It’s hard to believe another year has come and gone at Duke HomeCare & Hospice.Time seems to pass by so fast but memories of the patients, families, donors andvolunteers we have met over the past year stay with us. These individuals inspire usand uplift us on a daily basis.

We have come a long way since the community banded together more than 30 yearsago to develop a hospice program for local patients and their families. In looking back,

I am impressed by how much a community effort our hospice program has been. Tremendous support fromthe community has continued as we expanded to offer home healthcare and infusion services across theTriangle – providing a continuum of care from curative healthcare to palliative and end-of-life care.

I saw a quote by Henrik Ibsen recently. He said “A community is like a ship: everyone ought to be prepared totake the helm.” In this Annual Report, I hope you will see how so many people from so many different walksof life have taken the helm. They have laughed with us, cried with us and helped to create a nurturingenvironment for all stages of life and healthcare in our community.

In this annual report, you will meet some of those whose lives have been changed by their experiences withDuke HomeCare & Hospice. You’ll meet Sandi and Bill Graham who have inspired both our home health andhospice staff. You’ll also meet Warren Hicks, a police officer who faces his greatest challenge with help fromour home health, hospice and infusion teams. And, we’ll introduce you to volunteers David Kennedy andMarcia Ladd who share a passion for our hospice program.

We also want to share information about the past year with you – the people who have received services, thevolunteers and staff who have given tirelessly of themselves, and the friends who have helped us to innovateand expand our services. Overall, DHCH served: 1,297 Hospice patients and their families; 1,830 Triangleresidents through our Bereavement services; 3,496 patients through our Home Health program; and, 1,839patients in three states through our Infusion services. You’ll also read about how we enhanced painmanagement for hospice patients with an innovative Complementary Therapies program sponsored by the ParkFoundation.

As you read though the 2010-2011 Annual Report, I hope you will take pride in how our community has cometogether to help individuals and families in need. I hope you will also take pride in your own role in our work.Without you, we would not have been able to offer care to patients and comfort to families throughout thecommunities we serve. Your gifts of time, talent and treasure are reflected throughout this report and continueto sustain our good work regardless of a patient’s ability to pay for services.

Thank you for helping us to build a strong foundation for our community and the families who live here. After a lifetime spent supporting the Durhamcommunity by teaching its young children andserving as a mentor to other teachers, Sandi and herfamily were grateful for the support they receivedfrom the community, including the staff at the HockFamily Pavilion.

Bill says that the care that he and Sandi receivedfrom the Hock Family Pavilion team was“extraordinary. At the same time we were dealingwith this horrible inevitability, we were recipients ofincredible care and attention. For Sandi, this was adream come true. And I was the beneficiary as muchas Sandi because the staff took such good care ofme.”

One of the many legacies of Sandi’s life is that hertime with Duke HomeCare & Hospice opened thedoor for other ventilator patients, not only at theHock Family Pavilion but at other hospices whichare developing policies based on those that the DukeHomeCare & Hospice staff implemented.

“I can’t stress enough how important our experienceat the Hock Family Pavilion was,” says Bill. “Itobviously takes extraordinary people to takesomething that is so horrible and make it okay.Sandi died without being in pain, in a room filledwith love, completely at peace.”

Sandi Graham was passionate about nature.Whether she was collecting shells at the beach orwalking along the Eno River, her spirit was liftedby the sights and sounds of the natural world. Soit was fitting that when she passed away in Marchof 2011, she was surrounded by family in a roomat the Hock Family Pavilion with the windowsopen wide, framing a blooming dogwood tree justoutside.

Although she was diagnosed with ALS in 2008,Sandi never allowed herself to be defined by thedisease. A master teacher who spent 35 yearssharing her passion for children and education inDurham elementary schools, Sandi continuedteaching until her illness forced her to retire in2009. Less than a year later, after a health crisisnecessitated a tracheotomy, Sandi and herhusband Bill were introduced to Duke HomeCare& Hospice. A Duke occupational therapist,physical therapist and certified nursing assistantworked with Sandi in her home after her releasefrom the hospital.

“Thanks to those extraordinary women, Sandi wasable to enjoy life in a way that we never wouldhave thought possible due to her illness,” saidBill. “We appreciated their presence and all thatthey did for Sandi.” He says that through thepersistence and care of the physical therapist,“Sandi’s strength came back to such a greatdegree that she could wean herself off theventilator for longer periods of time. That gave usone more Christmas that Sandi could spend withour extended family and one more real Christmaswhen she wasn’t in bed.”

When it became clear the following March thatSandi was going to die, she was determined to goon her terms which included being at the HockFamily Pavilion. Sandi was familiar with the

care provided atthe Hock FamilyPavilion, havingspent time thereduring the lastdays of ateachingcolleague’s life.

A potentialobstacle was thatSandi was on aventilator, achallenge thathad never beenpresented by anyprevious patientat the HockFamily Pavilion.But that didn’tstop DarleneRutledge,clinical managerof hospiceinpatient services,who met with Sandi in the hospital andunderstood her fierce determination to come tothe Hock Family Pavilion. “I came back to ourstaff and said, ‘This woman has seen the care youdeliver and she wants you to take care of her.’”Darlene says that the staff rallied around Sandi’sdesire; within a few days, the staff had becomecompetent on caring for a patient on a ventilatorand had created the policies that needed to be inplace.

“Our staff love what they do and are giganticpatient advocates,” says Darlene. She says that ifa patient wants the care provided by the HockFamily Pavilion team, staff member will alwaysmeet the challenge.

Charity Patient Care Program for those patients who are uninsured orunderinsured to ensure they are able to receive quality end of life care.The Rite Aid Foundation is caring neighbors, involved in communityactivities in meaningful ways and committed to reflecting thediversity of each community they serve.

The Rite Aid Foundation, founded in July 2001, is a not-for-profitfoundation dedicated to helping people in the communities Rite Aidserves lead happier, healthier lives. They support specific programs ofnon-profit organizations that focus on health and wellness in thecommunities in which Rite Aid operates.

tHeraPIeS ProgramThanks to a generous grant from The Park Foundation of $15,000Duke Hospice was able to pilot a Complementary Therapies Programat the Hock Family Pavilion.

The Duke Hospice Complementary Therapies Program combinestraditional palliative healthcare in a hospice setting using holistictherapies to reduce pain, provide better symptom management andreduce the need for pharmacological intervention. These therapiesresult in a better quality of life for the patient and allow for morequality time with friends and family.

Our holistic therapies include: Musictherapy, massage therapy, Reiki (work withenergy fields in the body), Healing Touch,pet therapy and aromatherapy. Many ofthese modalities also benefit familymembers and staff helping them to dealwith stress and loss at a very difficult time.

The Park Foundation was formed in 1966.Its original focus was on education andgrant-making in communities where Park

Communications had interests. TheFoundation is dedicated to the aid and support of education, publicbroadcasting, environment, and other selected areas of interest to thePark family.

VISIon DISaBILItyIn a unique partnership, DukeHospice and LC Industries areworking together to support peoplewho are blind or have a visiondisability through end-of-life careat the Hock Family Pavilion inDurham. LC Industries pledged a$100,000 gift to support patientsserved in our hospice facility.

Duke Hospice hosted a receptionand ribbon-cutting ceremony onWednesday, March 23, at thefacility to name the facility’sfamily room and family kitchen inhonor of the gift from LCIndustries. CEO Bill Hudson(above r) and several boardmembers were honored at the event.”

Headquartered in Durham, NC, LCIndustries currently operates in tenstates, including over 32 retailstores on military bases, eightmanufacturing facilities thatproduce nearly 2,000 differentproducts and two distributioncenters that serve customers fromcoast to coast. All businessoperations support people who areblind or have a vision disability.

Over the past year, Warren has had several staysat the Hock Family Pavilion for painmanagement. Each time, he has been able toreturn home, thanks to weekly visits from aDuke HomeCare & Hospice Home Health nurse.Warren is grateful that home health care allowshim to be at home to enjoy what he says are thethree joys that keep him going: his 12-year-olddaughter, Kristen; his girlfriend, Lauren; and hiscongregation, Oxford United Methodist Church.

“Every one of the nurses who has been here hasbeen absolutely wonderful,” he says. “Whenyou have an IV that you have to be on 24 hours aday, it seems like your life is over. But when thenurse comes out with a smile on her face,making sure that she cares for me thoroughly, Ifeel better. It’s not only the nursing care thatthey give but it’s also the personal care of justbeing here for me that helps.”

WarreNHiCks

Warren Hicks has spent most of his adult lifetaking care of others. After graduating fromUNC Chapel Hill in 1990, he embarked on apublic service career that included seven yearsas a 911 communications telecommunicator anddispatcher, six years as a police officer and morethan a decade as a volunteer firefighter. “Thedesire to help people has always been in myblood,” he says.

When the congestive heart failure he wasdiagnosed with in 1996 progressed to the pointwhere Warren needed to be the recipient of care,rather than the provider, he turned to DukeHomeCare & Hospice and its breadth ofservices. “I had to retire in 2003 because Iphysically couldn’t do the job anymore,” hesays. Six years later, Warren’s illness hadbecome so debilitating that Duke Hospital staffrecommended that he start on infusion therapy.A picc (peripherally inserted central catheter)line was inserted so that he could receivemedication through his veins 24 hours a day.

Nurses from Duke HomeCare & HospiceInfusion Services began visiting Warren weeklyto change the dressing at the picc line insertionsite. “When I started using this medication, Ifelt an almost immediate increase in my energylevel,” says Warren. “My desire to get out anddo things really increased.” Warren says that theDuke HomeCare & Hospice Infusion nurseshelped him get a new lease on life.

Unfortunately, Warren’s condition required in-patient treatment at Duke Hospital. As hisdisease progressed the hospital staff referred himto hospice care in 2010.

“It was a surprise to know that you could go tohospice to get better,” he says. “When mydoctors told me that I needed hospice care, Ithought my life was over.” Warren says thatsoon after he arrived at the Hock FamilyPavilion, he understood that he was placed thereto get his symptoms under control. “The HockFamily Pavilion is a very quiet place withwonderful care,” he says. “I didn’t hesitate toask for anything because all of the staff memberswere so willing to help. They made sure I hadthe pain medicines I needed so I wasn’t in painanymore which was the best thing.”

This past May 21st and 22nd at Camp New Hope inOrange County, 36 campers, ranging in age from 5-14arrived for our 20th annual overnight bereavementcamp. What brought them together was simple; theyhad all recently lost a significant person in their lives.They came together to express how they felt about thisloss and to see that other children were also workingthrough those same feelings.

It was a powerful overnight camp where they enjoyedactivities such as sun-catcher making, scrapbooking,pet therapy, balloon release, roasting marshmallows,basketball and yoga, just to name a few. The camperssaid they were very happy to make new friends andwere glad to be with others who could empathize withtheir grief.

The annual Camp is open to all youth grades K-8thwho have experienced a death in the last two years.We are now accepting applications for next year’sCamp which will be held on May 19-20, 2012 atCamp New Hope. For a registration form, or moreinformation, please call 919.644.6869.

Last December, on a verywindy, cold evening, over 100people gathered to remembertheir loved ones by lightingluminaries in the CarverGardens just behind the UnicornBereavement Center inHillsborough. The brickpathway was lit up withbeautiful luminaries andmemorial placards were placedin front of each luminaryinscribed with thoughtfultributes. Each person could takehome their placard tocommemorate the evening.

This year’s service will takeplace on Tuesday, December13th and luminaries will be lit

from 6:00-7:30 p.m.

Once again this fall our Brick Dedication will take place in Carver Gardens at the Unicorn

Bereavement Center in Hillsborough on Saturday, November 5th at 9:30 a.m. This

service is for those who have purchased a memorial brick this year. A Service of

Remembrance will take place immediately following the brick dedication at 10:30 a.m. We

will only hold one Brick Dedication Service annually and our next one will be held in the

fall of 2012. If you are interested in receiving information or an order form, please contact

David Kennedy hasbeen a long-timevolunteer with DukeHospice. Originallyfrom Thomasville, NC(home of the “BigChair” and Thomas-ville Furniture). Heventured east forcollege where heattended NC State andalso served four yearsin the US Navy. Hisgood friend and fellowhospice volunteer,Kathy Parker, got himinvolved in 2004 by

helping with the Oh, What a Night! Gala and in 2005he became involved with Camp ReLEAF because hehas a passion for helping children as well. Trainedin grief counseling he continues to be involvedthrough our Fall and Holiday ReLEAF programsthat help children. For his day job, David works as aDistrict Service Officer with the North Carolina

Division of Veterans Affairs and helps veterans andtheir families obtain benefits from the federalDepartment of Veterans Affairs.

In his spare time, he enjoys most anything outdoors -especially biking, gardening and outdoor concerts atthe NC Museum of Art. The Symphony Pops andseveral local theater groups get David’s indoorattention.

David says his favorite parts about volunteering atHospice are working with so many wonderful peopleand the things you learn from helping children dealwith their grief. He continues by saying his mostmemorable moment relating to Hospice is theBalloon Release at Camp ReLEAF. You have beenhelping the children work through their emotionsand loss of a loved one all weekend. The BalloonRelease is like graduation: so often the childrenreveal so much at the moment when the balloon isreleased.

Thanks David for being a part of and giving back tothe community and for making such an impact onthose who receive services through Duke Hospice.

Two years ago DHCH started an employee giving program called the

“Give a Piece of Your Heart Campaign”. It is held for two weeks in

February starting around Valentine’s Day. This year, employees

participated across departments on the campaign committee and they

encouraged their co-workers to become involved in the campaign.

Collectively the campaign raised $6,840.50 for the Employee Fund for

Patient Assistance. The purpose of the fund is to provide patients or

family members with stipends up to $500 annually to meet needs that are

outside of their basic medical care. For example, disbursed funds might

be used to provide airfare

for a family member who

would not otherwise be

able to visit a hospice

patient at end-of-life. Or,

funds might be used to

help meet a patient’s

basic needs for food,

clothing, utilities or

shelter.

Here are some examples of those helped this past year:

n A mother of a four-year-old son, who is home bound due to

seizures and leukodystrophy, was unable to pay her power bill due

to lack of employment and applied for assistance from the program.

n A hotel room was paid for when a patient had to stay overnight unexpectedly.

n Helped patient purchase groceries while they were waiting on their disability to be approved.

n The primary care giver was unable to pay utility bills due to unexpected medication costs for his grandmother whose Medicaid renewal application was pending until August and this helped keep their lights and power on during that time.

Thanks to DHCH employees for making a difference in the lives of the

patients we have the privilege of serving!

fifteen years. Her story begins, as

so many of our donor stories do,

with her best friend who had

breast cancer and was in need of

hospice care. So many supporters

have a personal connection to

hospice and the services we

provide to those in the community.

Marcia has been a partner walking

hand-in-hand with Duke Hospice

each step of the way and she says,

“it is a privilege for her to work

with the nurses and patients we

serve.” She goes on to say that

“she continues her partnership

because she knows that with Duke

Hospice and Triangle Aftercare

that we are both the best in the

business.” As so many involved

with hospice, Marcia says her

father-in-law and two of her

drivers have also had family

members who were served by

Duke Hospice. Marcia also

believes that providing services to

those at the end of life is such an

“intimate and sweet thing to do”

and she is so happy to partner with

Duke Hospice.
