batching plants with suitable run-off of wastewater. The wastewater is needed for periodic

environmental monitoring quarterly. Please provide us the evidence of repairing and its

completion.

• There was overspreading excavated soil on site without any proper storage. This soil must store in

designated area for disposal area. For reuse purpose, this soil should store in a temporary place with

barricade and notification broad.

• Access road on site and batching plant were full of dust during transport. To avoid dirty road affecting

by construction activity, Contractor must regularly spray wate r at least 6 times/day, this frequency might

need to increase for either the hot and sunny day or increase of t ransport activity.

• Bentonite bags was spreading over bentonite t anks. This bag should be organized and re-a rranged.

• Bentonite bags left empty on the side of bentonite banks. These empty bags should be stored in

hazardous waste storage on site.

• Domestic waste was found on site and worker accommodation. Contractor must require cleaning activity

for these places and remind your worker to keep clean condition for their environment.

• Metal waste was stored next to the office and worker accommodation without a proper storage; this

material should be stored in a designatP.d <HP.a for with and (label) notification

broad as reference.

• The concrete pile head was found on site without a proper storage. Contractor must collect his concrete

Dear Mr. Huong,

First, we would like to appreciate your corrective action and feedback on our environmental

inspection report 04 Aug 2015 on the letter No. DDIS/CW1 C/0198 dated 061h August 2015. However,

after the inspection in your area, we realized that your construction site has not improved as reported

in your response letter CW1 C/DDIS/0175 on 17th August 2015. Many environmental issues are still

not resolved as mentioned in the attachment of environmental inspection report of 24th August 2015.

Further, we would like to give some comments on your response report/corrective actions as follows:

1. The metal storage area seems more messy and disorganized after your corrective action.

This situation is due to your improper barricades and notification board as well as

disorganized metal waste inside. We suggest you to install proper barricades and notification

board labelled for "metal waste storage" and all wastes should be well-organized and

grouped as to type or use;

2. The temporary spoils disposal area was not provided with barricade and the excavated soil

was dumped spreading everywhere;

3. The concrete waste from batching plant was not attended and therefore no corrective action

has been done

4. The concrete pile heads should not be mentioned "placed in proper location" in your report.

This waste should be fully collected and transferred into the approved disposal area by local

authority under DDIS supervision by site inspector and Environment Specialist.

• There was overspreading excavated soil on site without any proper storage. This soi l must store in

designated area for disposal area. For reuse purpose, this soil should store in a temporary place with

barricade and notification broad.

• Access road on site and batching plant were muddy and slippery during transport. Contractor must

regularly repair the road since this road can cause danger for transport and workers.

• Bentonite bags was spreading all over site. This bag should be collected, organized and stored in

hazardous waste area.

• Bentonite was spreading out inside and outside of construction area due to mishandling during transfer

the bentonite tanks. Contractor is requested to collect all spreading bentonite on site.

• Repairing machine on the batching plant area led to leak fuel on the ground. This area must be clean-

up.

• Domestic waste was found on site and worker accommodation. Contractor must require cleaning activity

for these places and remind your worker to keep clean condition for their environment.

• Metal waste was stored next to the office and worker accommodation without a proper storage; the

storage was messy and poor organized.

• The concrete pile head was found on site without a proper storage. Contractor must collect his concrete

pile head to disposal area as mentioned.

• Toilets for workers were terribly dirty and smelly without sanitary supply (e.g. papers and water basket).

• The improper design of settling pond in the batching plant caused the overspreading of concrete sludge

around batching plant and the settling pond was fully covered with roughly 2 meter depth by this sludge.

• Full of dust on the public road crossing with acess road of construction site cause the low visibility for
