proximally from the anteroinferior aspect of the lateralepicondyle of the humerus and blending distally with thefibres of the AL [10]. Proximally, the LUCL cannot be sepa-rated from the RCL as it originates at the inferior aspect of thelateral epicondyle [20]. The LUCL runs posteriorly to theradial head, where it may have fibres to the AL, and insertson the tubercle of the supinator crest of the proximal ulna(Fig. 3b) [21]. On MR imaging the LUCL can be constantlyidentified (Fig. 3c). However, in 15-23% ofMR examinationsin healthy volunteers the LUCL was only partially identified[22]. The LUCL prevents the ulna from rotating on its longaxis away from the trochlea and therefore is considered to bethe principal constraint of the elbow joint against posterolat-eral rotatory instability (PLRI) [9, 14]. The origin of thecommon extensor tendon is the secondary soft tissue con-straint of the lateral elbow with a stronger contribution toelbow stability with pronation of the forearm [18].

Medial soft tissue stabilisers

The medial collateral ligament complex is composed of ananterior (a) and posterior (p) bundle as well as the transverseligament (Fig. 4a). The origin of theMCL is the inferior aspectof the medial epicondyle [23]. Anatomically, the pMCL formsthe floor of the cubital tunnel and therefore has a direct rela-tionship to the course of the ulnar nerve. Undistinguishable bytheir morphology, the proximal aMCL and pMCL are bestdiscriminated by their anatomic location and relation to thecubital tunnel on transverseMR images. The aMCL is the onlypart of the collateral ligaments and is composed of a superficialand a deep layer [24]. The superficial layer is anatomicallyseparate from the joint capsule and is considered to be associ-ated with deep fibres of the flexor digitorum superficialismuscle tendon [25]. However, on MR imaging the anteriorbundle can only be identified as one ligamentous structure. OnMR imaging the fibres of the normal MCL show a striated

appearance in about 90 % of the cases, particularly in younghealthy volunteers [22]. Just beneath the fibres of the proximalMCL there is a little fat pad, which can be identified on non-fatsaturated MR images and may be obscured by oedema orhaemorrhage due to acute ligament injury (Fig. 4b) [26]. Littleanatomical variation has been shown in the insertional anato-my of the aMCL, but in most of the cases the ligament insertsflush with the sublime tubercle (Fig. 4c) [25, 27]. The aMCL isthe most important stabiliser against valgus forces. The pMCLinserts at the posteromedial margin of the trochlear notch and isa restraint against internal rotation. As the transverse ligamentspans between the ulnar insertions of the aMCL and pMCL, itis considered to have no relevant function and is fur-thermore not visualised on MR imaging. The overlyingcommon tendon of the flexor and pronator musclesserves as a secondary soft tissue restraint of the elbow,whereas the flexor carpi ulnaris and the flexor digitorumsuperficialis muscles contribute most to valgus stabilityof the elbow [17].

Imaging considerations

Radiography

Conventional radiographs represent the baseline imaging in-vestigation in the evaluation of elbow injuries. Standardanteroposterior and lateral radiographs of the elbow shouldbe obtained to evaluate joint congruency and presence offracture [28]. Additionally, the radial head view may providevaluable information regarding minimally or non-displacedfractures of the radial head, the coronoid process or thecapitellum [29]. It is a modified lateral view of the elbow withangulation of the tube 45° towards the radial head, to imagethe radiocapitellar joint without overlap of other osseousstructures. Independent of radiographic projection, the longaxis of the radius should always bisect the capitellum
