This includes: The response rate (%) : The percentage of all work area members who were invited to complete the survey and who then successfully submitted a completed survey. Demographic information about respondents : Information about the job categories of participants who completed the survey. Composite Scores : The HSOPS measures 10 aspects of safety culture. Each of these 10 aspects of safety culture is represented by a composite score. Each composite score represents the average of 3-4 specific questions. The HSOPS also measures two outcomes that are represented by a composite score (Overall perceptions of safety and Frequency of event reporting).This report includes scores for each aspect of safety culture listed in the table below, as well as scores on each specific question that makes up the composite score. Composite scores represent the percentage of positive responses (scores of 4 or 5) across related questions. Composite scores can be helpful in understanding safety culture, identifying strengths, and opportunities for improvement.

What is included in this report ? This report summarizes the results of the HSOPS survey. This includes:

The response rate (%) : The percentage of all work area members who were invited to complete the survey and who then successfully submitted a completed survey.

Composite Scores : The HSOPS measures 10 aspects of safety culture. Each of these 10 aspects of safety culture is represented by a composite score. Each composite score represents the average of 3-4 specific questions. The HSOPS also measures two outcomes that are represented by a composite score (Overall perceptions of safety and Frequency of event reporting).This report includes scores for each aspect of safety culture listed in the table below, as well as scores on each specific question that makes up the composite score. Composite scores represent the percentage of positive responses (scores of 4 or 5) across related questions. Composite scores can be helpful in understanding safety culture, identifying strengths, and opportunities for improvement.

Question Scores : This report also includes scores for each question that is used to create the 12 composite scores. Question scores represent the percentage of respondents who felt positively about a particular question; that is, each question score represents the percentage of respondents who scored that question positively (e.g., scored a 4 or 5). Examining question scores can be helpful in understanding why certain dimensions were high or low and can help provide more specific information to inform action planning and improvement.

Outcome Scores : The HSOPS survey includes four self-reported outcome variables.1. Overall perceptions of safety (4 questions): Included in Composite Score graph2. Frequency of event reporting (3 questions): Included in Composite Score graph3. Patient safety grade (1 question): Displayed in a bar chart. Participants are asked to rate their work area on a scale of A (Excellent), B (very Good), C (Acceptable), D (Poor), E

(Failing).4. Number of events reported (1 question): Displayed on a bar chart. Participants are asked to indicate the number of events they have reported in the previous 12 months.

How to read the graphs included in this report

There are three primary types of charts provided in this report.

1. Composite chart: A composite chart is used to display the 10 composite scores, as well as two outcome variables: Overall perceptions of safety and Frequency of event reporting.These charts display labels on the Y-axis and the percentage of positive responses on the X-axis.

2. Likert chat: These charts are used to display the question scores that make up each composite. These charts also display labels on the Y-axis and the percentage of positive responses, the percentage of neutral responses, and the percentage of negative responses to each question on the x-axis.

3. Bar Graph: Bar graphs are used to display results for Patient Safety Grade and Number of Events Reported. These graphs display each response option on the x-axis and the percentage of respondents who chose that response option on the y-axis.
