Direct Impact
MEDHOST Enterprise 2019 R1 SR1 makes 2019 eCQMs available, and MEDHOST Business Intelligence provides eCQM Scorecards for 2020. Facilities must use 2019 eCQMs and report one 2020 quarter of their chosen data selection by February 28, 2021. After a facility’s eCQM18 submission is successfully performed for calendar year 2019, MEDHOST recommends immediately transitioning to Reporting Version 20190510.
Overview eCQM Setup
This workflow guide simplifies the facility’s eCQM transition from the 2018 to 2019 version. When utilizing the 2018 reporting version and transitioning to 2019, the facility must:
Install MEDHOST 2019 R1 SR1
Confirm eCQM Mapping Version 20190510
As applicable, work with Excel CQM Data Element Mapping exceptions report via:
• Health Information Management
• Web Application File Maintenance
• Pharmacy
• Patient Accounting

About eCQM Mapping Version
When MEDHOST 2019 R1 SR1 is installed, the mapping version is automatically set to 20190510. Once installation is complete, access Health Information Management Menu to confirm the mapping version. When installing 2019 R1 SR1, the Reporting Version is 20180917, and the following codes are automatically, if applicable, copied from 2018 to 2019 mapping fields:
• Valid CQM 18 lists are copied to CQM 19 lists
• Valid Discharge Status Codes
• Valid Administration Route Codes
• Valid Missed Reason Dose Codes
• Valid Care Standard Override Codes
To access eCQM mapping versions
• Access Health Information Management Main Menu
• Click Meaningful Use Menu
• Click Clinical Quality Measures
• Click CQM Version Selection

The CQM Version Selection prompt displays with the correct values.
• Reporting Version 20180917
• Mapping Version 20190510
NOTE: When the mapping version is 20190510, the facility’s eCQM Mapping selection is current and no change to the mapping version is required. When the mapping version does not display as 20190510, continue to the Working With sections below, as applicable, within this guide.

eCQM Mapping Version transitions begin by accessing the Health Information Management (HIM) system. For example, when the mapping field displays a message such as Not Selected, or a mapping version other than 20190510 the facility must:
• Install MEDHOST 2019 R1 SR1
Modify CQM Mapping Version to 20190510
To modify mapping version
• Access Health Information Management Main Menu
• Click Meaningful Use Menu
• Click Clinical Quality Measures
• Click CQM Version Selection
• Select the Mapping Version
Click Map
Click OK

The facility is now ready to view and assess the CQM 20190510 Mapping Vocabulary Report.
To view the mapping vocabulary report
• Click CQM Vocabulary Report
The CQM Vocabulary Report dialog displays.
• Select the facility’s applicable CQM Data Element
• Click Open in Excel

NOTE: When Reporting/Mapping Vocabulary Code values match within their respective columns, the Reporting Code Valid for Mapping column displays Y. The facility may need, as applicable, to adjust the List Maintenance fields not defined within this Excel Report. Otherwise, the facility can now transition to Reporting Version 20190510 and continue to the Transitioning to 2019 eCQM section in this guide. Before transitioning to Reporting 20190510, the facility must locate the denoted N data element and update the applicable MU mapping code accordingly. Any exception displaying in the column as N must be modified, as applicable, in WAFM, Pharmacy, or Patient Accounting and is dependent upon the facility’s denoted data element.

When the facility is utilizing the 2018 reporting version and transitioning to 2019, the facility must:
• Install MEDHOST 2019 R1 SR1
Confirm facility adjustment to CQM Mapping Version 20190510
Work with facility defined CQM Data Element Mapping exceptions, as applicable in Web Application File Maintenance (WAFM)
To modify missed dose reason code
1. Access Web Application File Maintenance
2. Select eMAR
3. Click Code
4. Select Missed Dose Reason tab
a. The list of missed dose reasons displays.
b. Select the applicable blank MU Mapping – 2019 EH Update Vocabulary field
NOTE: When the MU Mapping – 2019 EH Update Vocabulary is blank, it is because the MU Reporting 2018 EH Vocabulary code did not carry forward and must be modified.

5. Click Edit The MU Mapping/Reporting Vocabulary prompt displays.

7. The Add Vocabulary – 2019 EH Update prompt dialog displays.
8. Select the facility’s applicable vocabulary description
9. Click Accept
The MU Mapping Vocabulary 2019 EH Update field displays with the updated vocabulary.
Click Save
The facility’s MU missed dose mapping vocabulary is saved.

To modify a care standard override reason code
1. Access Web Application File Maintenance
2. Click General Maintenance
3. Click Code The General Code Maintenance tabs display.
4. Select Care Standard Override Reason
5. Click Edit
6. Click 2019 EH Update The Add Vocabulary – 2019 EH Update displays.

To modify list response descriptions
1. Access Web Application File Maintenance
2. Select Patient Care
3. Select Form Creation
4. Click List
List Maintenance displays.
• Select the applicable CQM Description in the list

The List Maintenance prompt displays again, and the MEDHOST Patient Care question list response is updated per facility’s defined CQM response descriptions, as applicable.

The facility is ready to modify to a MU mapping administration route.
To modify a MU mapping administration route
1. Select the applicable administration Route Description

Select the applicable MU Mapping code in the list

To access discharge status codes
1. Select Patient Accounting Menu
• Click File Maintenance Menu II

The Discharge Status Display prompt displays.
The facility is now ready to modify patient’s discharge status codes.

1. Select the facility’s applicable Type discharge status Description
• Click OK
The Display Status Maintenance prompt displays.
Select the MU Mapping drop-down
Select the facility’s MU mapping code in the list

NOTE: MU Reporting 20180917 displays as denoted in 2018 Mapping Report. MU Mapping 20190510 is now complete. The facility can begin transitioning to the 2019 eCQMs.

Transitioning to 2019 eCQM
Now that the facility’s 2019 data element code exceptions are modified, and the following steps are completed:
• Install MEDHOST 2019 R1 MR1
• Confirm eCQM Mapping Version 20190510
• Work with CQM Data Element Mapping exceptions, as applicable:
o Health Information Management Menu (HIM)
o Web Application File Maintenance (WAFM)
o Pharmacy
o Patient Accounting
Facilities can now:
• Transition to eCQM Reporting Version 20190510
To transition to 20190510 eCQM reporting version
• Access Health Information Management Main Menu
• Click Meaningful Use Menu
• Click Clinical Quality Measures
• Click CQM Version Selection
NOTE: There may be workflow discrepancies dependent upon the facility’s menuing Setup. Health Info Mgmt > Meaningful Use Reports Menu >2014 Edition > Version Selection

The facility’s Reporting Version is updated to 20190510 for the 2020 reporting year.
NOTE: The facility Mapping Version correctly displays as Not selected.
Also, the reporting and mapping versions cannot be identical.
Deactivate the CQM 18 Assessments as applicable and complete the transition.
