prices and can engage in borrowing and scrip lending. The price of participatory interests (units) and the amount of income distributed or accumulated may go down as well as up. The PSG Money Market Fund will maintain a constant price. Money Market is not a bank deposit account.
