The PNP ENTRANCE EXAMINATION is open to all Filipino citizens who possess the age, height andeducational requirements as provided herein, as well as to Police Officers I (P01) whose appointments aretemporary for lack of appropriate eligibility. An Application Form (NAPOLCOM Form 1-A) and Index Card, duly accomplished by the applicantand two (2) recent and identical 1" x 1" ID pictures (in color) with white background and completename tag (indicate rank before the first, middle and last names, if PNP member).2. Police Examination Fee: Four Hundred Pesos (P400.00)

Transcript of Scholastic Record (with Special Order issued by CHED) or Diploma from CHEDaccredited schools, state universities and colleges.5. Birth Certificate issued by the Local Civil Registrar (LCR) or by the main and field offices of theNational Statistics Office (NSO)6. For a P01 with temporary appointment, a certified true copy of Attested Appointment, PlantillaAppointment or Special Order.

V. WHERE TO GET APPLICATION FORMS AND INDEX CARDSApplication Forms (NAPOLCOM Form 1-A) and Index Cards can be obtained for FREE at theNAPOLCOM National Capital Region (NCR) Office located at Jacinta Building II, #1840 Sta. Rita S1.,Guadalupe Nuevo, EDSA, Makati City, at any NAPOLCOM Regional Office nationwide, or may bedownloaded from the NAPOLCOM website at www.napolcom.gov.ph. These may also beREPRODUCED using the original size of the Application Form and Index Card.

VI. HOW TO FILEAn applicant may file his/her application, together with all the required supporting documents,PERSONALLY at any NAPOLCOM Regional or Provincial Office where he/she intends to take theexamination. APPLICATIONS RECEIVED THRU POSTAL MAIL SHALL NOT BE PROCESSED.

VII. NOTIFICATION OF EXAMINATIONA qualified applicant shall be issued a Notice of Admission on the day he/she files hislherapplication for the examination. VIII. EXAMINATION COVERAGEThe three-hour examination consists of 200 multiple choice items. EXAMINATION RESULTS - Report of Rating (RR) will be mailed to the examinee's mailingaddress. Verification may be made with the Examination Division, Personnel and AdministrativeService (PAS}, NAPOLCOM Central Office. An examinee who passed the test but did not receivehis/her RR may request for issuance of a certification from the same office, upon payment of OneHundred Fifty Pesos ( ~ 1 5 0 . 0 0 ) . For requests sent through postal mail, payment shall be in moneyorder form payable to the National Police Commission, # 371 Senator Gil J. Puyat Avenue, Makati City1200.

THE EXAMINATION DIVISION, PERSONNEL ANDADMINISTRATIVE SERVICE, NAPOLCOMCENTRAL OFFICE WILLNOTENTERTAIN REQUESTS FORCHANGE OR MODIFICATION OFANEXAMINEE'S DATE OF BIRTH AS REFLECTED IN HIS/HER REPORT OF RATING, APPLICATIONFORM AND INDEXCARD.

ADDITIONAL INFORMATION:It is understood that all data written by the applicant on the Application Form and Index Card,including information contained in the required submitted documents are considered TRUE ANDCORRECT and shall be treated as official data. AN APPLICANT'S QUALIFICATIONS ARE SUBJECT

TO FINAL REVIEW AND VERIFICATION BY THE EXAMINATION DIVISION, PERSONNEL ANDADMINISTRATIVE SERVICE (PAS). NAPOLCOM CENTRAL OFFICE. APPLICANTS MAY BEREQUIRED TO PRESENT ADDITIONAL DOCUMENTS, IF DEEMED NECESSARY. IF IT ISESTABLISHED THAT THE EXAMINEE DOES NOT QUALIFY FOR THE EXAMINATION, HIS/HERTEST RESULT SHALL BE CANCELLED I INVALIDATED.THE NATIONAL POLICE COMMISSION HAS A COMPUTER SYSTEM THAT KEEPS TRACKOF THE ANSWER PATTERNS OF EXAMINEES. IF TWO OR MORE EXAMINEES HAVE A HIGHPERCENTAGE OF SIMILAR ANSWER PATTERNS, THIS MAY RESULT IN THE INVALIDATION OFTHEIR TEST RESULTS.

ANYONE WHOCOMMITS ANYFORM OF EXAMINATION IRREGULARITY, SUCH AS CHEATING,IMPERSONATION, PERJURY, OR VIOLATION OF EXAMINATION RULES, ETC., SHALLBE DEALT WITHADMINISTRATIVELY AND/OR CRIMINALLY. IN ADDITION, HISIHER EXAMINATION RESULTS ANDELIGIBILITY SHALL BE INVALIDATED.
