Conventional Medical Model Delivery System is Incomplete
• Drivers of health outcomes lie outside of conventional medical model delivery
systems
• The U.S. spends more per capita on medical services than other nations; much
less on social services and safety net programs
o U.S. health care spending grew 4.3% in 2016, reaching $3.3 trillion or $10,348 per person
o Failure to address the social determinants of health increases utilization of services and
increases the cost of care
• New world order:
o Current system’s siloed policies and practices must evolve
o Advancing a health policy framework addressing physical, social, and behavioral
determinants of health would achieve better population health outcomes, reduce health
disparities, and lower costs

Network Hub Definitions
Hub Business Partners-those organizations that will have a stake in the development/design of the hub model/structure; share administrative oversight and governance responsibilities and contribute to defining the hub goals/mission.
Hub Network-other CBOs and or health care organizations (VNA, AAAs, home health, CILs, etc.) that may be part of the subcontracted network utilized to deliver Hub programs and services.
Hub Customers- entities to whom the Hub will be marketing its services for contracting, pilots, demonstrations, etc. and receive payment for services rendered.

Community-based organizations are increasingly banding together to develop collaborative community-
integrated networks to support older adults’ ability to live independently in their communities and develop
health care contracts to improve service delivery. This webinar will focus on the building blocks of
community-integrated networks and critical steps in developing an effective hub, including conducting an
environmental scan, evaluating organizational culture, and formalizing partnership roles and responsibilities.
