EHR Patient ReportsDeveloping Patient Reports in Allscripts Professional EHR helps to facilitate and support a continuous quality improvement mindset.
Patient Report Examples• Patients who have not reached
their treatment goal based on a lab value (eg, recent HbA1c test) or comorbidity
• Patients who are currently uncontrolled and have not had an HbA1c test in greater than 3 months
• Patients that are due for QDM metrics such as comprehensive foot and eye exams

Navigate to Patient Reports, Segments
If an Active Diabetic Patients segment exists, select it. or Create a new Segment with the following Criteria: • Diagnosis Criterion – Active problem for all applicable Diabetes diagnosis codes • Demographic Criterion - Status = Active • Demographic Criterion - Status >< Deceased
Execute the segment
Reports in Allscripts Professional EHR can be used to identify appropriate candidates not at treatment goal and gaps in care.
The following demonstrates how to run reports based on criteria in Allscripts Professional EHR.
To Access Reports from the Action Menu, Launch the Reporting Module.

Navigate to Patient Reports > Reports, click the “+” icon to add new Report
Link the report to the Active Diabetic Patients segment
Click the “+” to open a Criteria Set.
From Source, Select Labs
Query Example #1 Identify patients not at goal for the HbA1c:
Set the criteria:
Lab Test is Hgb A1c MFr Bld (H...)Value > 7 Add Start and End date
Execute the report.

As in example 1, use or create an Active Diabetic Patients segment
Execute the segment.
Create a new report and link it with the Active Diabetic Patients segment.
Criteria Example #2: Identify patients who have uncontrolled diabetes and have not had an HbA1c test in greater than 3 months
Set the criteria:
• Lab Test is Hgb A1c MFr Bld (H...)
• Value > 7
Check mark negative on each of the following criteria:
• Set Start and End dates to reflect the past 3 months using IS NOT
• Select Lab Test IS NOT Hgb A1c MFr Bld
Execute the Report.

As in example 1, use or create an Active Diabetic Patients segment
Execute the segment.
Create a new report and link it with the Active Diabetic Patients segment.
Set the criteria and check mark negative on each as follows:
• Health Maintenance History, Days Ago IS NOT <= 360
• Health Maintenance History IS NOT Diabetic Foot Exam
Execute the Report.
Criteria Example #3: Identify diabetic patients who have not had a Foot Exam
