PURPOSE OF PRESENTATION
• Address concerns brought to Board of County Commissioners
• Overview of Continuum of Care (CoC) structure
• Update on current CoC initiatives
• Housing Outcomes Snapshot
• Outstanding Need
• Opportunity for County to become involved in the solutions
IS THE SOUTHERN NEVADA COC
• Help Hope Home is the Southern Nevada Homeless CoC, or regional planning body, that coordinates funding for housing and services for families and individuals experiencing homelessness
• Help Hope Home aims to end homelessness in Southern Nevada
• Help Hope Home believes that homelessness should be:• RARE,
• BRIEF, and
• a ONE TIME occurrence
WHAT DOES HELP HOPE HOME DO?
• Drives systemic change
• Promotes communitywide commitment to the goal of ending homelessness
• Applies for and receives annual U.S. Department of Housing and Urban Development (HUD) SNH CoCfunds
• Plans for the SNH CoC’s geographic area
• Designates and operates a Homeless Management Information System (HMIS)
• Operates the SNH CoC’s governance and system operation, including coordinated assessment, written standards, and performance expectations and monitoring
WHY DOES THE COC EXIST?

WHO IS HELP HOPE HOME?
An inter-agency planning body committed to ending homelessness.

CURRENT COC INITIATIVES
• Diversion - to help people identify alternatives to entering shelter and prevent them from becoming homeless. Diversion is a process, not a program.
• Moving On – to become a conduit between supportive housing programs and partnering property owners and landlords to facilitate a coordinated exit strategy for individuals who are stable and no longer need supportive housing, thereby creating new openings for other individuals in need
• Veterans Aftercare – to promote sustainability in permanent housing for formerly homeless veterans in order to avoid recidivism
• Built for Zero – to end chronic homelessness; to establish and oversee a fundraising campaign for the addition of new Permanent Supportive Housing units needed for the identified chronic homeless individuals in the community
