Exhibit A CDS 200101A

Line Item Description of Work

1 Handicap Ramp and Railings – Front Entry

1) This work requires a Building permit. NOTE: This work shall be completed within 2 working weeks from date owner vacates the home unless other arrangements are made in writing with the Contractor, Owner, and Rehab Specialist.

2) Provide all material, labor, permits and subcontracts necessary to construct a new ADA-style Handicap Ramp and Railings.

3) Provide and install a new concrete landing, approximately 48-inches wide by 42-inches deep +/-, on top of the existing slab, up to the elevation of the interior floor at front entry.

4) Provide and install a new concrete ADA-type Ramp with welded aluminum tubular railings, embedded or securely bolted to the concrete pad and ramp and on side walls of the home. The ramp is to extend from the Front Door Entryway Landing, out and connect to the existing concrete walks, in both directions. One leading to the frontyard and the other along the front elevation of the home, on top of the existing concrete walkways.

5) The slope of the ramp must not exceed 1:12 at any location and if the direction of slope changes, a level landing surface must be between these changes.

6) Upper landing is to be level with the interior floor at the front door but have enough fall away from the home to allow water to run off.

7) All ramps shall have railings constructed on all sides.8) All railings along ramps shall have barriers no greater than 4” above

the top edge of the ramp surface or shall have a curb installed upon the top surface of the ramp, with a minimum width between curbs notless than that of the railings above and at a minimum vertical projection of 1-1/2”.

9) In addition, a rail shall be installed at the median point, vertically, between the lower barrier or curb and upper railing. This rail shall be constructed of aluminum materials placed with the 1-1/2” horizontal projection and running parallel with the upper railing.

10) Provide and install secure Handrails (Aluminum), along/on the walls (Min. 1-1/2” off wall) extending in each direction from the front entry door, to a point no less than 1 foot beyond the sloped area of the ramp. Additionally, provide and install secure Handrails along the outer side of the new ramp, extending in both directions along theramp, to a point not less than 1 foot beyond the sloped area.

11) All grab surfaces (Railings, etc.) are to be free of sharp edges or rough surfaces.

12) All sloped ramp surfaces shall be finished with a non-slip textured finish (“Light Broom-style” or similar) or shall have Non-skid materials applied directly to the surface, to help prevent slipping when moisture may be present. This may be painted on, tape applied, or

mechanically fastened materials, if not incorporated into concrete work.

13) All drawings, sketches or other documents or materials required for permitting or construction of the ramps and railings are the responsibility of the contractor.

2 Bathroom Accessible Rehab

1) This work WILL require a Building and Plumbing permit. NOTE: This work shall be completed within 2 working weeks from date owner vacates the home, unless other arrangements are made and agreed to in writing with the Contractor, Owner, and Rehab Specialist.

2) Bathroom Rehab MUST be completed and ready for client use within 2-weeks from the date the client originally vacates the home.

3) Remove and properly dispose of old pedestal sink, toilet and tub/shower in bathroom.

4) Remove and properly dispose of all simulated wall tile panels in tub/shower area.

5) Remove and properly dispose of drywall or backer materials around perimeter of tub/shower area up to a point no less than 12-inches fromthe ceiling to fully expose the wall cavity around the tub/shower area.

6) Remove and properly dispose of all bathroom floor and base tiles.

7) Remove and replace or properly repair all damaged or rotted studs or sill plates in the exposed walls.

8) Remove and dispose of all old plumbing valves and stops. 9) Cut out concrete slab under old tub/shower and repour a new

recessed/lowered slab to accommodate a flush, “Roll-In type” Shower Area.

10) Relocate and rough-in new drain in proper location, as needed. 11) Make all necessary repairs to existing drain pipes within the exposed

areas. Provide and install new materials if required to ensure all drain pipes are functioning properly.

12) If necessary, hydro-jet or “snake” drain pipes throughout home, including but not limited to tub/shower, toilet, bathroom vanity and kitchen sink, to provide or re-instate free and proper drainage out to “clean-out” or “sewer tie-in”, as needed.

13) Make all repairs and modifications necessary to surrounding walls, ceilings, floors and plumbing to accommodate a new 32”-36” x 60” +/- Roll-In shower and 84” high (Minimum) solid surface or 12” x 24” (minimum) tile surround, including new shampoo/soap niche centered between valve and gooseneck.

14) Provide and install new tiled bathroom floor. Owner to select tile and grout colors from contractor provided selections, but colors shall be lighter/neutral in nature.

15) Provide and install new Delta “Classic” Chrome Lever-style Shower Faucet with Valve and Trim, (gooseneck centered at/about 78”-80” Above Finished Floor, coordinate height with owner), trim to be Chrome finish.

16) Provide and install solid wood backing to accommodate three (3) new

ADA Grab Bars for shower. 42” on large back wall of shower area and a vertical or horizontal 16” to 18” on each end of the shower enclosure (Total of 3 bars to be installed in shower area) Coordinate the exact height and layout with owner prior to installation of backing or grab bars.

17) Provide, install and finish all new cementitious tile backer/wall board in shower stall area (Min. 84-inches up) and new moisture-resistant gypsum wall board on all other exposed walls in bathroom. Provide materials and labor necessary to repair ceilings if/as needed and retexture bathroom, walls and ceiling, with matching texture as needed.

18) Provide and install one large recessed shampoo/soap niche in Front center of shower wall, centered under Goose Neck, for shampoo and soap storage and new matching towel bar in shower area on rear wall.

19) Provide and install 24’ to 30” towel bar on upper rear shower wall. 20) Provide and install new Chrome, Delta showerhead with pull down

hand shower. Model #75490 suggested, OR Equal 21) Provide and install three (3) new ADA Chrome Grab Bars, with

escutcheons, in shower area. One 42” long mounted horizontally, from 33” rear to 36” front in elevation, on large wall and one each 16” to 18” mounted vertically or horizontally at the outer edge of each end of the shower area, at approximately 48” centered AFF if vertical or 33” to 36” if horizontal. Confirm heights and locations with owner before placement of deadwood or final installation.

22) Provide and install new 36” high +/- and approximately 30” wide (wall to wall, if possible), ½” AC plywood, bathroom Vanity cabinet with solid surface top and integrated 18” +/- sink. Cabinet to be finished with Formica-type or other durable finish.

23) Provide and install new standard recessed (if possible), mirrored medicine cabinet with three shelves

24) Provide and install new ¼ turn plumbing stops and supply lines for the hot and cold-water supplies and all required drain pipes.

25) Provide and install new Single Lever, Delta “Classic” Vanity Faucet, (4” center set) in Chrome, matching the shower valve trim.

26) Provide and apply “Kilz-type” mildew/stain resistant primer, OR Equal on walls and ceilings.

27) Provide and apply at least two finish-coats of Low/No VOC, Semi-gloss latex enamel on new Gypsum walls. Sherwin-Williams ProMar 200 OR Equal. Owner to choose color following contract signing.

28) Provide and apply full coverage finish-coat of Low/No VOC, WHITE, Semi-Gloss or Gloss Latex Enamel on all door(s)+ and trim in Main/Hallway Bathroom.

29) Provide and apply a full coverage coat of Sherwin Williams Low/No VOC, Mildew Resistant, Ceiling White, Flat Latex Enamel to ceilings in bathroom.

30) Provide and install new ¼ -turn supply stop and supply line, new wax ring and new flange or stainless-steel flange repair ring (If broken or damaged) and nylon flange bolts (suggested). Insure new toilet is

level and secured properly and does not leak, use matching grout or caulk around base of new toilet at floor.

31) Provide and install new American Standard Cadet, “Right Height”, elongated white 2-piece toilet, WaterSense Certified, 1.28gpf, w/chrome flush lever and seat/lid OR Equal/better.

32) Shower floor tile may be no larger than 2” x 2” in size and shall match the larger floor tiles throughout remainder of bathroom area.

33) Wall tile (12” by 24” minimum size) may be substituted for solid surface walls, with all other listed appurtenances remaining (niche, towel bar, etc.). Owner to select wall surface colors or tile color(s) from contractor provided options.

34) Insure code approved shower pan liner is used (If required by code for roll-in shower) and required slope to new drain is maintained and allinspections are passed.

35) Provide and install additional, new 24” – 30” +/- towel bar, new towel hook on wall behind/near vanity and new toilet paper holder. Chrome finish. Location to be selected by owner.

3 Electrical Rehab and Smoke Detectors

1) This work will require an Electrical Permit

2) If any of the fixtures being replaced need additional materials to make them safe or code compliant, those items are to be considered as INCLUDED in this work. (e.g. New bracket or electrical box, conduit – Flexible or rigid, etc.)

3) All new fixtures/bulbs are to be Energy Star Certified. 4) Provide and install new Bathroom lighting centered above new vanity

sink. New fixture is to be 3-bulb, “tulip-style” fixture (2-bulb only if necessary to fit) with changeable LED, 60-watt equivalent bulbs. Finish to be chrome.

5) Provide and install new LED, Vapor-Sealed/Wet Location approved Recessed-Can or other type light over shower area, per code. Provide and install new switch to operate this fixture.

6) In Bathroom, Provide and install, on new code compliant, dedicated Circuit Breaker(s), new 80CFM bathroom vent fan/LED light. If necessary, provide and install new wall switch(s) for fan and light functions. Suggested model Delta Breez “Green Builder” #80HLED or Equal or Better

7) In Bathroom, Provide and install new 4” semi-rigid aluminum duct from new exhaust vent fan and duct to the exterior of the home per code. If necessary, provide and install code approved exterior vent damper/termination in roof.

8) Provide and install new CO/Smoke Combo Alarms, throughout home. Minimum, one Smoke Alarm in each bedroom/sleeping space and one Combo CO/Smoke Alarm in each common-area immediately adjacent to sleeping space(s) per code. Also, provide and install a combination CO/Smoke alarm in room located between garage and Living Room. ALL Smoke and CO/Smoke Alarms MUST be Interconnected, hardwired (Where possible), on dedicated Arc Fault Breaker and, must

have 10-year, non-serviceable, battery backups. (If hardwired alarms are NOT possible/practical, the devices may be operated by a 10-year, not serviceable battery only, but MUST be Interconnected with all other smoke/CO Alarms throughout the home)

9) Check all existing snap switches, pull-chain switches and receptacle outlets for proper function. Provide and install new switches and receptacle outlets in place of any defective or burned devices found, including broken or damaged cover plates, color to match existing devices.

10) Provide and install new weather rated, GFCI, exterior receptacle outlets on front and rear of home if not currently present, and if necessary, an additional outlet within 25 feet and within sight of HVAC Condenser Unit on South side of the home.

11) Provide and install GFCI Outlets or breakers for ALL Outlets in Bathroom, Kitchen, Laundry/Utility room and Garage area and other “wet” areas, including exterior of home, as current NEC 2014 electrical code requires, if not currently present.

12) Remove and dispose of all non-functional, damaged or unsafe light fixtures and/or ceiling fans throughout the interior and exterior of the home, as needed.

13) Provide and install new LED Light fixtures suitable to area being serviced, with customer-changeable LED bulbs (when possible), in all locations that old, non-functioning fixtures are removed or where anew light is required per code.

14) Provide and install new Flush Mount 52” Ceiling Fan/LED Light (changeable bulbs) combo units in each bedroom. Provide and install appropriate ceiling fan mounting bracket/box, as required by code. Fans shall be capable of reverse air-flow and at least 3000 CFM’s of airmovement. Fans shall be operated by remote controls to be mounted on wall near switch. Fans shall be white,

15) Provide and install new exterior rated, LED Flush mount, motion activated light over front entry. Use “dual-bright” dusk to dawn type light in white. Suggested model #SH-4300-WH or equivalent. Provide and install all LED, dimmable bulbs.

16) Provide and install new exterior rated, LED “Wall-Pack” or similar, security light on rear of the home, in place of non-functional wall mounted security light. This shall be Dusk-to-Dawn/Motion.

17) If needed, provide and install code approved connection to new water heater. (Optional Item, Section #13)

4 HVAC Replacement

1) This work Will Require a Mechanical Permit. 2) Remove and properly dispose of existing HVAC System, Air Handler

and Condenser Units as well as old condensate line and condensate pump, if present. Remove and dispose of old plenum box.

3) Remove and dispose of ALL old ducts AND Provide and install new,

code compliant HVAC duct work (if ducts are damaged or if otherwise required), supply and return grills as needed. It is the responsibility of the contractor to make any necessary ceiling repairs (patch and paint), as a result of this work.

4) Insure all ducts are completely sealed with tape and mastic. 5) Provide and install a new, Code compliant 14SEER/better, Heat Pump

HVAC System, with manufacturer approved/compatible, illuminated, digital thermostat.

6) Insure all Electrical wiring, disconnects and breakers are sized and located correctly for the new equipment and per code. Coordinate with electrician as needed.

7) Provide and Install all new copper lines, pipe insulation as required by code and new thermostat wires for this system

8) Provide and install a new Plenum Base for HVAC unit as required and insure proper and fully sealed connection to return ducts (if installed) as needed.

9) Provide and install new 1-inch pleated filter, minimum of MERV-10 Rated or better. (Provide owner with a minimum of one additional replacement filter)

10) Provide and install new condensate lines. Insure condensate line drain is a minimum 12” off the side of the home and Provide and install “wet/float” style cut-off switch in drain side of HVAC Unit. If needed, provide and install new condensate pump and securely plumb and insulate to exterior of the home per code.

11) IF needed, cut-in new attic scuttle and provide and install a new sealed scuttle cover upon completion. Cover must be trimmed and painted to match surrounding ceiling finish.

12) If required by code or for permit, obtain and provide Energy Calculations and Manual D & J Reports. Per new Energy Calculations and Manuals D & J, make all necessary accommodations to properlysupply HVAC to all rooms of the home. Any alterations or renovations or repairs required in the home to complete HVAC installation and modification, will be considered included in this bid.

13) Provide Home Owner with Written HVAC Warranty and instructions on HVAC and thermostat use, proper care of system, filter size and maintenance intervals, and include brand, model and serial numbers of each piece of equipment and a copy of the Energy Calculations (if needed). Insure all equipment has been properly registered and that all warranty registrations, paperwork or documents have been completed and provided to the owner. Provide copy of same information (Not including full operator’s manual(s)) to Rehab Specialist at Final.

14) Provide AHRI Certificate for newly installed unit to the owner and a copy of same to Rehab Inspector, upon installation of HVAC System.

15) NOTE: It is the responsibility of the contractor to schedule any/all necessary/required inspections.

5 Insulation Upgrade

1) Provide and install new or upgraded insulation throughout 100% of the attic cavity over conditioned spaces, to bring up to minimum (R-30) or greater. Insulation may be fiberglass batt-type or “blown-in.”

2) Provide and install, if needed, new gasket seal around attic access.

3) Provide all required depth gauges throughout attic per code. 4) Following completion of attic insulation, provide a certificate, specific

to this project address/owner, indicating the Brand, type, depth and R-value and the total square footage covered for the home to the owner and Rehab Specialist.

6 Ceiling Repairs – Laundry Room

1) Remove and dispose of ceiling panels and all damaged ceiling drywall in the Laundry Room area.

2) Remove and properly dispose of old ceiling insulation, if any is present.

3) Confirm repairs to all structural framing members following Re-Roof work.

4) Provide and install new fiberglass Batt-style (Highest reasonable R-value) insulation in the open ceiling cavity.

5) Provide and install new ½” Ceiling drywall (minimum) materials, tape and bed all joints and apply new finish texture to 100% of ceiling. (smooth, orange peel, of light “skip-trowel” texture preferred)

6) Provide and apply “Kilz-type” mold and mildew resistant primer to ceiling.

7) Provide and apply at least one “Full-Coverage” coat of Sherwin-Williams, Flat White Ceiling Paint to new ceilings, wall to wall.

8) If necessary, provide and install new white, synthetic trim/mouldings around perimeter of laundry room to provide a complete and finished appearance.

7 Electric Range and Vent Hood

1) Remove and properly dispose of old Electric Range and old vent hood.

2) Provide and install new 30-inch Electric Range and appropriate power cord.

3) Range is to be white, smooth-top, self-cleaning oven with a glass window in the door. Suggested model is Whirlpool #WFE5050WOHW, or substantially equal.

10 OPTIONAL - Handicap Ramp and Railings – Rear

Entry

1) This work requires a Building permit. 2) Provide all material, labor, permits and subcontracts necessary to

construct a new ADA-style Handicap Ramp and Railings.3) NOTE: Provide and install a protective flashing or moisture/vapor

barrier between the house and the concrete landing. 4) Provide and install a new concrete landing, from the block wall to the

south, to approximately 12-inches beyond the North side of the rearentry door and approximately 48-inches out, away from rear entry door. Landing may be constructed on top of the existing materials, if stable/suitable, up to the elevation of the interior floor at rear entry.

5) Provide and install a new concrete ADA-type Ramp. The ramp is to be a minimum of 48-inches wide and extend out from the Rear Entry

Landing in line with the door, to the East, as far as necessary to achieve proper slope. There shall be a flat landing at the bottom of the sloped ramp, at grade level. The landing the same width as the ramp and shall be a minimum of 60-inches in length.

6) The slope of the ramp must not exceed 1:12 at any location and if the direction of slope changes, a level landing surface must be between these changes. Upper landing is to be level with the interior floor at the rear door but have enough fall away from the home to allow water to run off. Lower landing shall be even with existing grade

7) All landings and ramps shall have railings constructed on all sides.

8) Provide and install a secure Handrail (Aluminum), extending from the rear entry door, to a point no less than 1 foot beyond the sloped area of the ramp.

9) All railings along ramps shall have barriers no greater than 4” above the top edge of the ramp surface or shall have a curb installed upon the top surface of the ramp, with a minimum width between curbs not less than that of the railings above and at a minimum vertical projection of 1-1/2”.

10) In addition, a rail shall be installed at the median point, vertically, between the lower barrier or curb and upper railing. This rail shall be constructed of aluminum materials placed with the 1-1/2” horizontal projection and running parallel with the upper railing.

11) All grab surfaces (Railings, etc.) are to be free of sharp edges or rough surfaces.

12) All sloped ramp surfaces shall be finished with a non-slip textured finish (“Light Broom-style” or similar) or shall have Non-skid materials applied directly to the surface, to help prevent slipping when moisture may be present. This may be painted on, tape applied, or mechanically fastened materials, if not incorporated into concrete work.

13) Any drawings, sketches or other documents or materials required for permitting or construction are the responsibility of the contractor.

14) The general design requirements shall be reviewed at the Pre-bid meeting.

11OPTIONAL - Water Heater Replacement (Under

Counter) 1) This work WILL require a Plumbing permit, if included.

2) Remove and properly dispose of existing water heater under kitchen cabinets.

3) Remove and dispose of existing plumbing stop(s) (if any) on supply to water heater.

4) Provide and install all necessary plumbing materials and piping needed to install a new water heater unit in same location.

5) Provide and install new ¼-turn brass ball valve stop, per code. 6) Provide and install new drain pan and plumb to exterior per code. 7) Provide and install new 28/30 gal. +/- capacity, tank-style, electric

water heater, with 4500/4500-Watt elements, to go back under the

counter in same location as previous unit.8) If new electrical hookup is needed, it is the responsibility of the

contractor/electrical contractor to provide this and it shall be considered to be included in this bid.

9) Provide all owners manuals and warranty info to owner, and a copy of face pages/warranty info to Rehab Specialist, at Rehab Final Inspection.

12 Optional – Exterior Pressure Wash and Painting

Note: Lead Based Paint Detected on Some Surfaces, Contractor responsible to verify all information.

1) Carefully pressure wash/clean/prep all exterior walls of the home, including gables, for new primer and paint.

2) Use “Big Stretch” or Equal/Better water based elastomeric caulk to fill all cracks and small gaps around home. Make any other repairs to larger holes/damage around the perimeter of the home as needed, before painting.

3) Provide and apply at least one “Full-Coverage Coat” of Sherwin Williams “PrimeRx Peel Bonding Primer” OR “Extreme Bond Primer” OR Equal/Better (Provide MSDS Sheets for any requested alternate –MUST be pre-approved by Rehab Inspector) to all exterior walls and gables and windows and doors, as appropriate.

4) Provide and apply a minimum of one “Full-Coverage Coat” of Sherwin Williams “SuperPaint Exterior Acrylic Latex” or Equal/Better (Provide MSDS Sheets for any requested alternate – MUST be pre-approved byRehab Inspector), additional coats if necessary to provide consistent color, texture and sheen, to all exterior walls, gables and windows and doors, as appropriate. Owner to select color(s) following contract signing and it must also be approved by the Rehab Inspector. Only lighter color(s) may be selected for the body of the home. Trim colormay be contrasting.

13 PERMITS

1) It is the responsibility of the Contractor and/or Subcontractors to obtain any/all required permits necessary to perform the work described above and to properly post/display them clearly at the job site.

2) It is also the responsibility of the Contractors and Sub Contractors to insure all required inspections are requested and passed, up to and including the Final inspections from the City of Ocala Building Department and the Community Development Services Department.
