Secondary Endpoint: New Onset of Diabetes in Patients with Pre-diabetes
Pre-diabetic patients allocated to insulin glargine were 28% less likely to develop
diabetes than standard care participants
(i.e. 25% versus 31%: OR 0.72, 95%CI 0.58, 0.91; P=0.006)

Summary
ORIGIN is a landmark Lantus® trial, the longest and largest, worldwide in a
population at high CV risk, with either pre-diabetes in which insulins are not
currently indicated, or with early T2DM. ORIGIN was designed to assess the
effects of treatment with insulin glargine vs standard care on CV outcomes
Over the more than 6 years follow-up of the ORIGIN study, key findings are:
● Targeting fasting normoglycemia had a neutral effect on cardiovascular outcomes
● Lantus® had no increased risk in cancer incidence
● Early use of once-daily Lantus® let to targeted and sustainable glycemic control
● Majority of patients* with early use of Lantus® did not experience confirmed non-
severe symptomatic hypoglycemia; the rate of severe hypoglycemia was very low
(0.01 episodes per patient-year and 0.003 for standard care); overall hypoglycemia
was 0.1 episodes per patient-year with glargine and 0.027 for standard care)
● Lantus® reduced progression from pre-diabetes to diabetes**
*58% patients ** 28%, statistically significant

Glycemia Reduction Approaches in Diabetes
• Comparará en 7500 pacientes DM2 recientes drug naïve, los 5 tipos de tratamiento: SU, TZD, DPP4i, GLP1 e insulina, agregados a metformina en una combinación inicial o secuencial
• Durabilidad, EA, FRCV, QOL, costo efectividad, complicaciones como microalbuminuria, fenotipos asociados a fallos.
• Pretende identificar el medio más efectivo de tratar la DM2
• Nathan, Lachin, Kahn, Gerztein, Holman, Buse.
Conclusiones • El uso de Insulina debiera ser considerado luego de la
falla a metformina en pacientes con DM2.
• El uso de Insulina está incluido como alternativa en todas las guías de tratamiento universalmente aceptadas (ya sean guías de progresión escalonada o de abordaje simultáneo)
• Se deberá considerar que la Insulinoterapia se asocia con hipoglucemia y aumento de peso. Estos efectos colaterales a su vez se asocian con aumento de mortalidad
• Se deberan considerar objetivos combinados en el momento de generar un descenso de glucosa por lo cual la insulinoterapia deberá ser utilizada en el momento adecuado.
