Reactions 1474, p19 - 19 Oct 2013 S Etanercept Anaphylaxis in paediatric patients: 2 case reports A 10-year-old boy and a 15-year-old girl developed anaphylaxis while receiving SC injections of etanercept for juvenile idiopathic arthritis (JIA). The boy, who presented to an emergency department with an allergic reaction, had been receiving etanercept 25mg weekly. Immediately after his fifth injection, he had developed itching and mild periorbital swelling. The next day, he had developed a rash, nausea and difficulty breathing. On examination, he had generalised urticaria, diffuse facial oedema and mild angioedema. Etanercept was withdrawn, and he received epinephrine [adrenaline], sodium chloride, prednisolone, diphenhydramine and ranitidine. His swelling and respiratory distress resolved; however, his facial rash persisted throughout his hospitalisation. At last follow-up, he was receiving methotrexate and adalimumab with no adverse events. The girl began receiving etanercept 20mg twice weekly. Within minutes of her third injection, she developed dyspnoea and a generalised urticarial rash. She received antihistamines and prednisone with rapid improvement of her symptoms, and an allergic reaction was considered likely. Etanercept was withdrawn, and she later received methotrexate and adalimumab without adverse events.
