Definition EBM
Evidence based medicine is the conscientious, explicit, and judicious use of current best evidence in making decisions about the care of individual patients. The practice of evidence based medicine means integrating individual clinical expertise with the best available external clinical evidence from systematic research. By individual clinical expertise we mean the proficiency and judgment that individual clinicians acquire through clinical experience and clinical practice.

Les preuves issues des cinq petits essais contrôlés par placebo inclus dans cette revue ne permettent pas de recommander le recours aux ultrasons dans le traitement des entorses aiguës de la cheville. Les effets potentiels de l'ultrasonothérapie semblent globalement faibles et d'importance clinique probablement limitée, en particulier si l'on considère le temps de rétablissement généralement court de ces lésions. Néanmoins, les preuves disponibles sont insuffisantes pour exclure la possibilité d'effets bénéfiques associés à un calendrier posologique optimal en matière d'ultrasonothérapie.
The evidence from the five small placebo-controlled trials included in this review does not support the use of ultrasound in the treatment of acute ankle sprains. The potential treatment effects of ultrasound appear to be generally small and of probably of limited clinical importance, especially in the context of the usually short-term recovery period for these injuries. However, the available evidence is insufficient to rule out the possibility that there is an optimal dosage schedule for ultrasound therapy that may be of benefit.

CONCLUSIONS: Regarding MT for common lower extremity disorders, there is a level of B (fair evidence) for short-term and C (limited evidence) for long-term treatment of hip osteoarthritis. There is a level of B for short-term and C for long-term treatment of knee osteoarthritis, patellofemoral pain syndrome, and ankle inversion sprain. There is a level of B for short-term treatment of plantar fasciitis but C for short-term treatment of metatarsalgia and hallux limitus/rigidus and for loss of foot and/or ankle proprioception and balance. Finally, there is a level of I (insufficient evidence) for treatment of hallux abducto valgus. Further research is needed on MT as a treatment of lower extremity conditions, specifically larger trials with improved methodology.

There is some clinical evidence that straight leg raises in the early period after cruciate ligament surgery do no harm to the graft or cause other negative outcome.

CLINICAL SCENARIO:The straight leg raise is one of the open kinetic chain exercises that is inquestion of causing graft loosening through too much anterior tibial forces. Early muscular tension of the quadriceps muscle would be important for functional tasks, muscular pump and prevention of loss of muscular pattern and strength. Straight leg raises would be an easy exercise to be performed by the patient on his own.FOCUSSED CLINICAL QUESTION:Do straight leg raises with patients in the early period (two weeks) after anterior cruciate ligament (ACL) surgery have any negative effects on knee stability (transplant loosening, feeling of giving way) or healing (more edema, more pain)?SUMMARY of Search, ‘Best’ Evidence’ appraised, and Key Findings:

However, due to the limited number of high-quality RCTs and differences in the interventions and outcomes utilised within each of the included studies, any conclusions drawn must be interpreted with caution.
