APPLICANT AGREEMENT

I also authorize the references listed above and on my Personal History Statement to provide to the Lower Sioux Indian Community businesses all information concerning my previous employment and any pertinent information they may have. I release all parties from liability for any damage that may occur as a result of furnishing the aforementioned information.

4. In consideration of my employment, I agree to conform to the rules, regulations and policies of the Lower Sioux Indian Community businesses. 5. I understand and agree that, if hired, my employment is not guaranteed for any set period of time and may be terminated at any point without prior notice and without cause, regardless of the date of

payment of my wages or salary. 6. In accepting employment, I agree to be available for any hours scheduled. Schedules are subject to change at any time. 7. I recognize that all records, documents, materials, lists, drawings, books, programs and all other property of the Company made or received by me remain the exclusive property of the Company. 8. I understand that, before I can be formally hired by any Lower Sioux Indian Community-owned business, I will be required to take a test to screen for the presence of drugs and/or alcohol. I also

understand that the testing process will be observed by an individual of the same gender as my own. By signing this application, I consent to take the drug and alcohol test when requested. If the test results are positive, I understand that:

a. I will not be hired; b. I will not be considered for any other jobs at any other Lower Sioux Indian Community-owned business for a period of 12 months; c. The test results will be entered into a database which is accessible to all Lower Sioux Indian Community-owned businesses for a period of 2 years.

All Team Members at Jackpot Junction Casino Hotel are part of the entertainment industry. Each of us has the responsibility of showing our Guests a good time by creating an atmosphere that is safe, friendly and fun! Some of our Team Members work in job positions which place them in direct contact with or Guests on a daily basis. Others have jobs that take place behind the scenes. Being a pleasant, outgoing “people person” is a very important part of all of our employment opportunities. Our Guests and your fellow Team Members expect and deserve this type of treatment.

First impressions are lasting impressions. It is important that your personal appearance and hygiene always give our Guests and other Team Members the best possible impression. No matter how elaborate our facilities may be, our Guests and other Team Members will judge you based on their first impression. Therefore, it is your responsibility to create a favorable image. As you are considering employment with Jackpot Junction Casino Hotel, please be aware of the following Team Member appearance expectations:

UNIFORMS - Team Members may be required to wear a company-provided uniform. Uniforms must be worn according to company standards (i.e. pressed, buttoned, etc.).

CLOTHING - Clothing must comply with the requirements of the position and should be clean, pressed and in good repair. Jeans, leggings and yoga pants are not permitted. With the exception of Special Events and Dacotah Ridge team members, capris and shorts are not permitted. Skirts must be knee-length or longer. Proper undergarments must be worn at all times.

FOOTWEAR - Footwear must be suitable for the position and should be clean, polished and in good repair. Open-toed shoes are not permitted.

HAIR - Hair must be neatly trimmed, combed and held in place. Hair must be worn in a manner that will not block vision or cover ID Badge. Facial hair must be neatly trimmed and maintained; avoid beard stubble.

NAILS - Nails must be clean and neatly trimmed. If used, nail polish should be well-maintained.

COLOGNE/PERFUME - If worn, should be subtle and used in moderation.

HYGIENE - Personal hygiene and cleanliness, including bathing, teeth care and use of a personal deodorant, should be addressed daily.

Weekend and holiday work schedules are normal for all Team Members. It is typical to be required to work evenings, weekends and holidays at Jackpot Junction Casino Hotel. It is imperative that you are at work when scheduled in order to maintain total Guest satisfaction and smooth business operations. It is important that each Team Member has a dependable way of getting to and from work each day.

As you look at our job posting board, you will see that the starting pay rate is listed for each available opening. Team Members receive pay reviews based on the expectations outlined in the job description for their respective positions. If you feel that you are able to meet these expectations, please continue with the application process.
