HEALTH & IMMUNIZATION

In accordance with the Florida State Law requirements for immunizations,

all students entering PRE-SCHOOL 4, KINDERGARTEN, SEVENTH

GRADE and NEW to the SCHOOL,

must submit an updated SCHOOL ENTRY HEALTH EXAM (DH 3040-

Yellow form) along with an updated CERTIFICATION OF

IMMUNIZATION (DH 680 – Blue form) prior to entering class on the

first scheduled day of class.

All Children NEW to Epiphany School must also submit these heath

Records.

The law requires that all students found in non-compliance, will be refused

entrance into class and will continue to be excluded until records can

be obtained and submitted.

Required Summer Reading News

You will receive an order form the end of April/beginning of May for your child’s summer reading order.

Please check off the books that you need to purchase, make a check out to Epiphany School and return the order

form to the homeroom teacher(s) by the due date on the form.

Miss Moravec will collect all the forms and place the orders. You will receive the books by the end of May. They

will be delivered to the classrooms as soon as they arrive.

20% of the sale of the books will go to the school. If you have any questions, please call Miss Moravec through the

school office.

IHM MATH CONTEST 2017

In February, many of our seventh and eighth graders competed in the IHM Math Contest. Epiphany Catholic

School was well represented in the list of preliminary winners.

Within our Eighth Grade, Brianna Munoz earned third place, Patricia Garcia-Saladrigas earned second place,

and Christopher de la Viesca earned first place.

Within our Seventh Grade, Anna Alfano and Isabella Mestre tied for third place, Isabella Zumpano earned

Second place, and Camila Suarez earned first place.

We had three students who qualified to sit for the IHM Final Exam in April by placing in the top 100 of all

students taking the test. In our Seventh Grade, Camila Suarez earned this distinction. In our Eighth Grade,

Christopher de la Viesca and Patricia Garcia-Saladrigas did so.

Both Christopher and Patricia earned Honorable Mention in the IHM Final Exam by placing in the top

20 of all students taking the test.

Congratulations to these fine mathematicians for a job very well done.

Special congratulations to all our math teachers and parents who encourage and challenge our students to do their best.

Duke University Talent Identification Program 2017

Annually, Duke University sponsors a seventh grade Talent Search. Eligible students must have scored 95% or

higher in a sixth grade level standardized achievement test battery. Candidates are invited to complete an

SAT I Reasoning Test in a designated school on a Saturday in December or January. Duke University then provides

The participants with comparative information concerning their academic abilities and resources for unique

educational opportunities.

In our present seventh grade, eighteen students qualified to sit for the SAT. Of these, one earned state recognition, three

earned admission to the Academy for Summer Studies, and one earned admission to the Center for

Summer Studies. Amanda Hayes earned State Recognition. Andres Arguello, Isabel Penin and Camila Suarez

earned Admission to the Academy for Summer Studies. Amanda Hayes earned admission to the Center for

Summer Studies.

This year, we have a student who earned Grand Recognition.

Special congratulations to Amanda Hayes for achieving this honor.

Congratulations to all these academically talented seventh graders. Epiphany School is truly proud of all our TIP

participants. They have distinguished themselves by being members of this respected group.
