PA Board of Accountancy Allegheny County Constantine J. Panagulias, certificate and license no. CA036098L, of Pittsburgh, Allegheny County, had a

public reprimand placed on his permanent disciplinary record with the Board and paid a civil penalty of

$9,750 and costs of investigation of $423, because he engaged in the practice of public accounting during

a period in which he did not hold a license. (01/16/18)

Bucks County Roy Brito, certificate and license no. CA047427, of Newtown, Bucks County, had a public reprimand

placed on his permanent disciplinary record with the Board and paid a civil penalty or $4,650 and costs of

investigation of $234.59, because he engaged in the practice of public accounting in the Commonwealth

during a period in which he did not hold a current license. (01/16/18)

Montgomery County Joel Robert Epstein, certificate and license no. CA015247L, of Huntingdon Valley, Montgomery County,

was ordered to pay a civil penalty of $750 and had a public reprimand placed on his permanent

disciplinary record with the board, because he engaged in the practice of public accounting in the

Commonwealth when his license was expired and not active between January 1, 2016 and January 31,

pursuant to the Order of the Court of Common Pleas of Allegheny County dated September 11, 2017,

which the court issued under section 4355 of the Domestic Relations Code. The suspension is effective

Cumberland County Harvey Legree, license no. BL055229, of Mechanicsburg, Cumberland County, was suspended pursuant

to the Order of the Court of Common Pleas of Chester County dated October 10, 2017, which the court

penalty of five thousand dollars ($5,000.00), to pay an assessment for the costs of investigation in the

amount of one thousand dollars ($1,000.00), and to attend and successfully complete at least thirty-three

(33) hours of remedial education, because she failed to complete an appraisal assignment within the

requirements of the USAP, failed or refused to exercise reasonable diligence in developing an appraisal,

preparing an appraisal report, or communicating an appraisal, and negligently or incompetently developed

an appraisal, prepared an appraisal report, or communicated an appraisal. (10/26/17)

Montgomery County Harvey Louis Perelman, certificate no. RL001099L, of Huntingdon Valley, Montgomery County, was

ordered to pay a civil penalty of three thousand dollars ($3,000.00), an assessment for the costs of

investigation in the amount of one thousand dollars ($1,000.00), and to attend and successfully complete

at least twenty-two (22) total hours of remedial education, because he failed to complete an appraisal

assignment within the requirements of the USPAP, failed or refused to exercise reasonable diligence in

Lackawanna County Ingris Abrey d/b/a Image Y Style Dominican Hair Salon, license no. CB123316, of Scranton,

Lackawanna County, was indefinitely suspended, was assessed a $2,350 civil penalty and costs of

investigation in the amount of $34.17, based upon employing unlicensed individuals, aided and abetted in

the unlicensed practice of the profession, failed to have required equipment, failure to properly sanitize

equipment and was not managed by the owner or a licensed designee. (09/20/17)

PA Board of Dentistry Allegheny County Boris Karlusic, certificate no. DF003016, of Pittsburgh, Allegheny County, was suspended and assessed a $1500 civil penalty based on failing to submit proof of continuing education. (11/22/17)

Charles G. Hatfield, license no. DS017179L, of Clairton, Allegheny County, was reinstated and placed on

probation for a period of five years, subject to the board's terms and conditions, because he violated a

lawful order of the Board previously entered by the Board in a disciplinary proceeding. (01/19/18)

John M. Holovak, license no. DS019018L, of Bridgeville, Allegheny County, was ordered to pay a civil

penalty of $1000, investigation costs of $250, and a public reprimand placed on his permanent

disciplinary record with the Board, because he violated Board law by knowingly or negligently employing

a dental hygienist whose license or certificate was not current. (01/19/18)

placed on her permanent disciplinary record with the Board, paid a civil penalty of $1,000 and costs of

investigation of $5,801.45, and must submit successful proof of completion of ten hours of remedial

education within six months of the acceptance of the Consent Agreement because she engaged in

unprofessional conduct by failing to conform to the accepted standard of care regarding dental

Franklin County Cindy Ann Myers, license no. DH008546L, of Fayetteville, Franklin County, was ordered to pay a civil

penalty of $1,300 and a public reprimand placed on her permanent disciplinary record with the Board,

because she violated Board’s law by practicing as a dental hygienist or holding herself out as a

practitioner of or entitled or authorized to practice as a dental hygienist, or assuming any title of ‘dental

hygienist’ or other letters or titles in connection with her name which in any way represent her as being

engaged in the practice as a dental hygienist, or authorized so to do, without being duly licensed and

authorized to engage in such practice under the provisions of the Act. (01/19/18)

Montgomery County Nicole Baruffi, license nos. DH072753 and DHA003011, of North Wales, Montgomery County, was

indefinitely suspended retroactive to January 20, 2017 for failing to comply with a previous agreement

entered into with the Commonwealth. (11/08/17)

Mark J. Mele, license no. DS027434L, of Glenside, Montgomery County, was ordered to pay a civil

penalty of $500 and a public reprimand placed on his permanent disciplinary record with the Board,

because he violated board's law in that he had a license to practice dentistry in another state and had other

disciplinary action consented to by the proper licensing authority of that state. (01/19/18)

Northampton County Daniel J. Milavec, license no. DS024078L, of Northampton, Northampton County, was ordered to pay a

civil penalty of $2900 and a public reprimand place on his permanent disciplinary record with the Board,

because he practiced dentistry or held himself out as a practitioner of or entitled or authorized to practice

dentistry while his license was not renewed. (01/19/18)

Philadelphia County Susan A. Fox, license no. DS037400, of Philadelphia, Philadelphia County, was indefinitely suspended

for no less than three years immediately stayed in favor of probation for no less than three years for

violating a previous agreement entered into with the Commonwealth (11/30/17)

Westmoreland County Gregory Scott Bellotti, license no. DS029395L, of Irwin, Westmoreland County, paid the costs of

investigation of $6,631.75 and was indefinitely suspended for no less than three years, with such

suspension immediately stayed in favor of no less than three years of probation, subject to the Board’s

terms and conditions, because he is unable to practice dentistry with reasonable skill and safety to patients

by reason of illness, drunkenness, excessive use of controlled substances, chemicals or any other type of

material, or as the result of any mental or physical condition. The Order of Immediate Temporary

Suspension at file 17-46-03717 entered July 28, 2017 and all legal proceedings attached thereto are closed

and made final and superseded by the above-referenced Order. (01/19/18)

file no 10-46-04032 because he had a license to practice dentistry or dental hygiene or certificate for

expanded function dental assisting revoked, suspended or other disciplinary action imposed or consented

to by the proper licensing authority of another state, territory or country or had an application for license

refused, revoked or suspended by the proper licensing authority of another state, territory or country.

PA Board of Professional Engineers, Land Surveyors and Geologists Montgomery County Michael Damian Engel, license and registration no. PE073746, of Pottstown, Montgomery County, paid a

civil penalty of $500 and costs of investigation of $406.40, because he was guilty of misconduct in the

practice of engineering by practicing the profession of engineering without holding a current license and a

current registration in the Commonwealth. (01/10/18)

Out of State Robert Vivak Nangia, PA license and registration no. PE076619, of Cypress, Texas, had a public

reprimand placed on his permanent disciplinary record with the Board and was ordered to pay a civil

penalty of $1,000, because he was guilty of misconduct in the practice of engineering by having had

disciplinary action taken against a license to practice engineering for conduct relating to the practice of

engineering, by the proper licensing authority of another state. (01/10/18)

Danilo R Salameda, PA license and registration no. PE045167E, of Wilmington, Delaware, was ordered

to pay a civil penalty of $6,000, costs of investigation of $2,858.82, placed on probation for a period of at

least six months, and must submit proof of completion of 30 hours of ethics education prior to the

conclusion of the probation period, subject to the Board's terms and conditions, because he was guilty of

violating the code of ethics of the engineering profession by failing to act for his employer in professional

matters as a faithful agent or trustee and by accepting remuneration other than his stated recompense for

PA Board of Landscape Architects Out of State John Morgan Thomas, PA license no. LA000741E, of Lambertville, New Jersey, was reprimanded and

assessed a $1000 civil penalty based on findings he failed, refused or neglected to renew his certificate of

licensure within such time as the Board prescribes. (11/21/17)

PA Board of Massage Therapy

Philadelphia County Xiukun Gao, of Philadelphia, Philadelphia County, was denied a massage therapist license based on

previous criminal history. (10/27/17)

PA Board of Medicine Allegheny County Christopher A. Lindberg, license no. MA001572L, of Wexford, Allegheny County, was automatically

suspended for ten years based on felony convictions under the Drug Act. (11/13/17)

Cambria County Johnnie Wilson Barto, license no. MD015619E, of Johnstown, Cambria County, had his license to

practice as a Medical Physician and Surgeon temporarily suspended, pending a hearing, on the grounds

that his continued practice within the Commonwealth may be an immediate and clear danger to the public

health and safety. (01/22/18)

Clearfield County Henry G. Dela Torre, license nos. MD027890E and PAK000027, of DuBois, Clearfield County, had his

licenses to practice as a Medical Physician and Surgeon and Physician Acupuncturist temporarily

suspended, pending a hearing, on the grounds that his continued practice within the Commonwealth may

be an immediate and clear danger to the public health and safety. (01/22/18)

Holly J. Hipps, license no. MA051098, of Olanta, Clearfield County, had her license to practice as a

medical physician assistant temporarily suspended pending a hearing, on the grounds that her continued

practice within the Commonwealth may be an immediate and clear danger to the public health and safety.

Crawford County Luis L. Gomez, license no. MD018911Y, of Meadville, Crawford County, was ordered to pay a civil

penalty of $3,000.00, and a public reprimand was placed on his permanent disciplinary record with the

board, because he practiced on a lapsed license; practiced fraud or deceit in obtaining a license; and failed

to maintain the appropriate amount of professional liability insurance. (12/19/17)

practice as a medical physician and surgeon temporarily suspended, pending a hearing, on the grounds

that his continued practice within the Commonwealth may be an immediate and clear danger to the public

Montgomery County Highland Richard Campbell, license no. MD422335, of Mont Clare, Montgomery County, was

indefinitely suspended, because he is unable to practice the profession with reasonable skill and safety to

patients by reason of illness, addiction to drugs or alcohol, or mental incompetence. (12/19/17)

Philadelphia County Jessica L. Carroll, license no. MA003418L, of Philadelphia, Philadelphia County, was indefinitely

suspended for no less than three (3) years, such suspension to be immediately stayed in favor of no less

than three (3) years of probation, because she violated the law by receiving disposition in lieu of trial in

the disposition of felony charges in a court of another state. (12/19/17)

Ayesha M. Ferguson, license no. BH000483, of Philadelphia, Philadelphia County, was levied a civil

penalty of three thousand dollars, had a public reprimand placed on her permanent disciplinary record

with the board, and was ordered to attend and successfully complete at least twelve hours of remedial

education, because she received Accelerated Rehabilitative Disposition in the disposition of felony

charges in the courts of this Commonwealth. (12/19/17)

Out of State Thomas D’Amato, PA license no. MD435514, of Bayonne, New Jersey, was reprimanded based on

disciplinary action taken by the proper licensing authority of another state. (11/13/17) William V. Craig, PA license no. MD456530, of Pahrump, Nebraska, was reprimanded and issued a

$1000 civil penalty based on disciplinary action taken by the proper licensing authority of another state.

Christopher Stephen Litts, PA license no. MD061207L, of Plantation, Florida, was indefinitely suspended

$5000 civil penalty based on disciplinary action taken by the proper licensing authority of another state.

reprimanded, because he had a license or other authorization to practice the profession suspended,

Secretary of the Commonwealth – Notaries Public

Crawford County Robin Anne Thompson, notary commission no. 1215966, of Titusville, Crawford County, was revoked,

based on findings she failed to know through personal knowledge or have satisfactory evidence that the

person appearing before her as the person described in and who was executing the instrument, failed to

keep and maintain custody and control of an accurate chronological register of all official acts done by

virtue of Respondent’s notary commission, failed to be familiar with the duties of a notary public and was

not of sufficient ability to hold the office of notary public. (1-24-2018)

Montgomery County Alex Garzon, notary commission no. 1264834, of Norristown, Montgomery County, agreed to the

suspension of his notary commission for three months, such suspension immediately stayed in favor of

probation, to successfully complete a six-hour notary practice and procedure course, and to pay a civil

penalty in the amount of $250 and probation assessment fees in the amount of $150 because he failed to

be familiar with the duties and responsibilities of a notary public by engaging in incomplete or improper

notarization by failing to include the essential notarization language and failing to maintain an accurate

notary register by failing to record the character of the act, notary fee charged, and date of the instrument;

failed to maintain an accurate notary register; and failed to be of good character, integrity and ability by

engaging in incomplete or improper notarization by failing to include the essential notarization language

and failing to maintain an accurate notary register by failing to record the character of the act, notary fee

Philadelphia County Joaquin Rivera, notary commission no. 1138546, of Philadelphia, Philadelphia County, was suspended for
