North LincolnshireCommunity ChampionAwards • The awards are split into four categoriesVolunteers, Achievement, Community andInspiration.

• A young person is classed as 18 years oryounger on the 31 October 2015

• An adult is classed as 19 years or older on the 31 October 2015

• All people and organisations nominatedmust be based in North Lincolnshire.

• All individuals, groups and organisationsnominated must have been active within the last year 1 November 2014 – 31 October 2015.

• Please give specific examples of the work,achievements or commitment the person you are nominating has undertaken for thejudging panel to consider.

• All categories are open to disabled/nondisabled individuals and groups.

Achievement (sponsored by North Lincolnshire Council)

1 Anti Bullying Champion (sponsoredby North Lincolnshire Council)Nominate an individual of any age who has carried out work to challengebullying. For example they could havesupported someone who has beenbullied, challenged bullies or justworked hard to raise the awareness of bullying and the impact it can haveon people.

2 Personal Courage (sponsored byClugston Construction) Nominate anindividual of any age who has overcomepersonal challenges above and beyondthe normal to achieve their aims in life. The personal adversity they have had to overcome could be anything, forexample financial, physical or mentalhealth.

3 Sports Achiever of the Year (sponsored by Scunthorpe United F.C.) Nominate an individual to recognise their commitment andachievements in their chosen sport.They should have achieved a highstandard of performance for examplelocal, regional or national level.

4 Young Sports Achiever of the Year (sponsored by North LindseyCollege) Nominate a young person torecognise their commitment and achievements in their chosen sport. They

should have achieved a high standard ofperformance for example local, regionalor national level.

5 Young Arts Heritage Achiever of theYear (sponsored by John LeggottCollege) Nominate a young personwho has demonstrated commitmentand outstanding achievements in theirchosen art form(s), for example dance,drama, music, singing, archaeology,drawing and writing.

6 Adult Arts Heritage Achiever of the Year (sponsored by The BathsHall) Nominate an individual who has demonstrated commitment andoutstanding achievements in theirchosen art form(s), for example dance, drama, music, singing,archaeology, drawing and writing.

7 Outstanding Achievement (Young) (sponsored by North LindseyCollege) Nominate a young personwho has amazed you with a significantand outstanding achievement. Forexample raising large amounts ofmoney for charity, representing theircountry or a significant academicachievement.

8 Outstanding Achievement (Adult)(sponsored by Quality SolicitorsBradbury Roberts & Raby) Nominatean individual who has amazed you witha significant and outstandingachievement. For example raising largeamounts of money for charity orrepresenting their country.

4 Outstanding Contribution to aStronger Community (sponsoredby Humberside Fire & RescueService) Nominate a group orindividual of any age that makes yourcommunity a vibrant, lively, exciting andstimulating place to live. They mayprovide festivals or events where peoplecan meet and socialise. Tell us how theircontribution has had a positive impacton the community.

5 Community Contribution to CrimeReduction (sponsored byHumberside Police) An individual orgroup who has contributed to crimereduction in their community. This couldbe through neighbourhood watch,positive or diversionary activities orsomething that has transformed thewell-being of the community .

6 Community Project of the Year(sponsored by GS Kelsey Ltd)Nominate a project that has builtcommunity spirit and improved thequality of life for local people. Forexample:

•�A support group run by volunteersthat has helped you and made youfeel worthwhile.

A luncheon club that has given you a support network.

• A youth group run by volunteersthat has helped you make friends and find new interests.

•�A project that has helped you to improve your health bysupporting you to exercise or eat healthier.
