Secretary of State Pedro A. Cortés announced that 153 disciplinary actions have been taken by the Bureau of Professional and Occupational Affairs’ 29 professional licensing boards, the Bureau of Charitable Organizations and the Division of Legislation and Notaries of the Bureau of Commission, Elections, and Legislation:

PA Board of Certified Real Estate Appraisers Erie County Appraisal Institute of NWPA, appraiser education provider no. 003025 of Erie, Erie County, was ordered
to notify all attendees of a March 27, 2014, course entitled “PA Law, Regulations and Code 49 and Act
48” that the board's approval of the course had expired when the course was presented, requiring that the
course be re-offered at least one time prior to April 1, 2016; and requiring that any attendee of the March
27, 2014, Law Course be allowed to attend the substitute course free of charge, because they offered and
presented a continuing education course for which the Board's approval had expired. (02/25/16)
Lehigh County Donald W. Johnson, certificate no. RL138924, of Center Valley, Lehigh County, was ordered to pay a
civil penalty of $75 and for a period of twenty-four months shall be subject to non-random audit of
continuing education when/if he files any application for the renewal or issuance of a new authorization to
practice the profession, because he completed only twenty-six and one-half of the required twenty-eight
hours of acceptable continuing education necessary for the renewal of his certificate. (02/25/16)
Lycoming County Suzanne M. Fahien, certificate nos. RL003671 and AV001021L, of Montoursville, Lycoming County,
was ordered to pay a $250 civil penalty and be subject to a non-random audit of her continuing education
obligations over a twenty-four (24) month period, when/if the during said period, the board receives an
application for a renewal or issuance of a new authorization to practice the profession, because she failed
to meet the minimum qualifications for renewal of her certificates, by not successfully completing all of
the continuing education requirements as set forth by the board. (02/25/16)
Out of State Dennis James Carroll, certificate no. RL139980, of Cossayuna, New York was ordered to pay a civil
penalty of $250 and for a period of twenty-four months he shall be subject to non-random audit of his
continuing education when/if he files any application for the renewal or issuance of a new authorization to
practice the profession, because he failed to meet the minimum qualifications for renewal of his
certificates because he failed to successfully attend and complete the two hour LAW course as required.

Monroe County Chop Shop Kutz, license no. CB124006, of Stroudsburg, Monroe County, was ordered to pay a $1,250
civil penalty and to cease and desist from employing unlicensed employees in the practice of
cosmetology, aiding and abetting unlicensed practice activity in the profession of cosmetology, failing to
have an owner or designated licensee present during inspection of its salon, and failing to maintain all
areas of the salon in an orderly and sanitary condition because they operated as a cosmetology salon in a
grossly incompetent and unethical manner by employing unlicensed employees, aiding and abetting
unlicensed individuals in the profession of cosmetology, failing to have an owner or designated licensee
in charge present during inspection, failing to keep and maintain all areas of the salon in an orderly and
sanitary condition, and failing to have adequate lavatories on the premises. (03/07/16)
Philadelphia County Ramona Searles t/d/b/a Salon R S, license no. CB091381L, of Philadelphia, Philadelphia County, was
ordered to pay a $500 civil penalty and to cease and desist from operating as a cosmetology salon on a
license that is lapsed, expired, or otherwise not current with the board because she used or maintained any
place for the practice of cosmetology without a current and active license. (03/07/16)
Desiree A. Oglesby t/d/b/a Diadem, license no. CB116172, of Philadelphia, Philadelphia County, was
ordered to pay a $500 civil penalty and to cease and desist from operating as a cosmetology salon with a
license that is lapsed, expired, or otherwise not current with the board because she used or maintained any
place for the practice of cosmetology without a current and active license (03/07/16)
York County Harrison R. Hackworth, license no. CO228549L, of York, York County, was revoked because he engaged
in grossly incompetent and unethical conduct by engaging in criminal activities at or around his

guilty of a crime or misdemeanor involving moral turpitude or was found guilty of a felony in violation of

Out of State Christopher Keegan, license no. DS023602L, of Bayport, New York, voluntarily surrendered his license
to practice dentistry in the Commonwealth of Pennsylvania because he had a license to practice dentistry
revoked, suspended, or had other disciplinary action imposed or consented to, by the proper licensing
authority of another state, and had failed to report said action to Commonwealth authorities on a biennial
renewal application, or within ninety (90) days of a final disposition, whichever was sooner. (03/18/16)
Thomas R. Cherpak, license no. DS023414L, of North Port, Florida, permanently and voluntarily
surrendered his license because he had a license to practice dentistry or dental hygiene or certificate for
expanded function dental assisting revoked, suspended or had other disciplinary action imposed or
consented to by the proper licensing authority of another state, territory or country or had an application
for license refused, revoked or suspended by the proper licensing authority of another state, territory or

Laurie A. Sargent, license no. DS025814L, of Lihue, Hawaii, was ordered to pay a $1,000 civil penalty,
had a public reprimand placed on her permanent disciplinary record with the board, and was ordered to
successfully complete remedial education credits as set forth by the board because she had a license to
practice dentistry revoked, suspended or had other disciplinary action imposed, or consented to, by the

device, title, card or any similar item implying that he was a professional engineer or competent to

Union County Hasan Askari, license no. MD436856, of Lewisburg, Union County, had a public reprimand placed on his
permanent disciplinary record with the board, was ordered to demonstrate compliance with the request for
records from DPW (now DHS), was ordered to attend and successfully complete at least six hours of
remedial education on the topic of medical record retention and management, and was ordered to pay a
civil penalty of $1,000 because he is guilty of immoral or unprofessional conduct, including the departure
from, or failing to conform to the ethical or quality standard of the medical profession. (03/15/16)
Out of State Mario Winn Katigbak, license no. MD434512, of Glastonbury, Connecticut, was ordered to pay a civil
penalty of $1,000 because he had a license or other authorization to practice the profession disciplined by

Anthony Vernon Dallas, Jr., license no. MD442784, of Hendersonville, Tennessee, had a public
reprimand placed on his permanent disciplinary record with the board because he had a license or other

Thomas Bartley Benz, license no. MD031201E, of Parkersburg, West Virginia, was indefinitely
suspended, with the suspension immediately stayed in favor of an indefinite period of probation subject to

and safety to patients by reason of illness or addiction to drugs or alcohol; and had a license to practice the

year, with such suspension immediately stayed in favor of probation for a period of at least one year,

commission suspended for a period of six (6) months, with such suspension immediately stayed in favor
of probation, ordered to attend and successfully complete a six-hour notary practice and procedure course
approved by the Secretary or his designee, and was ordered to pay a civil penalty of $250 and a probation
assessment of $300 for the costs of continued monitoring because he failed to keep and maintain custody
of an accurate chronological register of all official notary acts and failed to be familiar with the duties and

Philadelphia County Tracey M. Anderson, notary commission no. 1206854, of Philadelphia, Philadelphia County, permanently
and voluntarily surrendered her notary commission because she failed to maintain custody and control of

Berks County Connie Sue Linebarger, license no. PN256171L, of Morgantown, Berks County, was ordered to pay a
civil penalty of $500 and a public reprimand was placed on her permanent disciplinary record with the

Blair County Heather A. Panek, license no. PN283334, of Tyrone, Blair County, was automatically suspended
retroactive to August 19, 2015, for misdemeanor convictions under the Drug Act. (09/24/15)
Bucks County Data Godgift Ogolo, license nos. RN678687 and PN263988, of Yardley, Bucks County, was ordered to
pay a $750 civil penalty and have her licenses to practice professional and practical nursing in the
Commonwealth of Pennsylvania suspended for two (2) years, with said suspension immediately stayed in
favor of probation, subject to the board's terms and conditions, because she has proven to be unfit or
incompetent by reason of negligence, habits or other causes, thereby being guilty of immoral or

Joseph Charles Kelly, license no. PN295794, of Warrington, Bucks County, agreed to the voluntary
surrender of his license because he is addicted to alcohol or is addicted to hallucinogenic or narcotic drugs

because she was unable to practice nursing with reasonable skill and safety to patients by reason of

Cambria County Roseann V. Ricks, license no. PN275188, of Johnstown, Cambria County, was automatically suspended

voluntary surrender of her license because she is unable to practice professional nursing with reasonable
skill and safety to patients by reason of mental or physical illness or condition or physiological or
psychological dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs which tend to

Matthew John Ryan, license no. RN635365, of Phoenixville, Chester County, was automatically
suspended for one year and indefinitely suspended for no less than three years to run concurrently, with
such suspensions to be immediately stayed in favor of no less than three years of probation, subject to the
board's terms and conditions, because he is unable to practice the profession with reasonable skill and
safety to patients by reason of mental or physical illness or condition or physiological or psychological

actively suspended until such time as the board grants a reinstatement of her license, subject to the board's
terms and conditions, because she was convicted or pled guilty or entered a plea of nolo contendere, or
was found guilty by a judge or jury of a felony, or a crime of moral turpitude, or received probation
without verdict, disposition in lieu of trial, or an Accelerated Rehabilitative Disposition of felony charges.

suspended for one year and indefinitely suspended for no less than three years retroactive to February 23,
2015 to run concurrently, with such suspensions being immediately stayed in favor of no less than three
years of probation retroactive to February 23, 2015, subject to the board’s terms and conditions, because
she is unable to practice the profession with reasonable skill and safety to patients by reason of mental or
physical illness or condition or physiological or psychological dependence upon alcohol, hallucinogenic
