FROM: Caroline Musin Berkowitz RE: Driver Education As in the past, CJHS will host a driver education class by Adams School of Driving on our campus. Classes will meet here after school from 4:10 to 5:10 p.m. on select Monday, Tuesday, and Thursday afternoons throughout the second semester, except for holidays, school breaks, etc. We encourage our families to choose to take driver education here; we have found that having the class over an extended period of time helps our students to balance their homework better than the other scheduling options also provided by Adams. These dates have been selected with great care to minimize conflicts; however, we realize that conflicts will occur. You should work out any conflicts with the driver education instructor, as this is an Adams class located on the CJHS campus, not a CJHS class. Classes will take place on the dates listed below. This year, Adams will not provide a permit trip. Students will be given the permit applications and may go to the DOT as soon as they are 15 years old to obtain their permits. In this attachment, you will find the following documents: an enrollment form, rules and regulations, and a driver education approval form (to be signed by Mr. Scher). Please complete the enrollment forms and send them, along with payment, directly to Adams School of Driving. All driver education fees and paperwork go directly to Adams. Do not bring them to schoolexcept to have Mr. Scher sign your approval form. You will want to submit your enrollment forms and fees as soon as possible, as class begins Monday, February 6. Please also let me know that you are signing up for Driver Education so that we can plan appropriately for space. ORIGINAL WITH A $270.00 DEPOSIT FOR THE ADVANTAGE-PLUS PROGRAM, OR $195.OO FOR

THE BASIC COURSE. THE BALANCE OF $300.00 IS PAYABLE THE FIRST DAY OF CLASS. OF

COURSE, THE COMPLETE COURSE MUST BE PAID IN FULL UPON ENROLLMENT. PLEASE MAKE

CHECKS PAYABLE TO ADAMS SCHOOL OF DRIVING AND SEND TO THE ABOVE ADDRESS.

If a refund is desired, one-week notice prior to the class starting date is required. After that, an administrative

fee of $35.00 will be charged. All class work, behind-the-wheel, and observation must be completed within 5

months from start of class, or student may be subject to any increase in price.

Dear Parents: Please read carefully and sign where indicated below-

I. PAYMENT – The cost of the Advantage Plus Program is $570.00; the Basic Course is $495.00. The balance of either

course is due the first day of class. No student will be allowed to receive his or her permit until the course is paid in full.

A $25.00 fee will be charged on any returned check.

II. ATTENDANCE - is required by the State of Illinois. Only four (4) absences are allowed, and must be made up within 60

days at an Adams classroom convenient to you. Failure to make up classes missed will result in a failing grade. No

refund of any kind will be given since Adams is willing and able to fulfill its obligations to its students. Unexcused

absences or tardiness will result with penalty points assessed on following quiz.

III. PERMIT TRIP – To go for your permit, you MUST be 15 years old and bring with you the following IDs: Make sure that

you get the permit application back from the instructor BEFORE going to the secretary of state office.

IV. BEHIND-THE-WHEEL LESSONS - students will be picked up and dropped off at home, work, or school at no extra

charge. We encourage all students to keep scheduled appointments in order to avoid delays in receiving certification. We

require a minimum of 24-hours notice if a student is unable to keep their behind-the-wheel appointment as

scheduled (a Monday appointment must be cancelled by 2:00 pm on Saturday). The lesson must be cancelled

by a telephone, and by speaking with a phone counselor. Cancellations will not be accepted on line. A

rescheduling fee of $65.00 will be charged if appropriate notice is not provided ($50.00 for a 45 minute

lesson). If a student cannot keep an appointment due to Illness on the day of their lesson, they must call between

7:45 am and 8:00 am in order to avoid a cancellation fee (Sunday appointments must be cancelled by 2:00

pm Saturday). NO EXCEPTION OR DISCUSSION OF THE ABOVE RULE WILL BE TOLERATED! Future

lessons will not be scheduled; any lessons already scheduled will be cancelled until the fee is paid. Payment

must be received within 14 days of missed appointment, or permit may be cancelled. Students must have their

permit with them for all lessons, or the instructor cannot give the lesson and the student will be charged the same as if it

were a broken appointment.

Parents acknowledge that students on the Advantage Plus Program will have private lessons and will be driving alone in

the car with the instructor. At times, students taking the Basic Course may be driving without another student in the

car (i.e.: if going during the school day), but must complete 6 hours of observation.

V. STUDENTS must wait for the instructor at the pre-determined location for 15 minutes and then call the school office.

Suburbs call (847/965-6565), Chicago call (773/262-3430). If the student leaves the pick-up point without first

calling the office, there will be a $65.00 charge. NO EXCEPTIONS!!

VI. CONDUCT - All students are expected to conduct them in an orderly manner. If a student misbehaves or causes any

disruption of the class, parents will be called. If the misbehavior continues, students will be removed from the class and

no refund will be given.

VII. UNPAID tuition or cancellation charges, uncompleted work or examinations, unsatisfactory grades, etc., certification

will be withheld until completion or corrections are made.

Our staff is available to answer your questions or help with problems. For your convenience, our office is open Monday thru

Thursday 7:45 am to 9:00 pm, Friday 7:45 am to 6:00 pm, & Saturday 8:00 am to 5:00 pm, 847-965-6565.
