Requirements for Addition of Spouse Name (Indian National)1. Duly filled application form 2A, and 6 in English.

2. Page 4 to be filled in case name of wife to be added in the husbands passport and page 5 to be

filled in case name of husband to be added in the wifes passport.3. Passport copies of the applicant and spouse.

4. Original passport of the applicant and spouse.5. Joint photograph (applicant and spouse together.)

6. Supporting documents to prove the relationship.7. In case applicants name is not entered in the spouse passport, Original Marriage Certificate to

be produced.8. Original Marriage Certificate duly attested by Home Department of the concerned State

Government in India.9. Photocopy of Marriage Certificate.

10. If marriage has taken place in the Kuwait, Original Marriage Certificate must be duly attested

by the Ministry of Justice and the Ministry of Foreign Affairs of the State of Kuwait.

11. Applicant has to come in person.

Note: If the Marriage Certificate is not attested by the Home Department of the concerned State

Government in India/ Ministry of Justice and Ministry of Foreign Affairs in the State of Kuwait, then

Joint Affidavit Form (Fees: KWD 6.500) has to be filled and signed by applicant and his spouse.

Physical presence of the applicant and his spouse is required in the Embassy at the time of submission

of application

Requirements for Addition of Spouse Name (if spouse is a foreign national)

1. Duly filled application form 2A, and 6 in English.

2. Passport copies of applicant and spouse.

3. Original passport of the applicant and spouse.4. Joint photograph (applicant and spouse together.)5. Supporting documents to prove the relationship.

6. In case applicants name is not entered in the spouse passport, Original Marriage Certificate tobe produced.

7. If marriage has taken place in India, Original Marriage Certificate must be duly attested by theHome Department of the concerned state in India. If marriage has taken place in Kuwait,

Original Marriage Certificate must be duly attested by the Ministry of Justice and the Ministry

of Foreign Affairs of the State of Kuwait.

8. Photocopy of Marriage Certificate.

9. Applicant has to come in person.

Note: If the Marriage Certificate is not attested by the Home Department of the concerned StateGovernment in India/ Ministry of Justice and Ministry of Foreign Affairs in the State of Kuwait, then

Joint Affidavit Form (Fees: KWD 6.500) has to be filled and signed by applicant and his spouse.
