Abstract
The three Oligopeptides TP3 (thymotrinan; Arg-Lys-Asp), TP4 (thymocartin;Arg-Lys-Asp-Val), and TP5 (thymopentin; Arg-Lys-Asp-Val-Tyr) are biologicallyactive fragments of the 49 amino acid polypetidethymopoietin (TP). TP3 and TP4 were
undergoing developmentas immunoregulatorytherapeutic agents in humans, whereas a
parenteral dosage form of thymopentin is on the market. A recent study revealed that
orally given TP4 affects TNF-oc plasma levels in rats during septicemia. Other
investigations showed immunoregulating effects of TP peptides after oral gavage in
mice. These immunoregulating properties of peptides are remarkable because the
bioavailabilitiesof many orally applied peptides are rather low due to their enzymaticdegradationand due to their poor permeation properties. In the presentwork, therefore,
the intestinal stabilities and permeabilities of the three peptides were investigated in
order to obtain detailed informationfor the subsequentlyperformeddevelopmentof oral
dosage forms and for their in vivo evaluation in rats.
Colon targeting as means for oral peptide delivery. Chapter I of this thesis
first provides a survey of different strategies to target the colon as release site for oral
dosage forms. Several case histories of colon delivery of therapeutic peptides such as
insulin and calcitonin are presented in the second part of this review. Five different
approaches of colon targeting are presented. The most marketedcolon-targeted dosageforms are based on pH dependent polymers which are designed to be dissolved in the
colon. However, the variabilityof the luminal pH in the small intestine might triggerdrug release before the dosage form enters the colon. The enzymatic and the time-
controlled release of drugs represent more sophisticated approaches which are
influencing the developmentof colonic drug delivery Systems more and more. Even
more recent approaches are based on pressure- or bioadhesion-controlledrelease, but
they need to be further evaluatedin human trials. Colonic delivery of costly peptides is
critical since the colonic permeability of peptides is generally very low. Chemical
peptide modifications, absorption enhancers, and protease inhibitors are promisingpossibilities to increase the colonic permeability, but they must still be shown to be safe
for human use. Furthermore, the animal modeis used in colonic absorptionexperimentsare questionable due to differences in physiology and function between laboratoryanimals and humans. Moreover, high Standard deviations are noticed in colonic
absorption pointing to the difficulties in managing the intersubject absorptionvariabilities.
Intestinal stabilities of the three thymopoietin peptides. In Chapter II the
intestinal stabilities and degradationsof TP3, TP4, and TP5 are investigated. In the
presence of brush border membranevesicles, crude pancreas extract, and evertedringsfrom duodenum, jejunum, ileum, and colon, all peptides were shown to be degradedboth by pancreatic enzymes and brush border aminopeptidases.Degradation clearances
(Cldeg) of TP3, TP4, and TP5 were calculated for a quantitative comparisonof peptidestability. In the presence of crude pancreas extract, there was a rapid degradationof TP5
(Cldeg 17.9 ml/min) in comparison with TP3 and TP4 (Cldeg 0.95 and 0.56 ml/min,
respectively, at 0.2 mM peptide concentration) caused by the cleavage of the C-terminal
tyrosine by carboxypeptidase A, whereas TP3 and TP4 underwent hydrolysis byaminopeptidase N. In the presence of brush border membrane vesicles, the degradationclearances were 3.9, 3.1, and 2.4 ml/min at 0.2 mM concentrations of TP4, TP5, and
TP3, respectively. The clearance of all peptides was lowered with increasing peptideconcentrations, indicating saturable degradation processes. The degradation of the
thymopoietin Oligopeptides in the presence of brush border membrane enzymes was
exclusively catalyzedby aminopeptidase N. The degradationof all peptides was highlydependent on the intestinal segment, with the lowest degradation clearance observed in
the colon.
Intestinal permeabilitiesof TP3, TP4, and TP5. In Chapter III the intestinal
permeabilities of the three thymopoietin fragmentsare assessedto evaluate the peptides'potential for peroral drug delivery. The permeation studies through both Caco-2
monolayers and porcine intestinal tissues were performed in Ussing-type diffusion
Chambers. Additionally, the metabolic degradationof the peptides was investigated with
porcine intestinal tissue in a reflection kinetics set-up. The permeabilitycoefficients
ranged from 0.16*10"7 cm/s to 2.02* 10"7 cm/s and rose with increasing peptideconcentrations. The effective permeabilities in the absorptive direction were higherthanin the secretory direction. However, the permeabilities in the secretory direction were
close to that in absorptive direction, when the aminopeptidase inhibitor puromycin was
present in the compartment facing the apical membrane. Influences of the di- and
tripeptide carrier and of P-glycoproteinon the permeation could be excluded. An

analysis of degradationproducts in the presence of Caco-2 monolayers revealedthat at
the apical membrane TP3, TP4, and TP5 were degraded by sequential cleavage of the
N-terminal amino acids. In the permeation studies with porcine intestinal tissue, no
permeated TP3, TP4, and TP5 could be foundin the receiver compartment. Similarly to
the Caco-2 experiments,the degradationproducts resulted from the metabolic cleavageof the N-terminal amino acids, in case of TP5 there was an additional cleavage of the C-
terminal tyrosine. The results suggest that the enzymatic degradation of the three
studied peptides has a marked impact on the permeabilities of TP3, TP4, and TP5.
Furthermore, the observed low permeabilities of the three studied peptides lead to the
conclusionthat reasonable bioavailabilitiesafter peroral application of the peptides are
not expected without the addition of enzyme inhibitors and/orpermeation enhancers.
Developmentand in vivo testing of oral dosage forms. Based on the results of
the stability and permeability studies, enteric-coated TP4 containing pellets were
developed and tested in rats which is describedin Chapter rV. The aim of this proof-of-concept study was to show enteric availability of TP4 in different intestinal segments.
Microcrystalline cellulose spheres were loaded with an aqueous-ethanolicSolution of
TP4 by solution-adsorption. The obtained pellets were enteric-coated with PMMA
(polymethylmethacrylate,Eudragit®)by a Wurster-type fluid-bed process to protect the
peptide against gastric juice and the enzymes of the upper gastrointestinaltract. The
pharmaceuticalproperties of the batcheswere examined by content uniformitytests anddissolution experiments.The release of TP4 was marginal in acidic medium, but at pH6.0, 93.7% ± 3.4% of the peptidewas released from the pellets coatedwith Eudragit®L100-55 withinone hour. At pH 6.8, 87.9% ± 2.8% of the peptide was released from the
pellets coated with Eudragit®S 100 withinone hour. In vivo tests in rats were conducted
with TP4 by dosing the pellets into the stomachof the rats four days and one day beforea septic shock was induced by intravenousinjection of endotoxin (lipopolysaccharide,LPS) which raised the TNF-a level. Plasma concentrationsof TNF-a were determined
with commercially available ELISA kits. The obtained results show that the different
TP4 dosage forms have no significantimpact on the endotoxin-inducedrelease of TNF-
a indicating that enteric-coated formulationsdid not overcome the barriers for peroraldelivery of TP4.

In summary, the present work embraces both biopharmaceuticalcharacterizations of the three TP peptides and the technological developmentof dosageforms with subsequent in vivo testing. TP3, TP4, and TP5 show susceptibilities in the
presence of intestinal enzymes and low permeabilities which might result in poor
bioavailabilities after peroral administration. Moreover, varioustechniques of stabilitytesting and permeation modeis are presentedin this thesis which can be used to explainand to predict the interactions between absorption processes and concomitant
metabolismin human intestinal tissue. The resulting knowledge is of great importancefor the further developmentof effective and safe oral dosage forms.
ZusammenfassungDie drei Oligopeptide TP3 (Thymotrinan; Arg-Lys-Asp), TP4 (Thymocartin;
Arg-Lys-Asp-Val) und TP5 (Thymopentin; Arg-Lys-Asp-Val-Tyr) sind biologischaktive Fragmente des aus 49 Aminosäuren bestehenden PolypeptidsThympoietin(TP).TP3 und TP4 wurden als Immunregulantien für die Anwendung am Menschen
entwickelt, während sich eine parenterale Arzneiformdes Thymopentin auf dem Markt
befindet. Eine neuere Studie hat gezeigt, dass oral verabreichtes TP4 die TNF-oc-
Plasmaspiegel von Ratten beeinflusst, in denen eine Sepsis durch Injektion von
Endotoxin induziert wurde. Andere Untersuchungen zeigten immunregulierendeWirkungen von TP-Peptiden nach oraler Gabe an Mäusen. Diese immunregulierendenEigenschaften der Peptide sind bemerkenswert, da nach oraler Anwendung die
Bioverfügbarkeitvieler Peptide aufgrund ihrer enzymatischen Degradierung und wegen
ihrer massigen Permeationseigenschaften eher gering ist. Daher wurde in der
vorliegenden Arbeit die intestinale Stabilität und Permeabilität der drei Peptideuntersucht, um detaillierte Informationen für die anschliessend durchgeführteEntwicklungvon oralen Anwendungsformen und deren In-vivo-Evaluationan Ratten zu
erhalten.
Colon-Targeting als Möglichkeit der oralen Arzneistoffabgabe von
Peptiden. Kapitel I dieser Dissertation gibt zuerst eine Übersicht über verschiedene
Strategien, das Kolon als Freisetzungsort für orale Arzneiformenanzusteuern. EinigeFallbeispiele für die Arzneistoffabgabevon therapeutischenPeptiden im Dickdarm wie
z.B. Insulin und Calcitonin werden im zweiten Teil der Übersicht vorgestellt. Fünf
verschiedene Ansätze für das Colon-Targeting werden vorgestellt. Die meisten
vermarkteten Arzneiformen, die das Kolon als Freisetzungsort ansteuern, basieren auf
pH-abhängigenPolymeren, die sich erst im Kolon auflösen sollen. Die Schwankungendes luminalen pH im Dünndarm lösen jedoch die Arzneistoffabgabebereits aus, bevor
die Arzneiform ins Kolon gelangt. Die enzymatisch-kontrollierte und die
zeitkontrollierteFreisetzungvon Arzneistoffenstellen ausgeklügeltere Ansätze dar, die
die Entwicklung von Systemen mit Arzneistoffabgäbe im Kolon mehr und mehr
beeinflussen. Noch neuere Ansätze basieren auf druckkontrollierter oder
bioadhäsionskontrollierter Freisetzung, aber sie müssen in Versuchen am Mensch
weiter evaluiert werden. Die Arzneistoffabgabevon teuren Peptiden im Dickdarm ist

kritisch, da die Permeabilität von Peptidenim Kolon generell sehr gering ist. Chemische
Peptidmodifikationen, absorptionsverbesserndeAgenden und Proteasehemmer sind
vielversprechendeMöglichkeiten, die Permeabilität im Kolon zu erhöhen, aber ihre
Sicherheit für die Anwendung am Menschenmussnoch gezeigt werden. Ferner sind die
Tiermodelle, die in den Absorptionsexperimenten im Kolon benutzt wurden, aufgrundder physiologischen und funktionalen Unterschiede zwischen Labortieren und
Menschen fragwürdig. Ausserdem werden hohe Standardabweichungen in der
Absorption im Kolon beobachtet, was auf die Schwierigkeiten hinweist, die
Absorptionsvariabilität zwischen Versuchstierenbzw. Versuchspersonen in den Griff zu
bekommen.
Intestinale Stabilität der drei Thymopoietinpeptide.In Kapitel II werden die
intestinale Stabilität und Degradation von TP3, TP4 und TP5 genauer beleuchtet. In
Anwesenheitvon Bürstensaummembranvesikeln,Pankreasrohextraktund umgedrehten
Darmwandringen aus Duodenum,Jejunum, Ileum und Kolon wurden alle drei Peptidesowohl durch Pankreasenzyme als auch durch Aminopeptidasen des Bürstensaums
degradiert. Es wurden die Degradierungsclearances (Cldeg) für TP3, TP4 und TP5
berechnet, um die Peptidstabilitäten quantitativ miteinander zu vergleichen. In
Anwesenheit von Pankreasrohextraktwurde eine rasche Degradierungvon TP5 (Cldeg17.9 ml/min) im Vergleich zu TP3 und TP4 (Cldeg 0.95 bzw. 0.56 ml/min bei einer
Peptidkonzentration von 0.2 mM) beobachtet, was durch die Abspaltung des C-
terminalen Tyrosins durch Carboxypeptidase A verursacht wurde, wogegen sich TP3
und TP4 der Hydrolyse durch AminopeptidaseN unterzogen. In Anwesenheit von
Bürstensaummembranvesikelnwaren die Degradierungsclearances 3.9, 3.1 und 2.4
ml/min bei 0.2 mM Konzentrationen von TP4, TP5 bzw. TP3. Die Clearances aller
Peptide sanken mit steigender Peptidkonzentration, was auf sättigbareDegradierungsprozesse hindeutet. Die Degradierung der Thymopoietinoligopeptide in
Anwesenheit von Bürstensaummembranenzymen wurde ausschliesslich durch
AminopeptidaseN katalysiert. Die Degradierung aller Peptide war stark abhängig von
dem jeweiligen intestinalen Segment, wobei die niedrigste Degradierungsclearance imKolon beobachtet wurde.
Intestinale Permeabilität von TP3, TP4 und TP5. In Kapitel IE wird die
intestinalePermeabilität der drei Thymopoietinfragmente untersucht, um das Potential

der Peptide hinsichtlich einer peroralen Arzneistoffabgabe zu evaluieren. Die
Permeationsstudien wurden sowohl mit Caco-2-Monolayerals auch mit intestinalem
Gewebe vom Schwein in Ussing-Diffusionskammern durchgeführt. Zusätzlich wurde
die metabolische Degradierung der Peptide mit intestinalem Gewebe vom Schwein in
einem Reflektionskinetikaufbauuntersucht. Die Permeabilitätskoeffizienten lagen imBereich von 0.16-10"7 cm/s bis 2.02-10"7 cm/s und stiegen mit grösser werdender
Peptidkonzentration. Die effektiven Permeabilitäten waren in der absorptivenRichtunghöher als in der sekretorischen Richtung. Jedoch lagen die Permeabilitäten der
sekretorischen Richtung nahe an denen der absorptiven Richtung, wenn der
Aminopeptidasenhemmer Puromycin im Kompartiment, das der apikalen Membran
zugewandt war, vorlag. Einflüsse des Di-/Tripeptidtransporters und des P-
Glykoproteins auf die Permeation konnten ausgeschlossen werden. Eine Analyse der
Abbauproduktein Anwesenheitder Caco-2-Monolayerzeigten, dass TP3, TP4 und TP5
an der apikalenMembrandurch sequentielle Abspaltung der N-terminalen Aminosäuren
abgebaut werden. In den Permeationsstudien mit intestinalem Gewebe vom Schwein
konnte kein permeiertes TP3, TP4 und TP5 im Empfängerkompartiment gefundenwerden. Ähnlich wie bei den Caco-2-Experimenten resultiertendie Abbauprodukteausder metabolischen Abspaltung der N-terminalen Aminosäuren,im Fall von TP5 gab es
noch zusätzlich eine Abspaltung des C-terminalen Tyrosins. Die Ergebnisse geben zu
erkennen, dass die enzymatische Degradierung der drei untersuchten Peptide einen
deutlichen Einfluss auf die Permeabilitätenvon TP3, TP4 und TP5 hat. Ausserdem
legen die beobachteten geringen Permeabilitäten der drei untersuchten Peptide den
Schluss nahe, dass passableBioverfügbarkeitennach peroraler Anwendung der Peptidenicht ohne den Zusatz von Enzymhemmern und/oder permeationsverbesserndenAgendenerwartet werden.
Entwicklung und In-vivo-Evaluation von oralen Arzneiformen. Basierend
auf den Resultatender Stabilitäts- und Permeabilitätsstudien, wurden magensaftresistentüberzogene TP4-Pellets entwickelt und an der Ratte getestet, was in Kapitel IV
beschrieben wird. Ziel dieser konzeptionsnachweisenden Studie war es, die intestinale
Verfügbarkeit von TP4 in verschiedenen Darmsegmenten zu zeigen. Mkrokristalline
Cellulosepellets wurden mit einer wässrig-alkoholischen TP4-Lösung mittels
Lösungsadsorption beladen. Die erhaltenen Pellets wurden dann in der Wirbelschicht
mit PMMA (Polymethylmethacrylat,Eudragit®) magensaftresistent überzogen, um das

Peptidgegen den Magensaftund gegendie Enzyme des oberen Dünndarmszu schützen.
Die pharmazeutischen Eigenschaften der hergestellten Chargen wurden hinsichtlich
Gleichförmigkeitdes Gehalts und hinsichtlich der Dissolutiongeprüft. Die Freisetzungvon TP4 in saurem Mediumwar marginal, bei pH 6.0 aber wurden 93.7% ± 3.4% des
Peptids aus den mit Eudragit®L 100-55 überzogenenPellets innerhalb einer Stunde
freigesetzt. Bei pH 6.8 wurden 87.9% ± 2.8% des Peptids aus den mit Eudragit®Süberzogenen Pellets innerhalb einer Stunde freigesetzt. Die In-vivo-Tests mit TP4
wurden an der Rattedurchgeführt, indem die Pellets in den Magender Ratte verabreicht
wurden. Dies erfolgte vier Tage und einen Tag bevor der septische Schock durch
intravenöse Injektion von Endotoxin (Lipopolysaccharid,LPS) herbeigeführt wurde,
was die TNF-a-Spiegelansteigen liess. Die TNF-cc-Plasmakonzentrationen wurden mit
einem handelsüblichen ELISA-Nachweis bestimmt. Die erhaltenen Ergebnisse zeigen,dass die verschiedenen TP4-Arzneiformenkeinen signifikanten Einfluss auf die mit
EndotoxinherbeigeführteFreisetzungvon TNF-a haben, was darauf hindeutet, dass die
magensaftresistent überzogenen Formulierungen die Barrieren der peroralen TP4-
Arzneistoffabgabenicht überwindenkonnte.
Zusammengefasst lässt sich sagen, dass die vorliegende Arbeit sowohl
biopharmazeutische Charakterisierungen der drei TP-Peptide als auch die
technologische Entwicklung von Arzneiformen mit deren anschliessenderIn-vivo-
Evaluation umfasst. TP3, TP4 und TP5 zeigen Anfälligkeiten gegenüber intestinalen
Enzymen und geringe Permeabilitäten, die in dürftigen Bioverfügbarkeiten nach
peroraler Verabreichung resultieren dürften. Ausserdem werden in dieser Dissertation
verschiedene Methoden der Stabilitätsprüfung und Permeationsmodelle vorgestellt, die
zur Erklärung und zur Voraussage der Wechselwirkungen zwischen
Absorptionsvorgängen und dabei auftretendem Metabolismus in menschlichem
Darmgewebe genutztwerden können. Das sich darausergebende Wissen ist von grosser
Bedeutung für die weitere Entwicklung von wirksamen und sicheren oralen
Arzneiformen.
