1995 Established the MS Program UT Southwestern Medical Center Currently we follow in excess of 5,000 families 2002-Current Designed the Clincal Center for MS at POB1 I raised funds for the furnishing and equipment costs Served as Director of this Facility since its inception 2010 Designed and raised half of the fundings for the new MS academic and research space on POB 1; 9th floor 1995- My longterm fundraising efforts have been germane to the

building of one of the most prolific MS clinical and research centers in the world today. These efforts have led to many

strategic recruitments of both clinical and research faculty. This funding has also be instrumental in assembling and

retaining one of the most talented multidisciplinary care teams for MS and related disorders anywhere. This team inlcudes physicians, nurses, physician extenders, social workers, technicians, and support staff. Southwestern’s MS Program is widely recognized as among the most successful and significant MS initiatives anywhere.

2001 My authorship of a Congressional Bill led to the

establishment of the first and still sole National MS Treatment Training Program’s in the United States. To date we have trained in excess of 1,000 participants, including about 300 residents (many of whom have gone on to careers in MS and Neuroimmunology).

2011- Establishment of the Total Life Care (TLC) Clinic at UT

Southwestern. This multidisciplinary clinic targets our most afflicted MS patients, with the goal of mitigating morbidity and mortality, while enhancing quality of life. The nurse Director, and co-creator of the TLC, Diana Logan,

recent was the recipient of the Strauss Award at UT Southwestern Medical Center for best nurse.

Fellows and Students

1. Hui-Juan Zhang, M.D., Ph.D. (one of our former Chief Residents) was a clinical fellow in the MS group from July 2000 through June 2001. She was offered a faculty position in the department of neurology at UT southwestern, but declined when her husband was unable to obtain a Texas License. She is currently in private practice in Washington State and directs a comprehensive community based MS Program.

2. Amber Salter, BA, worked in the MS Program during summers while at UT Austin for undergraduate studies. She went on to complete a masters in public health and then joined our group as a research associate from 2005-2008. She entered the biostatistics training program at the University of Alabama in 2008 with Dr. Gary Cutter. She was the recipient of the Best Young Investigator Poster Award at the American Committee for the Treatment and Research of MS (ACTRIMS) in 2007.

3. Of the 250 neurology residents that have completed the US Congressionally sponsored MS Comprehensive treatment training program, 24 decided to pursue either MS or neuroimmunology fellowships. A number of these are now MS Program Directors at academic institutions.

4. We have had a multitude of neurology resident and medical student rotators work with us in the MS Clinical Center or in our various neuroimmunology research laboratories.

5. Heather Winslow, M.D.-Clinical Post-Doctoral Fellow from July 2004 to June 2005. Project involved the development of validation of OCT for optic neuropathy in MS. In collaboration with Laura Balcer (UPENN). Heather was instrumental in establishing the OCT laboratory at UT Southwestern Medical Center in the MS Clinical Center and has served as co-author and principal author on a number of peer review publications. She is completing ophthalmology residency training at UT Southwestern Medical Center.

6. Douglas Woo, M.D. Chief Resident at Medical College of Wisconsin. Served as a National MS Society Funded Fellow at our Center 2005-2006. He returned to the Medical College of Wisconsin to serve as Director of the MS Program as well as Director of the Neurology Residency Training Program.

7. Douglas Mills, B.A. was a student research associate for 2006/2007 from Washington University St. Louis. During his tenure with us, he conducted studies on the changes in eye movement physiology of MS patients during heating and cooling experiements. His work has also focused on the safety implications of these studies on driving and walking. His work was published (he served as first author) in the journal Neurology. Currently he is in medical school in Chicago.

8. Angela Bates, M.D. Chief Resident at UT Southwestern. NMSS fellowship awarded for 2007/2008. Currently Assistant Professor of Neurology at UT Southwestern Medical Center.

9. Wanda Castro, M.D., Chief Resident at U of Connecticut. NMSS fellowship awarded for 2007/2008. Following her role as Instructor and later Asst. Professor of Neurology at UT Southwestern Medical Center, she moved on to become the Director of the MS Program at the University of Connecticut.

10. Gina Remington, BS, RN. – John Dystel NMSS Nursing Fellowship Award- 2007. Currently Senior Research and Clinical Nurse at UT Southwestern Medical Center. Recent recipient of the prestigious UT Southwestern Medical Center Strauss Award.

11. Ryan Winger-an undergraduate summer student from Ohio is focused on laboratory studies involving amyloid protein studies with Drs. Olaf Stuve and Roger Rosenbert.

12. Scott McMclure, a UT Austin undergraduate pre-medical student, he wil complete a summer fellowship with us. His work focused on ocular motor pathophysiology of MS, and on autonomic pathophysiology in MS under the direction of Dr. Scott Davis. He co-authored a research paper withour group based on this work. Currently he is a second year resident in Emergency Medicine at UT Southwestern Medical Center.

13. Erik Burton, M.D., Senior Resident at UT Southwestern. NMSS fellowship awarded for 2008/2009. Recetly awaded a Harvard Partner’s Fellowsip for $120,000 for two years of training. He has also been awarded a TEVA Neuroscience clinical fellowship for 2009/2010. He joined the UT Southwestern Medical Center Scholars Program in 2009/2010. Subsequently he joined the MS Center at the University of New Mexico.

14. Melissa Korkmass, B.A., Ocular motor and autonomic research associate 2007-2010. Melissa will enter the Physician Assistant Program at Duke University in July 2010.

15. Ardith Courtney, D.O., NMSS fellowship awarded for 2008/2009. Following her tenure with us as Asst. Professor of Neurology from 2009 till 2012, she became Director of the MS Program at the University of California at Irvine.

summer fellowship training periods culminated in the following platform presentation at the American Academy of Neurology in 2011 (Hawaii); "Pupillary Reflex Metrics in MS: Objective Characterization of the Afferent Pupillary Defect (APD)". Subsequently Paul served as 1st author on a paper published in Neurology entitled ‘Objective Characterization of the relative afferent pupillary defect in MS.’

20. Adnan Subei, D.O., MS/Neuro-Ophthalmology Fellow from Michigan State, he spent 2009/2010 as a post-graduate (medical school) clinical research fellow before returning back to Michigan State for neurology residency. He helped design and excute a novel study utilizing wavelength restricted light therapy to treat fatigue in MS.

21. Diana Logan, RN, CNP. John Dystel NMSS Nursing Fellowship Award- 2009. Recipient of the first UT Southwestern Medical Center Nurse Practitioner Award. Diana is the architect of the Center’s Total Life Care (TLC) Clinic; a multidisciplinary initiative developed on behalf of our most complex MS patients and their families. She was the recipient of the ‘Best Platform Presentation’ at the 2012 meeting of the Consortium of MS Centers. More recently she was the recipient of the highly coveted Strauss Award for 2012/2013, which recognizes excellence in nursing, patient advocacy, and innovative development of novel programs that seek to better serve our deserving patients.

22. Audrey Frohman-A high school summer research student helped to establish our multifocal visual evoked potential (mfVEP) laboratory in the summer of 2010. Audrey presented her work as a lecture at the weekly Neuroimmunology section meeting at Johns Hopkins in August 2010. Her work in conjunction with Zane Schnurman led to the following presentation at the American Academy of Neurology in 2011 (Hawaii); "Pattern Reversal Multifocal Visual Evoked Potential Timing (Latency) and Amplitude Cortical Responses Are Contingent upon Stimulus Contrast in Patients with Multiple Sclerosis." This work led to a peer-reviewed manuscript published in Neurology, and triggered the lead editorial authored by Dr. Matthew Thurtell (U of Iowa) and Dr. Steven Galetta (Chairman of Neurology, New York University).

23. Zane Schnurman-A first year college student from Brandeis University served as a summer research student working in conjunction with Audrey Frohman on the establishment of our mfVEP laboratory. Their collaborative work resulted in the following presentation at the American Academy of Neurology in 2011 (Hawaii); "Pattern Reversal Multifocal Visual Evoked Potential Timing (Latency) and Amplitude Cortical Responses Are Contingent upon Stimulus Contrast in Patients with Multiple Sclerosis." Zane returned to Brandeis and was nominated for the Barry Goldwater Fellowship Award based on his productive scientific work in our group. He subsequently joined our group as an undergraduate by transferring from Brandeis to the University of Pennsylvania in the fall

of 2011. He puplished his work regarding the optic nerve head component of the mfERG in Neurology 2014;82:1888-1896. He is attending medical school at New York University (August 2013).

24. Audrey Bartosh-A summer research student from Vanderbilt University joined us in 2010 to help facilitate our research investigation entitled Wavelength Restricted Light Therapy for Fatigue in Multiple Sclerosis: A Pilot Study. Following her tenure with us, Audrey transferred to SMU.

25. Carmen Lopez-A summer research student from Notre Dame. Carmen worked in our laboratory during the summer of 2011, 2012, taking on the role of principal coordinator for the STRATIFY study, characterizing the JC virus IgG status of patients being treated with natalizumab. Following graduation from Nortre Dame Carmen returned to serve as a clinical trial coordinator on the ACTHAR vs Methylpredisolone acute Optic Neuritis trial with us for the 2013/2014 academic year. She will attend medical school in the fall.

26. Monica Blazek-A summer high school student who worked in the laboratory of Dr. Nancy Monson for the summer of 2011, and funded by the MS Program. Monica worked on B cell immunology studies related to the immunopathogenesis of MS.

27. Lindsay Horton-A UT Southwestern Medical Student who was awarded a one-year Doris Duke Fellowship, worked in our neuro-ophthalmology research laboroatory for a year (2011-2012) under the direct supervision of Dr. Benjamin Greenberg and my neuro-ophthalmology team (myself, Amy and Darrel Conger, and Teresa Frohman). Her work is focused on the role of aminopyridines in the pathophysiology of MS within the anterior visual system, and was published this year in the journal Neurology, and entitled ‘The Effect of 4-aminopyridine on Vision in MS patients with Optic Neuropathy’. Lindsay was the winner of the 2013 Department of Neurology Prize for best student and the American Academy of Neurology Medical Student Prize for Excellence in Neurology.

28. Ethan Meltzer-A medical student at UT Southwestern Medical Center who has joined with his classmate and collaborator, Peter Squigny, to develop a novel wave-adjustable ligtht-stimulation device for the purpose of activating the retinohypothalamic tract system; and to ultimately develop clinical trial initiatives focused on the treatment of fatigue, depression, and derangements of neuroendocrine reflex arcs, secondary to CNS degenerative disorders such as MS. Ethan presented his work at the Keystone MS meeting in February at Big Sky Montana. Ethan was the recipient of the 2013 Medical Student Research Prize from the Consortium of MS Centers. He, in collaboration with another medical student, Peter Sguigna, presented their work at MS and Neuroimmunology Rounds at Johns Hopkins, and Department of Neurology Grand Rounds at NYU the very next day. Both Ethan and Peter had their abstract accepted

Peter Squigny- A medical student at UT Southwestern Medical Center who has joined with his classmate and collaborator, Ethan Meltzer, to develop a novel wave-adjustable ligtht-stimulation device for the purpose of activating the retinohypothalamic tract system; and to ultimately develop clinical trial initiatives focused on the treatment of fatigue, depression, and derangements of neuroendocrine reflex arcs, secondary to CNS degenerative disorders such as MS. Peter presented his collaborative work with Ethan Meltzer at the Keystone meeting on MS in February 2013, and subsequently at both Johns Hopkins and NYU. Peter was the recipient of the 2013 Medical Student Research Prize from the Consortium of MS Centers. Both Ethan and Peter had their abstract accepted for presentation at the European Committee for Treatment and Research in MS (ECTRIMS) in Copenhagen Denmark, in October of 2013. Ethan and Peter were awarded one of five (out of ~2000 submitted abstracts) “Best MS Research” Prizes for 2013.

30. Ethan Meltzer and Peter Squigna recently were awarded the The American Academy of Neurology; Neurology Award (the first time this coveted award has been bestowed upon two students). The joint award recognizes the unique nature of the collaboration among these two students.

31. Shin Josh Beh-A neurology resident at UT Southwestern Medical Center who has been participating in our research intitiatives since 2011. He recently delivered a platform presentation at the American Academy of Neurology describing his work in our laboratory on the optic nerve head component of the mfERG. He was awarded the Fred Baskin Award for the most promising young investigator at UT Southwestern Medical Center. He also was awarded ‘Best Resident Teacher’. Josh completed a two-year fellowship in July 2012-2014, where he spent 8 months at UT Southwestern Medical Center, 8 months at Johns Hopkins, and 8 months at the New York University. He is the first fellow to be trained within the context of our multicenter vision research collaboration. He is a recipient of a Biogen Idec Fellowship Award for 2012. Having achieved an O-1 VISA, Josh will join our faculty as Assistant Professor of Neurology, and Director of Vestibular Neurology and MS Neuro-Ophthalmology.

32. Marlen and Mirna Lucero-Undergraduate students who worked in our eye laboratory for the summer of 2012 and 2013, focused on mfERG studies. Marlen returned to spend the entire year conducting research with us for 2014-2015.

33. Ashley Frohman, a college senior at Texas Tech this fall, has worked our laboraory this summer, specifically organizing our heating and cooling

studies involving MS patients with a history of acute optic neuritis and internuclear ophthalmoparesis. Ashley recently worked on a monograph and video series on Aquatic Therapy in MS for the MS Association of America (MSAA), where she servied as 1st author. Ashley and her laboratory partner, Derek Sears, are the first exchange students as part of the Dallas-Melbourne Collaborative MS and Cognition Alliance. She and Derek spent the first half of a 12 week summer student Fellowship at the Royal Melbourne University Hospital.

34. Derek Sears, a medical student at Texas Tech Medical School in El Paso. Derek Sears and Ashley Frohman, are the first exchange students as part of the Dallas-Melbourne Collaborative MS and Cognition Alliance. 35. Benjamin Frohman, a recent high school graduate, has joined our summer student research initiative, where he has been working on our melanopsin studies in collaboration with Ethan Meltzer and Peter Sguigna.

36. Sara Qureshi recently joined us from Albert Einstein Medical College in New York, and was awarded a National Multiple Sclerosis Society Fellowship for 2013-2014.

37. Richard B. Dewey, Jr. III is a 1st year medical student who was recently awarded a UT Southwestern Medical Student Summer Research Award. He will be working in our laboratory (Dr. Frohman serving as mentor) on the melanopsin pathway derangements in MS. Richard was recipient of the 2013 Medical Student Research Prize from the Consortium of MS Centers.

Faculty Recruitments

1. Dr. Michael Racke was recruited from Washington University in St. Louis in 1999. He was promoted to full Professor with Tenure while here at Southwestern. He was appointed Chairman of Neurology and Director of the Neuroscience Research Institute at Ohio State University in 2006. He is widely recognized as one of the most accomplished neuroimmunologists in the world today.

2. Dr. Kathleen Hawker-was recruited as a clinician investigator Assistant Professor of Neurology in 2001. She moved to Ohio to get married in June of 2005 and subsequently joined Dr. Michael Racke as an Associate Professor of Neurology at Ohio State University. More recently she has moved to Eli Lilly as a medical science director for MS clinical trials. She served as the principal investigator of the rituximab in primary progressive clinical trial (Olympus Trial).

3. Dr. Olaf Stuve was recruited by Dr. Racke and myself to UT Southwestern in 2004 from UCSF to serve as a physician investigator. His work is focused on neuroimmunology and the development of clinical trial initiatives. He is currently

Associate Professor of Neurology and Neurotherapeutics at UT Southwestern Medical Center, and serves as the Director of the Neurology section at the Dallas VA Hospital.

4. Dr. Anjali Shah was recruited as Assistant Professor of PM&R and Neurology from the University of Washington in Seattle in the fall of 2005 to assume the Directorship of MS Neurorehabilitation. Dr. Shah is a physical medicine and rehabilitation specialist. She completed an MS Fellowship with Dr. George Kraft at the University of Seattle in Washington. She is currently focused on MS Neurorehabilitation.

5. Dr. Scott Davis was recruited as Assistant Professor of Neurology in September of 2006 from the Exercise and Physiology Institute at Presbyterian Hosptial in Dallas. He directs our MS Neurophysiology research program. Dr. Davis has focused on autonomic abnormalities in MS and the role of thermoregulation and its impact upon vision and other areas of functional performance. Currently he is Associate Professor of Physiology at Southern Methodist University, and is adjunct Associate Professor of Neurology and Neurotherapeutics at UT Southwestern Medical Center.

6. Dr. Todd Eager was recruited as Assistant Professor of Neurology in 2006 from UCSF where he worked in Jeff Bluestone’s laboratory on the NOTCH signalling pathway. His work is focused on modeling inflammatory mechanisms of demyelination, neurodegeneration, and neuroprotection and remyelination.

7. Dr. Angela Bates-Flores, our former Chief Resident in neurology (2007), she joined us as an NMSS sponsored clinical fellow, followed by assumming the faculty position of Instructor in 2008. She was subsequently promoted to Assistant Professor.

8. Dr. Ardith Courtney joined our faculty as Assistant Professor of Neurology upon the completion of her NMSS MS Fellowship. She is now Director of the MS Program at the University of California at Irvine.

9. Dr. Wanda Castro joined our faculty as Instructor of Neurology after completing her NMSS MS Fellowship. She is now the Director of the MS Program at the University of Connecticut.

10. Dr. Benjamin Greenberg joined our group as Assistant Professor from Johns Hopkins in January 2009 and serves as the Cain Denius Scholar in MS, and Director of the Transverse Myelitis and Neuromyelitis Optica Programs. He is also Deputy Director of the MS Program and Director of our new Neuroimmunology Biomarker Development Program. He will organize a new hosptial-based neuroimmunology consultation service spanning both adult and pediatric populations. A Pediatric Eye Research Program is soon to be established at the Children’s Hospital of Dallas.

11. Dr. Erik Burton is a former UT Southwestern neurology resident. He is completing an NMSS funded fellowship for 2008/2009. He joined our MS Program faculty as Assistant Professor of Neurology and participated in the UT Southwestern Medical Center Scholars program. In 2012 he joined the University of New Mexico as Assistant Director of the MS Program.

12. Dr. Jeffrey Orstadt is a sleep specialist from Northern California. I assisted the Department of Neurology with his recruitment. Since his arrival he has become involved in research studies focused on sleep derangements in MS.

13. Dr. Donna Graves served as an NMSS funded fellow at our Center for 2009/2010. She joined our faculty in July 2010 as Assistant Professor of Neurology and Deputy Director of the Pediatric Demyelinating Disease Program at the Dallas Children’s Hospital.

14. Dr. Shanan Munoz recently joined our group as an MS clinical specialist. 15. Dr. Darin Okuda recently joined our team as a clinician investigator, and has been

officially recruited as Director of MS Imaging Research, and recently accepted the position of Deputy Director of the MS Program.

16. Teresa C. Frohman, PA-C has the longest standing role both in my clinical care, research, and teaching career. She worked with me to both architect and write the 2000 United States Congressional Bill that established the National MS Treatment Training Program, in collaboration with the National MS Society. From the inception of the Program, Teresa has served as the Executive Director of this National Training initiative. She has also served as the Executive Director of the MS Program and the Clinical Center for MS; since its inception. An author of many peer-review manuscripts, she served as senior Editor of the National MS Society commissioned book entitled “Multiple Sclerosis for the Physician Assistant”. Teresa recently published the first characterization of the optic nerve head component (ONHC) response in MS, in the journal Neurology. This article garnered a lead editorial in the August 6th 2013 issue of Neurology in addition to an on line PodCast from the American Academy of Neurology. She has served as the senior author on what has been widely recognized as the seminal paper on the MLF in ocular motor physiology. Teresa has lectured throughout the world, including delivering Neuroimmunology or Neurology Grand Rounds lectures at KU, Johns Hopkins, NYU, and the University of Iowa. Recently she was invited to lecture at the highly prestigious MS Center at the Brigham and Women’s Hospital at Harvard Medical School. Teresa’s reputation as a passionate, humanistic and dedicated care provider for MS patients and families, and as an emerging clinical investigator and educator is skyrocketing. She was recently invited to lecture on symptom management in MS at the American Academy of Neurology meetings (she will serve as the first PA to serve on the teaching faculty at the AAN). Teresa was recently awarded the coveted UT Southwestern Medical Center Strauss Award, for her excellence in clinical care.

17. Dr. Josh Beh has served as our neurology resident and two-year, 3-Center MS Fellow. Josh was awarded an O-1 Visa in the spring of 2014, and will join our faculty as Asst. Professor of Neurology and Neurotherapeutics, and will serve as Director of Vestibular Neurology & MS Neuro-Ophthalmology
