SUMMARY

Crescent Fashions Ltd. factory, a sister concerned company under Baximco Group, is established in a five storeyed, shared building and four separate sheds. Crescent Fashions Ltd.

occupies ground and first floors, and third floor through fifth floors are occupied by Beximco

Apparels Ltd.

The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd.

(WEC). The purpose of the survey was to identify significant electrical safety issues and to provide

recommendations for remediation based on applicable standards specified by the Accord. The

scope of this initial electrical safety inspection was limited to the review and identification of major

electrical safety issues. The inspection did not include identification of minor deficiencies, which will

be further addressed as part of follow-up inspections.

Table below summarizes the major electrical safety issues identified during the inspection.

Recommendations have been provided to address each issue.

An implementation schedule shall be developed by the factory to remediate each of the

findings. The specific timing of improvements, including any requested extensions due to

design / installation constraints, shall be submitted to the Accord for approval.

FINDINGS AND RECOMMENDATIONS

Finding No: E- 1

Service cable dropping down from 11kV overhead line(left) and laid underground till electrical substation room(right)

Category: SERVICE LINE

Finding:

HT Cable dropping from 11kV OH lineis not firmly supported to the poleand not protected at the base of the pole above ground level.

Recommendation:

HT cable dropping from 11kV pole must be firmly fixed to the pole with supports and clamps and protected by steel/PVC ridged conduit upto minimum of 2 meters above ground level. Remediation Timeframe: 3 month

Finding No: E- 2

Service cable directly burried underground from HT pole to electrical room.

Category: SERVICE LINE

Finding: Service cable buried underground, exposed.

Recommendation: UG cable must be buried underground at proper depth (min 0.5m), installing sand bedding and protective covers. Where necessary cables must be installed with cable route markers.

Two power transformers installed side-by-side nearthe warehouse along the road. One of the transformerssupply to Crescent Fashions and Design Ltd.

Category: TRANSFORMER ROOM

Finding:

Two power transformers installed close to road and adjacent to warehouse, guarded with GI chain linkfence. Recommendation:

Two power transformer must be barred by barrier wall and maintain adequate working space around it.

Remediation Timeframe: 3 month

Breathers of two power transformers filled with silica gel and oil cup attached beneath.


Finding:

Silica gel in transformer breather, deteriorated and oil cup, empty.

Recommendation:

Replace silica gel in breathers and fill the oil cup with as per the manufacturer's instruction.

HT cable connecting power transformer to different switchboard panels laid on ground without protection.


Finding:

HT cable terminating to power

transformer laid on the floor/groundand not supported at termination point.

Recommendation:

HT cable terminating at Transformer

must be firmly supported on riser to

avoid stress at the terminations(transformer bushing) and cables laid on

floor may be protected in cable trenches/

ducts.

Finding:

Locally fabricated manual interlocks arrangement used MCCBoperations. Recommendation:

Locally fabricated manual interlocking system for breakers may be avoided. Breakers must be electrically interlocked and then provide approved and certified mechanical locking system.

Cables inside panel touching bare bus bar whilecrossing it.

Category: SWITCH BOARD & PANELS

Finding:

Cable inside panel touching bare bus bar.

Recommendation:

Cables terminating in the panel must not contact the bare bus bar. Cables inside panels must be firmly fixed through safe routes.

Power cables/wires in the production floor, supported onBBT and temporarily fixed to the wall

Finding:

Sections of distribution cables are not supported and not fixed firmly.

Recommendation:

Power cables and wirings may besupported in cable raceways/trays and securely fixed at regular intervals.

Power cables installed close to steam lines, crossing and touching the steam lines.

Recommendation:

Existing power cables installed alongside the steam line must be covered to protect against external heat from the steam lines. Adequate separetion may be provided from the steam lines.


Finding No: E- 10

Power distribution cables rising from ground floor to first floor without proper supporting structures.


Finding:

Cables dropping down without supports to walls outside building.

Recommendation:

Cables must be supported inladders/trays/ducts and securedlyfixed to supports at regular intervals.

Connection inside distribution panels. Photos are ramdomly picked from different production floors

Finding:

Wiring inside the panel are randomlyconnected to breakers without considering the spatial arrangements of cables and switchgears

Recommendation:

Wiring inside the panel has to be organized neatly and the switchgears must be placed systematically
