8D Notification When an 8D has been issued to your company, the Quality Manager designated in your Supplier Profile will receive notification via e-mail. Below is a sample e-mail:

The quality manager will also receive a second e-mail reminding them of their logon information.

To log into the site, point your browser to http://supplierquality.duraauto.com. Dura will supply you with your username and password. The Dura Quality person who generated an 8D can also supply you with your Username and Password. The username and password are not case sensitive. Enter these in the boxes provided, and click “Login”. If you are experiencing technical problems logging in please contact the system administrator via email – U.S. ([email protected]) or Europe ([email protected]) with a detailed explanation and screenshot of the problem. All correspondence must be in English.

Homepage/Navigation Any dates and times shown on this page are displayed in your local time zone.

The navbar for the application is located in the upper right corner. Going from left to right these links are:

• Home – Returns you to your homepage.

• My Profile – A link to your supplier profile. If the link doesn’t work it is probably due to a pop-up blocker program running on your computer. You can bypass the pop-up blocker issue by accessing your profile directly at http://extranet.duraauto.com.

• 8D Search – The 8D search link takes you to the 8D search page so you can search for an 8D report.

• Logout – Logs you out of the system, and returns you to the Login screen. You must logout before closing your browser. Failure to do so keeps you logged into the site and prevents Dura from accessing the 8D and entering data.

• FAQ – shows frequently asked questions.

• Documents – provides links to various documents for your reference.

Your homepage shows the last 13 months of PPM’s for your facility for all Dura plants combined that you ship to:

8D Search

To find an 8D to edit/complete, open the 8D Search screen and type in a partial or complete 8D number or Part number. You can also use the following filters to search for an 8D: Creation Date range, Close Date range, and Status. You can sort the 8D list by 8D Number or 8D Initiation Date. Select the Search button. A list of 8D’s matching your search is displayed. Clicking the hyperlink of the 8D you wish to edit opens the Edit 8D screen. To prevent two people from opening the same 8D at the same time and overwriting each other’s work, the search screen will only allow one person at a time to open an 8D for writing. All subsequent people will be able to open the 8D for reading, but won’t be able to modify it until that user closes their screen. A pop-up box will appear telling you who has the 8D open.

Edit 8D You will see this form in the language selected by the Dura Quality user that initiated it. All labels, ToolTips, error messages, button labels, etc. have been translated into one of 6 languages: Czech, English, French, German, Portuguese, and Spanish. Once it is issued in a specific language it cannot be changed to another language.

At the very top of the screenshot above, the items in gray can’t be modified by you. The “Raised By” field indicates the name of the Dura person who created the 8D and holding the mouse over the name will show that person’s contact information. The Quantity Affected by Rejection is a total of the defects that this 8D represents. The D2 Problem Statement shows the defects that have been entered by the Dura Quality user. If there is a quantity greater than 0 in this area, you may click on the number to pop up a window which lists the defects and their dates.

The information in the white area in the screenshot above can only be edited by a Dura Quality user. You will only be able to read the information. The “Print 8D Report” button allows you print the current 8D in a printer-friendly format.

All the fields in D1 must be completed before you can move on to the D3 section. The e-mail field must have a valid e-mail address listed. If there are no CFT members then click the Delete button to remove the blank line.

All fields in sections Emergency Response Actions, D1, and D3 must be filled in within 24 hours of the 8D being initiated otherwise the 8D will be considered late and you will lose points on your supplier rating. You will lose points every month the section is incomplete. The system will date and time-stamp the section when it is complete.

All fields in section D4 must be filled in within 5 days of the 8D being initiated.

All fields in section D5 must be filled in within 14 days of the 8D being initiated otherwise the 8D will be considered late and you will lose points on your supplier rating. You will lose points every month the section is incomplete. The system will date and time-stamp the section when it is complete.

The Cost(s) section can only be edited by a Dura user. You will see the information as Read-Only. Dura can add as many charges to the 8D as necessary. Each one is date and time stamped when they were added to this 8D (and as a result, chargebacks may span multiple months). The Related Documents area is available for both Dura and Supplier users. Each user can upload supporting documentation, as well as view any document that has been uploaded to this 8D (whether it was uploaded by Dura or by the Supplier). You can only delete documents that you have uploaded. At this time you are not allowed to upload documents with the following file extensions for security reasons: .exe, .pif, .htm, .html, .js, .vbs, .com, .asp, .aspx, and .bat. Dura will be notified when a new document has been uploaded. Related Documents are not required for the 8D to be

closed (by either user). Ninety (90) days after an 8D is closed by Dura, all documents uploaded for an 8D will be deleted from the site. The 8D itself will remain available. All fields must be filled in and Saved, except for the “Dura Date Closed”, “Reported By”, and “Dura Validation Method” fields (at the bottom of the screen) before you can close the 8D. The Costs section can only be edited by Dura and must have either “yes” or “no” checked off before you can close the 8D. The “yes” designation must have at least one type of cost entered by Dura. A completely closed 8D will be read-only and can only be re-opened by Dura. Once you have completed a session on the site you must click Logout from the navigation bar. Closing your browser without logging out will not allow Dura to update the 8D. Whenever you click the “Save” button, an email is generated alerting the Dura user that created the 8D report that it has been updated. When Dura clicks the “Save” button after their updates you will receive an email alerting you to the update.

Monthly Supplier Emails On the 15th of every month an email will be sent in English language, showing the same table of information shown on your homepage. The line chart is not included in this email. The e-mail is sent to the Quality Manager designated in your Supplier Profile. You can modify this contact information by going to http://extranet.duraauto.com and logging in with the Username and Password issued to your company. The chart will show the previous 13 complete months of PPM’s and your overall Supplier Rating. For example, if today was June 15th, 2004 the email (show below) would include information from May 2003 through May 2004. -------------------------------------------------------------------------------------------------------- The table below details the Dura Quality and Delivery measurements for the past 13 months for ABC Company. If you have any questions regarding this monthly update please forward them to the Dura facility(s) that you ship to or visit the Dura Supplier Quality site at http://supplierquality.duraauto.com.

Late 8D Tracking Emails A late 8D tracking e-mail will be sent out if 8D sections Emergency Response Actions, D1, D3, and/or D5 are not completed within the following deadlines:

• The Emergency Response Actions, D1, and D3 need to be filled out within 24 hours of the 8D being issued. Weekends and holidays are not counted in this 24 hour period. So, for example, if an 8D was initiated on a Friday at 5pm, it would not be considered late until the following Monday at 5pm.

• D5 need to be filled out within 14 days of the 8D being issued. These are 14 calendar days, which means that holidays and weekends are counted. So, for example, if an 8D was initiated on Friday, July 2 at 5pm, it would not be considered late until Friday, July 16 at 5pm.

All fields in sections Emergency Response Actions, D1, D3, and D5 need to be completed within the required deadline otherwise the 8D will be considered late and you will lose points on your supplier rating. You will receive a reminder e-mail every day until these fields are filled in. Below is a sample e-mail that will be sent out if the deadlines are not met.

System Announcements On your homepage you will see a link to the “System Announcements” screen, as well as the date that they were last updated:

The System Announcements screen shows all announcements that the system administrator has entered.

The announcements are sorted in descending order by posting date (so that the most recent one appears first).

Reports There are seven reports defined for the Supplier Quality site: 8D Modification History Report, 8D Report, Chargebacks Report, Concessions Report, Defects Report, Open 8D Summary Report, and Open/Late 8D Summary Report. The Report Engine allows you to generate printer-friendly reports for any data in the system and Excel-friendly reports for the Defects and 8D Summary reports. It will initially prompt you to select which report you would like to generate:

Figure 14 - Reports/Select Report Depending on which report you select will determine the filters that are displayed. Please see the individual reports for more details. You have a choice of viewers, either the older ActiveX or the new HTML, for all reports. We recommend using the HTML format. The HTML viewer has all of the same features as the ActiveX viewer, and it doesn’t require you to download anything to run the reports.

8D Modification History Report This report shows the modifications made to an 8D by Dura:

Figure 15 - 8D Modification History Report The report filters allow you to search for one or more 8Ds by number or by the originator (the Dura Quality user). To search for an 8D by number, enter part or all of the 8D number in the search box provided, and click the “Search” button. This will bring up a list of the matching 8Ds in the box below the search box. Select one or more from the list and click “Add”. Only 8Ds that are added to the “8D(s) selected:” list will be generated. The search by originator works analogously to the search by 8D Number. Once the filters are selected, the modification history report is displayed. If more than one 8D is returned by the filters, then they 8Ds are displayed one after the other.

Figure 18 - 8D Report Filter (Part 2 of 2) The report filters allow you to search for one or more 8Ds by number (using the top set of controls), or by a variety of other filters (using the bottom set of controls). To search for an 8D by number, enter part or all of the 8D number in the search box provided, and click the “Search” button. This will bring up a list of the matching 8Ds in the box below the search box. Select one or more from the list and click “Add”. Only 8Ds that are added to the “8D(s) selected:” list will be generated. To search for an 8D by part number, enter part or all of the part number in the search box and click the “Search” button. The list will be filled with parts that matched what you typed in. Select one or more parts from the list and click on “Add”. Only the 8Ds that contain those part numbers will be generated. This list of 8Ds will override any filters specified by the second set of controls.

To search for an 8D by other means, use the bottom set of controls. This set allows you to search based on one or more of the following filters:

1. Date Range – this filter searches by the date that the 8D was opened.

2. Control System – this filter allows you to quickly add every plant in a particular Control System Group to the list of plants (see the next filter on Plants).

a. Cable Operations Europe – Daun, Gehren, Guarda, Koprivnice, Le Mans, and Timisoara.

b. Shifter System Europe – Daun, Einbeck, Koprivnice, La Talaudiere, and Pamplona.

c. Control Systems Europe – includes all plants from Cable Systems Europe and Shifter System Europe.n

d. Body & Glass Europe – Birmingham, Blatna, Carregado, Plettenberg, Selbecke, Strakonice, Stupava, Timisoara, and Valencia

e. Cable Operations NA – Hannibal North, Matamoros 2, and Milan

f. Brake & Mechanical Assy NA – Gladwin, Jacksonville, Mancelona, and Moberly

g. Shifter & Cable Assy NA – Fremont, Hannibal North, Matamoros 1, Matamoros 2, Matamoros 3, and Milan

h. Body & Glass NA – Lawrenceburg, Matamoros 1, and Queretaro

i. SEAT Group NA – Gordonsville and Stockton

j. Mexico Plants - Matamoros 1, Matamoros 2, Matamoros 3, and Queretaro

3. Plant – this allows you to search for plants by name, and then add one or more of the search results to the “Plant(s) Selected” list. This works similar to the 8D number search. If you select a Control System, all plants in it will be added to this list as well.

4. Problem Code – this allows you to return 8Ds with a specific Problem Code defined.

5. Symptom – this allows you to return 8Ds with a specific Symptom defined.

6. Problem – this allows you to return 8Ds with a specific Problem defined.

7. 8D Status – this allows you to return 8Ds by their status (All/Open/Closed).

Once the filters are selected, the 8D reports are displayed like below. If more than one 8D is returned by the filters, then they 8Ds are displayed one after the other.

Figure 19 - 8D Report The report shows the 8D in the language specified by Dura. Additionally, the report will print a document control number in the lower left corner for 8Ds issued by European plants. This number does not appear for non-European 8Ds.

Open 8D Summary Report This report shows a summary of what sections have been completed and which ones haven’t. It uses all of the same filters as the 8D Report does (see that section for specifics on the filters). The check mark symbol indicates that a section is complete. Blanks indicate that a section is not complete.

• 8D - 8D Number • Plant - Originating Plant • Supplier Name – Supplier that the 8D was issued to • Part Number – Part number on 8D • Defects - Number of defects on 8D • ERA – Emergency Response Action section • D1 – D8 – checks indicate what sections have been completed • Supplier Closed Date – date supplier closed the 8D (reminder: the 8D is

considered Open until the Dura Closed fields have been filled in) You can generate an Excel-friendly version of the same report by clicking “Download Report”. This report shows the information shown on the Preview tab, but in a form that you can open in Excel for further manipulation.

Chargebacks Report This report displays the chargeback information for your 8D’s.

Figure 21 - Chargeback Report Filters The report filters allow you to search based on one or more of the following:

Currency – this allows you to determine which currency to show the report in. All monetary values will be converted to the selected currency.

Cost Incurred Date Range – this filter searches by the date that each cost was entered (each cost has its own date stamp).

Closed Date Range – this filter searches by the date the 8D was closed out.

Control System – this filter allows you to quickly add every plant in a particular Control System Group to the list of plants (see the next filter on Plants).

Plant – this allows you to search for plants by name, and then add one or more of the search results to the “Plant(s) Selected” list. This works similar to the 8D number search. If you select a Control System, all plants in it will be added to this list as well.

Problem Code– this allows you to return 8Ds with a specific Problem Statement defined.

Symptom – this allows you to return 8Ds with a specific Symptom defined.

Problem - this allows you to return 8Ds with a specific Problem defined.

8D Status – this allows you to return 8Ds by their status (All/Open/Closed).

Click on the “View Report” button to view the online report displayed below. Click on “Download Report” to create a tab delimited file for download.

The detailed chargeback report lists out each 8D and its details. For each chargeback entered, the 8D number, date and time, charge type, and cost are listed. The information is sorted in alphabetic order by 8D Number first, and then by date/time entered.

Note: Date and time stamps for chargebacks prior to 7/2/2004 will show the origination date of the 8D, not the date the charge was entered.

Concessions Report This report displays the concession information for your company. It only tracks the # of Concession Granted. It does not track the # of Concessions Denied. This report uses the same set of filters as the Chargebacks report (see that section for more information):

Defects Report Selecting the Defects Report displays the same set of report filters as the Chargebacks report (see that section for more information). When the report appears it will show a summary of your activity. Click on your company name to see the details (see below):

Figure 25 - Defects Report If no information has been entered, then the message of “No data has been entered for this supplier.” will be displayed. The report columns are defined as follows.

1. Total # Items Early/Late – corresponds to the column by the same name on the Add New Defects screen.

2. Total Delivery Defects – tallies up the delivery defects for the date range specified.

3. Total Product Defects – tallies up the product defects the date range specified.

4. Total Receipts – tallies up the receipts entered for the date range specified.

5. On Time Delivery % – this is calculated by taking Total # Items Early/Late, dividing by Total Receipts, and multiplying by 1,000,000.

6. Delivery PPM – this is calculated by taking Total # Delivery Defects, dividing by Total Receipts, and multiplying by 1,000,000.

7. Quality PPM – this is calculated by taking Total # Product Defects, dividing by Total Receipts, and multiplying by 1,000,000.

8. Total 8Ds Raised – shows how many 8D reports were issued for the date range specified.

9. Total Open 8Ds – shows how many 8D’s are open for the date range specified.

10. Total Overdue 8Ds – shows how many 8D’s are overdue. An “overdue” 8D is defined as one where the difference in days between the open date and the D5 Date Implemented is greater than 14.

11. Total Chargeback – shows the sum of chargebacks for the date range specified.

The report calculates a summary line for all the data. This appears on the last page of the report. To return to the main form simply click on the “Preview” tab provided. You can generate an Excel-friendly version of the same report by clicking “Download Report”. This report shows the information shown on the Preview tab, but in a form that you can open in Excel for further manipulation. The report also shows a link to a Chart for your information that meets the search criteria entered. This line chart shows the Delivery and Quality PPM charted on a monthly basis for you for the timeframe defined by the search criteria.

Open/Late 8D Summary Report Selecting the Open/Late 8D Summary Report displays the following set of report filters:

Set the filters for the Date Range, Plant(s), and/or Suppliers you want to see and click View Report or Download Report. Both formats return the same information but can only be run one at a time.

The report will look as follows. The data represents the 8D activity for the Date Range you specified.

Set the filters for the Date Range, Plant(s), and/or Suppliers you want to see and click View Report or Download Report.
