What is care and support?

Care and support helps people to be independent, active and healthy throughout their lives. It is about helping people to do day-to-day things, such as living in their home, working, cooking, shopping and caring for their family.

The care and support system includes a wide range of services, for example; meals on wheels, home adaptations, housing support services, support to help disabled people live independently, benefits for disabled people, occupational therapy, day care, care homes and support for carers.

You have a right as a resident to access the whole range of services including leisure, adult education, warm centres and many more provided by Newham Council and its partners.

Adult social care in Newham is changing

The council is changing the way it provides care and support to people who receive social care.

In the old system:

We are changing to a new system. The new system is known nationally as Personalisation. Personalisation is a new way to think about your care needs, support and how they can be provided.

The new system:

•givespeoplemorechoice,controlandflexibility

•youtelluswhatyoufeelisbestsuitedforyourneeds

•wewillsupportyouandgiveyouthemoneytospend

•youmakethechoiceandyouareincontrol.

Why are we changing?

The government wants all local authorities to change their systems. The government asked councils who provide social services to:

•givepeoplemorecontrolovertheirownlife,theirleveloffreedomandtheirchoice of care services

•helppeoplegettherightsupportquickly.Supportshouldbegoodqualityand make the best use of the money

•helppeopleaccesstheservicestheychoosetosuittheirownlifestyles.

As a result, Newham Council is setting up a new way of how you get your care and support. The new system is called Self Directed Support.

What is Self Directed Support?

Self Directed Support is a new way to provide the supportyouneed.Itgivesyoumorechoice,flexibilityand freedom about how your support is organised. Self Directed Support gives you the money to choose and control your social care support. With Self Directed Support:

•youtelluswhatyouneed

•youchoosethesupportyouwantandwhenyou want it

•youareincontrolofthemoneyyougetforyour support

•youchoosehowyouspendthemoney(withincertain rules)

•youareincontrol.

Self Directed Support can be provided either through a Direct Payment or an Individual Budget. Direct Payments are cash only payments given to people instead of social care services. Unlike a Direct Payment, an Individual Budget does not have to be in the form of cash payment only. It can include both cash and services. An Individual Budget can be used to buy support or services from the council, from people in the community, or other organisations.Itcoverssocialcare,communityequipment,disabledfacilitiesgrants and access to work.

Who can receive Self Directed Support?

Anyone who is assessed as needing a social care service may be able to receive Self Directed Support.

The following people can apply:

•olderpeople(anyoneovertheageof65 years)

•peopleaged16yearsandoverwhohavephysical disabilities, learning disabilities, mental health issues or a sensory impairment

•peoplelivingwithHIVandAIDS

•carers

•peoplewithalcoholandsubstanceissues

•peoplewithlongtermhealthconditions.

Different service areas have slightly different schemes; to find out more information you will need to speak to your social worker or care manager.

What is an Individual Budget?

An Individual Budget is money from the council which you can spend on support and services. It gives you greater control and choice of what support is best for you. Individual Budgets can bring together different funds and grants into one payment. The money could come from:

•SocialCareServices

•TheIndependentLivingFund(ILF)

•AccesstoWork

•SupportingPeopleFunding.

A Personal Budget is the same as an Individual Budget. When the money is only from the council, it is called a Personal Budget. In these booklets, we use Individual Budget to mean both Personal Budget and Individual Budget.

What can you use an Individual Budget for?

With an Individual Budget, the money can be spent to meet your needs in different ways. The money you get is based on your Self Assessment Questionnaire.

Individual Budgets cannot be spent on:

•anythingillegal

•gambling

•payingforthingsthatothersourcesofincomewouldnormallypayfor,suchas day-to-day food bills, rent or mortgage payments, utility bills, cigarettes or alcohol

•healthcareneedsthatshouldbemetbythehealthservice.

What is a Support Plan?

A Support Plan is a document which sets out your needs, what is important to you, what you would like to achieve and how you will spend your Individual Budget. It should include how your needs will be met and how you will be safe. The plans are owned by you and must be agreed with your social worker or care manager as a representative of the council.

You can complete the plan yourself or get help and advice from someone you trust – family, friends, independent organisations, or your social worker or care manager.

Here are some other examples and what people said:

Mrs T, aged 80, suffers with restricted mobility and uses a walking stick and stair lift for going up and down her stairs. She has arthritis of the spine, is hard of hearing and was socially isolated. She has a daughter with a disability who lives some distance away who offers support via the telephone. With an Individual Budget Mrs T used her money to buy two days a week visiting a council run day centre and a personal assistant from one of the council’s approved agencies. Mrs T said: “Receiving an Individual Budget has transformed my life.” She has made new friends at the day centre and decides what she wants to buy.

Mr A is 27 years old and a graduate in business management. He has cerebral palsy and lives with his parents. Previously he had a ‘traditional’ care plan and employed a personal assistant to help with personal care in the morning and evening. This included one longer session each week to give his father a chance to socialise with his friends. Now he has an Individual Budget and is using it to continue to employ his personal assistant, who assists him in job searching and accompanies him to do voluntary work. He plans to work towards paid employment and his own home.

How does Self Directed Support work?

There are seven steps you need to follow in the Self Directed Support process.

Step 1 – Self Assessment Questionnaire

You need to contact the council to find out if you are eligible for help with your social care and support needs.

If you are eligible for help, you will be asked to complete a Self Assessment Questionnaire.

You can get help from your social worker or care manager, your family and friends, your carer or an independent organisation.

How long: this process should be completed within 28 days from the date of referral.

Step 2 – Indicative Allocation

You will be given an estimate for the cost of the care and support you may need, based on the answers you give when you fill in the Self Assessment Questionnaire. The estimate is called your Indicative Allocation. This tells you how much the council will give you towards your care.

How long: steps 2, 3 and 4 should be completed within a further 28 days.

Step 3 – Support Planning

You will need to tell us how you want to spend your Indicative Allocation by making a Support Plan. You can get help from your social worker or care manager, your family and friends, your carer, or a broker.

Step 4 – getting your plan agreed

Your social worker or care manager will assess your Support Plan and make sure that your plan is right for you. Once this is agreed, we will arrange for the money to be paid to you. This is your Individual Budget.

How long: steps 2, 3 and 4 should be completed within a further 28 days.

Step 5 – managing your Individual Budget

Your Individual Budget allows you to choose the best ways to meet your support needs and reach your goals. You will need to find the best way to manage your Individual Budget.

You can look after the money yourself or choose someone else to look after the money such as; your social worker or care manager, a friend or family member, a broker, a Trust or a service provider. Your Individual Budget will include the money to pay for this.

Step 6 – arranging your support

You can spend your Individual Budget to get the support you want. This is about living your life your way. You must make sure that the things you choose are written in your Support Plan and it meets your social care needs.

Step 7 – review and learn

You should meet with your social worker or care manager regularly to check if you are happy with the care and support you have chosen. Your social worker or care manager needs to know how your plan is working and how you are spending the money. You can agree changes to your Support Plan at any time.

How long: Your first review visit should be arranged to take place six weeks after the Support Plan starts.

What do these words mean?

Access to Work – is a programme run by the Employment Service(ES)whichprovidessupporttopeoplewithdisabilitiestohelp them find or keep employment.

 

Advocate – a person who helps you to speak up and put forward your views and interests.

  Broker – an expert you pay to help you sort out your Support Plan. Brokerage involves helping you to work out what your choices are and what support you will need to make your choices happen.

 

Criminal Record Bureau (CRB) – an agency of the Home Office which provides access to criminal records information. Organisations can ask the CRB to check applicants for jobs, to see if they have any criminal records which would make them unsuitable for certain work, especially work which involves children or vulnerable adults.

  Deputy or Attorney – the Mental Capacity Act says some people must have a deputy or attorney to make decisions about money, health and support. This is for people who cannot take the decision for themselves.

  Direct Payments – where you are given an amount of money to buy and arrange your care and support services yourself. A Direct Payment allows you to receive money instead of services offered from the council.

  Indicative Allocation – this is an estimate of the amount to be given to you by the council. It tells you how much money you may get towards your care.

 

Individual Budget – is the money you get to spend on what you have put in your Support Plan. It is called an Individual Budget when it is from the council as well as other money.

In Control – a national organisation which supports Self Directed Support.Independent Living Fund – makes payments to disabled people to help them lead a more independent life. You can use payments from the fund to pay a care agency or to employ someone to give you personal and domestic care in your home.Independent Safeguarding Authority – a public body set up by the Home Office. Their role is to help prevent unsuitable people from working with children or vulnerable adults.

  Resource Allocation System (RAS) – is a point system used to calculate how much money you get, based on the level of your needs. It provides a fair and clear system of looking at how much money is given to service users in their Individual Budget.

  Personal Assistant – someone you employ to support you in your home or the community and you pay for the hours they are employed.

Personal Budget – same as Individual Budget but only council money.

  Personalisation – is about people being more in control of the services they receive, whether or not they have an Individual Budget.

 

Service Provider – an organisation that runs services.

  Self Assessment Questionnaire – a form you fill in with information which will be used to work out how much money you can get for your support.

 

Self Directed Support – is a new way to provide the support you need. You decide and you control the money you get for your support.

  Support Plan – is the name for the plan that shows how you will meet your needs, achieve your end result and how you will be spending your individual budget.

 

Supporting People Fund – a Government funded programme which pays for housing related support that makes it possible for vulnerable people to carry on living independently.

The Mental Capacity Act – the Mental Capacity Act 2005 is a law about how people can be supported to make decisions for themselves. It sets out rules for how decisions should be made for people who cannot make decisions for themselves and are said to ‘lack capacity’.

 

Trust – a group of people that the law says can look after your money for you.
