DOCUMENTARY REQUIREMENTS FOR HIRING A SKILLED WORKERTHROUGH A PHILIPPINE AGENCY

1. Special Power of Attorney executed by the Companys

President/Authorized Representative duly notarized.

2. Recruitment Agreement between the Japanese company and PhilippineAgency duly notarized.

3. Job Order with salary scale executed by the President/AuthorizedRepresentative of the Japanese company.

4. Company Registration (with English Translation) has to pass through thefollowing Japanese government offices:

a. Japanese Notary Public (Koshonin Yakuba)b. Ministry of Foreign Affairs (Gaimusho)

5. Dispatch Permit issued by the Ministry of Health, Labor and Welfare ofJapan (with English translation).

6. Copy of Passport of the Companys President/Authorized Representative.

7. Copy of Philippine Agencys License/Certificate of Renewal issued byPOEA.

8. Employer for Interview at the Office of the Labor Attache, Embassy of theRepublic of the Philippines.

NOTE: Please attached one (1) set of photocopies.
