hese quilts commemorate those who have given the git

of life through their donation of organs and/or tissue.

Each square is created by family members to relect their

feelings about their loved one. he quilt squares are then

sewn together by a professional quilter to form a beautiful,

memorable quilt. he quilts are displayed at special events

in Colorado and Wyoming to honor those who gave the

git of life and encourage people to register to be organ and

tissue donors.

Your quilt piece should be 8” square: your 7” design plus a

½” margin (see pattern on reverse side). Your square can

be made by sewing decorative stitches, appliqué, computer

art, silk screen, paint, drawing, or any other method you

choose. Feel free to include a poem, quotation or other

symbol of your loved one’s life.

Please submit only one quilt square in honor of your loved

one. In order for your quilt square to be displayed, you

must return the signed Quilt Square Release Form with

your completed quilt square. You may send your quilt

square to Donor Alliance at any time.

Thank you for your interest in contributing a square in memory of your loved one. The donor

family quilt enables us to share stories of donor heroes with the community. Each quilt square submitted to Donor Alliance will be sewn into a quilt which will be displayed at various public events, the Donor Alliance web site and may be included in media coverage. You must return this signed release form with your completed quilt square indicating your authorization

for Donor Alliance to publicly display your quilt square.

Please note: If this form is not signed and returned to us, we will be unable to includeyour quilt square in a completed quilt.

I hereby authorize Donor Alliance to use my quilt square in a donor quilt which will befeatured on the organization’s web site and may be displayed in various public venues.
