Sharing information to improve the care you
receive
Your Care Connected programme
A simple, yet quite ambitious aim:
To provide a more joined-up NHS service in Birmingham,
Sandwell and Solihull
• To support direct patient care
• To give those treating patients the
information they need when they need it
The record will be:
• Viewed via “lookup” method – no data
warehouse or extraction involved
• Accessed only with the permission of the
patient at the point of care, unless they’ve
opted out
• Completely auditable
Why are we doing this? / Who does this help?
Saving lives• Most accurate patient information available
• The right information when it’s needed
• Reduces duplication of tests
Saving time• Vastly reduces calls from secondary care
• Frees up practice resource
• Improves patient experience
Protecting information• More secure than sending patient information by fax
• Completely auditable
• No data extraction or warehousing involved

IG as the primary driver: the YCC DSA has become a national
exemplar of best practice, assured by two independent expert
barristers, the BMA ethics committee & signed off by Birmingham &
Solihull LMC
Communications & Engagement: using all available channels
to explaining, educating, championing, promoting the initiative
amid many competing priorities
Programme & Governance: designed with strong stakeholder and
partner contribution, based on DSA blueprint recently noted by the ICO
Solution design: DSA driven, a two factor consent model – implied
consent to share, explicit consent at the point of sharing
From proof of concept to operation
A comprehensive & successful proof of concept: building
the interoperability solution, testing the delivery method
Strong focus on data quality: providing testing,
remediation advice and supporting QOF and CQC
Our Change & Direct Mail Campaign: providing patients with
the opportunity to opt out, maximizing patient touch-points
Supporting Practices & Providers: expert central team support
at each step in the process from DSA sign-off to service activation
Current Position: Over 1 million patients have been mailed
and live with 950,000 patients
