Butler County Scott M. Karner, certificate no. RL139081, of Cranberry Township, Butler County, was ordered to pay a

civil penalty of $5,000 and an assessment for the cost of investigation of $1,380 and attend and

successfully complete at least 45 hours of remedial education because he failed to complete an appraisal

assignment within the requirements of the USPAP; failed or refused to exercise reasonable diligence in

developing an appraisal, preparing an appraisal report, or communicating an appraisal; and negligently or

incompetently developed an appraisal, prepared an appraisal report, or communicated an appraisal.

Somerset County David James Murphy, certificate nos. GA000700L and RL000035L, of Berlin, Somerset County, had his

certificates to practice as a certified general and residential real estate appraiser in the Commonwealth of

Pennsylvania actively and indefinitely suspended for a minimum period of one (1) year, after which he

may apply for reinstatement, to be subject to the Board's terms and conditions, because he had been

convicted of a felony. (02/25/16)

Out of State Novo Appraisal Management Corporation, registration no. AMC000007, of Atlanta, Georgia, was

ordered to pay an assessment for the costs of investigation in the amount of $10,000 because it was the

subject of disciplinary actions by an agency of another jurisdiction responsible for regulating appraisers,

mortgage brokers, real estate salespersons or real estate brokers. (02/25/16)

PA Board of Chiropractic

Allegheny County Charles H. Mehnert, license no. DC009306, of Pittsburgh, Allegheny County, was ordered to pay a civil

penalty of $1,000 because he had a license suspended, revoked or refused or received other disciplinary

Montgomery County Ethel C. Harvey, license no. DC006838L, of North Wales, Montgomery County, was indefinitely

suspended but said suspension is stayed in favor of indefinite probation subject to terms and conditions

and a public reprimand was placed on her permanent disciplinary record with the board because she was

convicted of a felony, a misdemeanor in the practice of chiropractic, or received probation without

verdict, disposition in lieu of trial or an Accelerated Rehabilitative Disposition in the disposition of felony

charges, in the courts of this Commonwealth, a Federal courts, or a court of any other state, territory,

Philadelphia County David M. Gladstone, license no. DC010590, of Philadelphia, Philadelphia County, had a public reprimand

placed on his permanent disciplinary record with the board and was ordered to pay a civil penalty in the

amount of $1,000 because he had a license to practice chiropractic suspended, revoked, or refused or

received other disciplinary action by the proper chiropractic licensed authority of another state, territory,

PA Board of Cosmetology

Allegheny County LA Nails, license no. CY195534, of Pittsburgh, Allegheny County, was suspended for failing to pay a

previously imposed civil penalty. (11/04/15)

Genilee Schultz, of Pittsburgh, Allegheny County, was denied licensure due to lacking good moral

character. (11/19/15)

Lackawanna County Veronica Smith, license no. CO254869, of Moscow, Lackawanna County, was suspended for failing to

pay a previously imposed civil penalty. (12/07/15)

Union County Brett I. Bittner, license no. CO264252, of Lewisburg, Union County was suspended pursuant to the Order

of the Court of Common Pleas of Union County dated December 14, 2015, which the court issued under

section 4355 of the Domestic Relations Code. The suspension is effective immediately. (12/24/15)

York County Amanda Spillane, of York, York County, was denied licensure due to lacking good moral character.

a dental hygienist suspended, pending a hearing, on the grounds that her continued practice within the

Washington County Bryan L. Lawson, of Donora, Washington County, was granted a license as a funeral trainee and placed

on probation for one year due to his felony convictions. (12/09/15)

PA Board of Massage Therapy

Chester County Steven Lee t/d/b/a Floris Nails, unlicensed, of Exton, Chester County, was ordered to pay a civil penalty

of $10,000 and to cease and desist from hiring or employing any individual(s) to perform any activity or

service requiring a license to practice as a massage therapist or from performing any other activity which

requires a license, registration, certificate, authorization, approval or permit issued by the board unless

and until the board issues the appropriate authorization to practice the profession because he, while acting

as the owner/operator/responsible employee of Elise Spa and Nails, employed an unlicensed individual to

provide massage therapy services for clients of Floris Nails. (02/09/16)

Wei Min Zheng, unlicensed, of Exton, Chester County, was ordered to pay a civil penalty of $2,000 and

to cease and desist from holding himself out as a massage therapist or from engaging in the practice of

massage therapy or from performing any other activity which requires a license, registration, certificate,

authorization, approval or permit issued by the board because she practiced massage therapy without

Cumberland County Gai Li Shi, an unlicensed individual t/d/b/a Asian Spa. of Carlisle, Cumberland County, was ordered to

pay a civil penalty of $3,500 and costs of investigation of $987.87, to cease and desist from hiring or

employing any individual(s) to perform any activity or service requiring a license to practice as a massage

therapist in the Commonwealth of Pennsylvania or from performing any other activity which requires a

license, registration, certificate, authorization, approval or permit issued by the board unless and until the

board issues the appropriate authorization to practice the profession, and shall immediately remove from

its business name as well as from all advertisements describing its activities, including but not limited to

signage, brochures, business cards, price lists, telephone listings, and internet sites, the words “massage”,

“massage therapist”, “massage practitioner”, “masseur”, “myotherapist”, or any derivative of these terms

and their related abbreviations, which implies directly or indirectly that massage therapy services are

being provided because she, as responsible employee/officer of Asian Spa, utilized the word "massage" in

advertisements describing the services provided by Asian Spa which implied directly or indirectly that

massage therapy services were being provided even though the services of the business were not being

provided by licensees and that she utilized the services of unlicensed individuals as either employees or

independent contractors to provide massage therapy services for customers of Asian Spa. (02/09/16)

Lancaster County Robert W. Courtois, license no. MSG000343, of Landisville, Lancaster County, was revoked and assessed

a $5,000 civil penalty plus $208.40 for the cost of the investigation because he engaged in immoral or

unprofessional conduct during a professional relationship. (12/04/15)

Lehigh County Christopher C. Atanazy, license no. MSG007393, of Allentown, Lehigh County, had his license

temporarily suspended, pending a hearing, on the grounds that his continued practice within the

Commonwealth may be an immediate and clear danger to the public health and safety. (02/05/16)

Monroe County Leslie Monk, license number MSG004157, of Tannersville, Monroe County, was ordered to personally

and voluntarily surrender her license because she hired unlicensed individuals to practice massage therapy

and she was convicted of a crime of moral turpitude. (02/09/16)

Montgomery County Jong Uk Chae, unlicensed, of Wynnewood, Montgomery County, was ordered to pay a civil penalty of

$10,000, to cease and desist from holding himself out as a massage therapist or from engaging in the

practice of massage therapy or from performing any other activity which requires a license, registration,

certificate, authorization, approval or permit issued by the State Board of Massage Therapy, and agreed to

never apply for a license to practice massage therapy because he practiced massage therapy or held

himself out as a massage therapist without being licensed. (02/09/16)

Northumberland County Erica Nicole Haring, license no. MSG004157, of Bethlehem, Northampton County, was ordered to

permanently and voluntarily surrender her license because she engaged in immoral conduct, sexual

impropriety, sexual misconduct, and committed sexual violations. (02/09/16)

Philadelphia County Steven Lee, unlicensed, of Philadelphia, Philadelphia County, was ordered to pay a civil penalty of

$50,000 because he, as the owner/operator/responsible employee of Elise Spa and Nails, employed an

unlicensed individual to provide massage therapy services. (02/09/16)

Song Cha Rivera, of Philadelphia, Philadelphia County, was denied a massage therapist license based on

Berks County Bryan W. Hawk, license no. YM010037, of Fleetwood, Berks County, was indefinitely suspended for no

less than five years, such suspension to be immediately stayed in favor of no less than five years of

probation, subject to the board's terms and conditions, because he was unable to practice the profession

with reasonable skill and safety to patients by reason of illness, addiction to drugs or alcohol, or mental

Chester County Karen Marie Squire, license no. MD031820E, of West Chester, Chester County, was ordered to pay a

$500 civil penalty because she had a license to practice the profession disciplined by the proper licensing

Janet Lynn Baird, license no. MD428044, of Glenmoore, Chester County, was ordered to pay a $500 civil

penalty because she had continued to practice while her license or certificate was expired, not registered,

Cumberland County Peter Anthony Cardinal, license no. MD074264L, of Lemoyne, Cumberland County, was suspended

indefinitely but for no less than one year retroactive to February 3, 2015, and shall be immediately and

prospectively stayed in favor of indefinite probation subject to terms and conditions, because he departed

Erie County Matthew Gerald Verga, license no. YM014981, of Erie, Erie County, was ordered to pay a civil penalty of

$250 because he continued to practice while his license or certificate was expired. (02/02/16)

Fayette County Dominic W. Dileo, license no MD016685E, of Fayette County, had his license to practice as a medical

physician and surgeon temporarily suspended, pending a hearing, on the grounds that his continued

practice within the Commonwealth may be an immediate and clear danger to the public health and safety.

Franklin County Tunde Tolani Tijani, license no. MD056431L, of Greencastle, Franklin County, had his license to practice

as a medical physician and surgeon temporarily suspended, pending a hearing, on the grounds that his

Mifflin County Trista Lea Baker, license no. YM012351, of McClure, Mifflin County, was indefinitely suspended for no

practitioner with reasonable skill and safety to patients by reason of illness, addiction to hallucinogenic,

refused or otherwise disciplined by the proper licensing authority of another state, territory, possession or

Farrell Rondall Crouse, license no. MD026611L, of Pilesgrove, New Jersey, agreed to the permanent

voluntary surrender of his license because he had a license or other authorization to practice the

of the federal government and did not report any disciplinary action taken in such other jurisdiction on the

placed on his permanent disciplinary record with the board because he had a license or other authorization

assessed a $1,000 civil penalty, based on him receiving disciplinary action by the proper licensing

Sean Patrick Rhoads, PA license no. MA052146, of Martinsburg, West Virginia, was suspended for no

less than three years retroactive to October 26, 2011, based on his violation of a previous Board order.

Allegheny County Dawn M. Furman f.k.a. Klaas, notary commission no. 1197382, of Pittsburgh, Allegheny County,

permanently and voluntarily surrendered her notary commission because she failed to: be familiar with

the duties and responsibilities of a notary public; keep and maintain custody of an accurate chronological

register of all official notary acts; give notice of any address change to the Secretary of the

Commonwealth and the recorder of deeds within five days of such change; sign her name exactly and

only as it appears on the commission; and give notice of any name change to the Secretary of the

Commonwealth and the recorder of deeds within 30 days of such change. (02/29/16)

Lancaster County Dana R. Regan, notary commission no. 1121722, of Landisville, Lancaster County, had her notary

commission suspended for a period of three (3) months, such suspension to be immediately stayed in

favor of probation subject to certain terms and conditions, ordered to attend and successfully complete a

six-hour notary practice and procedure course approved by the Secretary or his designee, and pay a civil

penalty of $250 which will be waived upon successful completion of continuing education and a

probation assessment fee of $150 for the costs of continued monitoring because she had failed to maintain

an accurate chronological register of all official acts performed. (01/20/16)

PA Board of Nursing

Berks County Pamela A. Riegel, license no. RN515546L, of Mohrsville, Berks, was indefinitely suspended retroactive

to July 28, 2015, for violating a previous Board order. (10/06/15)

Blair County Phyllis Ruppel Halbig, license no. PN255329L, of Hollidaysburg, Blair County, agreed to the voluntary

surrender of her license to practice as a practical nurse in the Commonwealth of Pennsylvania because she

was convicted or has pleaded guilty or entered a plea of nolo contendere or has been found guilty by a

judge or jury of a felony or a crime of moral turpitude in the courts of Commonwealth, the United States,

or any other state, territory or country, or has received probation without verdict, disposition in lieu of

trial or an Accelerated Rehabilitative Disposition in the disposition of felony charges. (01/21/16)

Bucks County Eugenia Roytenberg, license no. RN568564, of Holland, Bucks County, was indefinitely suspended with

such suspension being immediately stayed in favor of indefinite probation to run concurrently with her

criminal probation and subject to the board's terms and conditions, and had a public reprimand placed on

her permanent disciplinary record with the board, because she was convicted, or entered a plea of guilty,

or entered a plea of nolo contendere, or has been found guilty by a judge or jury, of a felony or a crime of

moral turpitude, or has received probation without verdict, disposition in lieu of trial or an Accelerated

Rehabilitative Disposition in the disposition of felony charges, in the courts of this Commonwealth, the

United States or any other state; and because she has been convicted of any felony. (01/21/16)

Butler County Jennifer E. West, license no. RN538138, of Zelienople, Butler County, was indefinitely suspended for no

less than three years, such suspension immediately stayed in favor of no less than three years of probation

subject to the terms and conditions of the board because she is unable to practice nursing with reasonable

skill and safety to patients by reason of physical or mental illness or condition or physiological or

psychological dependence upon drugs or alcohol; and because she has violated a lawful disciplinary order

Centre County Trudi K. Zeigler, license no. RN506481L, of State College, Centre County, was suspended for no less

than three years retroactive to May 12, 2015, for violating a previous Board order. (07/27/15)

Dauphin County Tina Marie Pak, license no. RN603638, of Harrisburg, Dauphin County, was placed on probation for no

less than three years retroactive to February 26, 2013, because she violated a previous Board order.

Delaware County Laura J. Owens, license no. RN506984L, of Newtown Square, Delaware County, was indefinitely

suspended retroactive to March 11, 2015, and assessed a $500 civil penalty, based on being disciplined by

the Florida Board of Nursing and failing to report same to the Board. (09/14/15)

Joanna K. Savercool, license no. PN100485L of Upland, Delaware County was indefinitely suspended

retroactive to March 30, 2015, based on findings she is unable to practice with reasonable skill and safety

to patients by reason of mental or physical illness or condition or physiological dependence upon alcohol,
