SUMMARY

Anam Garments Ltd. factory was constructed in 1997 and production started in 2004. The six storied building was constructed as an industrial structure with total floor area of 16,350.0 sq. ft. and 19.2 meters tall. The building is fully occupied by the factory from ground floor to roof. There are no separate sheds other than the main building. There were 300 workers in the factory during the inspection.

The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd.

(WEC). The purpose of the survey was to identify significant electrical safety issues and to provide

recommendations for remediation based on applicable standards specified by the Accord. The

scope of this initial electrical safety inspection was limited to the review and identification of major

electrical safety issues. The inspection did not include identification of minor deficiencies, which will

be further addressed as part of follow-up inspections.

Table below summarizes the major electrical safety issues identified during the inspection.

Recommendations have been provided to address each issue.

An implementation schedule shall be developed by the factory to remediate each of the

findings. The specific timing of improvements, including any requested extensions due to

design / installation constraints, shall be submitted to the Accord for approval.

FINDING NO: E- 1

Service line drawn from nearby DESCO pole-mounted distribution power transformer using ABC conductors.

CATEGORY:SERVICE LINE FINDING:

LT service line entering building through GI goose neck service mast mechanically stressed with sharp edge of the GI mast RECOMMENDATION:

Provide rubber bushing at the cable stress point with GI goose neck mast. ABC (air bundle conductors) conductors must be anchored firmly to the wall using appropriated sized dead end clamps. PRIORITY: P2 REMEDIATION TIMEFRAME: 5 WEEKS

Breather of 100kVA power transformer showing deteriorated silica gel and empty oil cup

CATEGORY:TRANSFORMER ROOM FINDING:

Silica gel in the transformer breather deteriorated and transformer oil in the oil cup dried up.

RECOMMENDATION:

Change the transformer breather with fresh silica gel and fill the Breather oil cup with transformer oil up to the required level as instructed by the manufacturer. Establish a routine maintenance & inspection program for transformer as well as all other electrical equipment to ensure any future repetition of the occurrence. PRIORITY: P2

CATEGORY:WIRINGS FINDING:

Surface and exposed wiring in generator room and production floors encased in flexible and rigid PVC hose without proper support and laid in angular position.

RECOMMENDATION:

Permanent wiring encased in flexible PVC conduit must be supported with rigid conduits or noncombustible battens attached to vertical walls at orthogonal angles and clamped at regular interval of 600mm. Exposed wirings must be encased in rigid GI or PVC conduits

Exhaust fans controlled by 3P MCCBs in production floors.

CATEGORY:EQUIPMENT & MACHINES FINDING:

Motors having rating exceeding 0.376kW connected directly to MCCB.

RECOMMENDATION:

Motors with rating exceeding 0.376kW must be provided with means to prevent automatic restarting after stoppage due to voltage drop or failure. DOL controller switch may be installed after removing MCCB used for controlling motor.

Wooden distribution cable trays in the production floors. Cut out fuse fixed to wooden tray (left. bottom) and wires inside the cable tray (right bottom). (Typical in all the floors)

CATEGORY:CABLE & SUPPORTS FINDING:

Wooden cable tray used to support cables and wires in production floor. The cable trays are filled with lint and dust and rewire-able cut out fuses used.

RECOMMENDATION:

Replace the entire wooden cable ducts with covered metallic cable trays or channels to avoid fire hazard. The metallic tray must be connected to main earth at one point. Establish a routine cleaning program to keep the channels run over the production floor neat and clean. And the re-wire able cut out fuses must be replaced with MCBs or MCCBs depending upon the load.

CATEGORY:DISTRIBUTION PANELS FINDING:

Bus bars not insulated and multiple cables/wires terminated in single lug. Panel enclosure earthing and door earth bond not provided (typical in most of the panels) RECOMMENDATION:

Live bus-bars in the panel must be adequately insulated with noncombustible insulating material. Multiple wire/cable termination in single lug must be removed and cable//wire must be provided with independent lug and connection point at bus. Panel enclosure and door should be connected to main earthing system to ensure zero potential all the time. PRIORITY: P1

Generator and main system earthing looped together by rough twists and turns and exposed. It is located at the generator room in the ground floor beside the stair case.

CATEGORY:EARTHING SYSTEM FINDING:

Main earth looping exposed and connection done by twisting conductors together without proper bus and bus bar chamber. RECOMMENDATION:

All earth looping must be made from proper sized bus installed in metallic bus-bar chamber. The bus must be insulated with noncombustible material after branch loop tapping. Bus-bar chamber or the enclosure must be located not less than 1ft. above the finished floor level. PRIORITY: P1

Cable termination at Changeover switch and the rusted fastening bolts. It is located in the electrical room at the production floor.

CATEGORY:DISTRIBUTION PANELS FINDING:

Cables connecting to COS not supported. The connection nuts and bolts are rusted and phase barriers between the phases are inadequate.

RECOMMENDATION:

Cables connecting to COS must be firmly secured with cable trays or ladders. The cable trays must always be laid in orthogonal position and not at an angle. Proper bending radius must be ensured at cable bents. The rusted connection nuts and bolts must be replaced with new non-corrosive GI fasteners bolts.
